1993-06-26: DHC8 ONT463 + DC9 ACA460 - ONT463 DEPARTED SOUTHBOND FOR CLEVELAND FROM RWY 24L, CLIMBING TO 5000'. ACA460 DEPARTED FROM RWY 24R FOR OTTAWA & WAS PUT ON A VECTOR CLIMB TO 5000'. TRAINING WAS IN PROGRESS AT THE DEPARTURE CONTROLLER POSITION. THE MONITOR THOUGHT THAT ACA460 WAS CLIMBING TO 7000'. (THE NORMAL PROCEDURE IS TO GO OVER THE TOP). THE MONITOR REALIZED THAT ACA460 WAS NOT CLIMBING TO 7000', TOOK OVER CONTROL OF THE FREQUENCY & CLIMBED TO ACA460 TO 7000'. A/C CAME WITHIN 1.5 MILES OF EACH OTHER WITH ACA460 PASSING BEHIND ONT463 WHILE BOTH AIRCRAFT WERE AT 5000'. TIME - 2130Z. ACTION - PAX PAE TSB IKO-PIA.
