Incident at HAMILTON

Summary

1996-09-03: C208 CARAVAN SIMO AIR LTD C-FESJ WAS ABOUT TO BEGIN A SCHEDULED COURIER FLIGHT AND HAD JUST BEGUN TO TAXI WHEN THE ENGINE DECELERATED TO ABOUT 30% NG. THE ENGINE CONTINUED TO OPERATE WITH ITT INDICATING 500 DEGREES CELCIUS . THE ENGINE WAS SHUT DOWN AND EXAMINED BY COMPANY MAINTENANCE. NO DISCREPANCIES WERE FOUND OTHER THAN THE FACT THAT THE ENGINE WOULD NOT ACCELERATE PAST 27% NG WHEN A RESTART WAS ATTEMPTED. ABOUT ONE GALLON OF FUEL WAS SAMPLED FROM THE A/C FUEL SYSTEM. IT WAS FOUND TO CONTAIN ABOUT ONE INCH OF AN AMBER COLOURED CONTAMINANT. A PRELIMINARY EXAMINATION REVEALED THAT IT WAS A HIGH CONCENTRATION OF PRIST. SOURCE UNKNOWN NOT FROM FUEL SUPPLIERS. FROM A TSB INITIAL NOTIFICATION. TIME = 0230Z. ACTION = PAX PAH PAE PKOO.
