Incident at KIRKLAND LAKE

Summary

1996-03-18: DHC6 TWIN OTTER C-GGVX WHILE IN CRUISE FLIGHT THE CREW NOTICED A SLIGHT REDUCTION IN POWER IN THE #2 ENGINE AND REDUCED POWER IN THE #1 ENGINE TO CORRESPOND WITH IT. DURING THE LANDING FLARE THE POWER LEVERS WERE REDUCED TO IDLE AND THE #2 ENGINE LOST ALL POWER AND STOPPED. AFTER MAINTENANCE ATTENDED TO THE AIRCRAFT THE A/C WAS ON A NON REVENUE FLIGHT WHEN THE #2 ENGINE LOST ALL POWER AND STOPPED. FURTHER EXAMINATION REVEALED THAT THE #2 ENGINE AIRFRAME FUEL FILTER WAS CONTAMINATED WITH ICE WHICH RESULTED IN FUEL STARVATION. AN EXAMINATION OF #1 ENGINE REVEALED A SMALL AMOUNT OF ICE CONTAMINATION. NO EVIDENCE OF FUEL CONTAMINATION. FROM AN TSB INITIAL NOTIFICATION. TIME = 0145Z. ACTION = PAX PAH.
