A Wasaya Airways Beech 1900D (C-FWAX/WSG913) from Sioux Lookout, ON (CYXL) to Big Trout Lake, ON (CYTL) reported medical emergency on board and advised returning to CYXL. No impact to operations.

Incident at SIOUX LOOKOUT ON (CYXL)

A Wasaya Airways Beech 1900D (C-FQWA/WSG931) on a flight from Sioux Lookout, ON (CYXL) to Winnipeg/James Armstrong Richardson, MB (CYWG) departed CYXL and established contact with the controller passing 5,000 feet. The aircraft had received and read back correctly 4,000 feet as part of their clearance. No impact to operations.

Incident at SIOUX LOOKOUT ON (CYXL)

UPDATE: TSB Report#A13C0168: A Beech 1900D, C-FWZK operated by Wasaya Airways as flight WSG913, was on approach at Fort Hope, ON. When the landing gear extended the left main indicator light did not illuminate. The crew conducted a missed approach and carried out checklist procedures and determined the light was serviceable, but could not obtain a left main gear down indication. The crew diverted the flight to Sioux Lookout, ON, and declared an emergency. On arrival at CYXL, the landing gear was extended and the left main gear indicator initially did not illuminate, but did illuminate about two minutes later. The flight landed without further incident. Company maintenance cleaned the left gear actuator and downlock switch cannon plug, and replaced the left downlock switch. The aircraft has returned to service.
East LOW controller advised a Wasaya Airways Beech 1900D (C-FWZK/ WSG913) from Fort Hope, ON (CYFH) to Sioux Lookout, ON (CYXL) declaring an emergency at approx 2208Z and estimating in 30 minutes. Gear issue. Requested emergency vehicles on standby, emergency vehicles were dispatched and standing by at 2225Z. WSG913 advised they would do a low approach for maintenance to determine if gear was down. WSG913 was able to get three green 5 miles final and land safely at 2239Z. Staff vehicle completed runway check. No further operational impact.

A Wasaya Airways Beech 1900D (C-FWXL / WSG913) from Pickle Lake, ON (CYPL) to Sioux Lookout, ON (CYXL) reported a bird strike on landing. No apparent damage to the aircraft. The remains of one goose were recovered. No operational impact.

2013-06-17 2325Z - Wasaya Airways WSG913 (B190) landed on RWY 16 and advised there was a turtle on runway 16/34. Turtle was removed by airport staff. No impact on airport operations. 2013-06-18 2250Z - Bearskin Lake Air BLS311 (SW4) on departure advised he saw a turtle on runway 16/34. Turtle was removed by airport staff. No impact on airport operations. 2013-06-20 0130Z - Wasaya Airways WSG991(B190) advised there was a turtle on taxiway A while taxiing on taxiway A. Airport staff removed the turtle before the next arrival. No impact on airport operations.

Incident at PICKLE LAKE ON (CYPL) (Aerodrome - visual aids)

The Wasaya Airways Limited Partnership Pilatus PC-12/45 aircraft (C-FWAV, operating as WSG1400) was on an IFR flight from Thunder Bay Airport (CYQT) to Pickle Lake Airport (CYPL). The Wasaya Airways Limited Partnership Beech 1900D aircraft (C-GZVJ, operating as WSG913) was on an IFR flight from Webequie Airport (CYWP) to Pickle Lake Airport (CYPL). The Wasaya Airways Limited Partnership Cessna 208 aircraft (operating as WSG126) was preparing to depart on an IFR flight from Pickle Lake Airport (CYPL) to [destination not reported by NAV CANADA]. NAV CANADA staff at Thunder Bay F.S.S. were notified that the Pickle Lake Airport runway edge lights for runway 09/27 were unserviceable at approximately 0010Z. At the time, two aircraft (WSG1400 and WSG913) were inbound to Pickle Lake Airport and one aircraft (WSG126) was preparing to depart from Pickle Lake Airport. The Airport Manager was called out at 0011Z to fix the problem. WSG913 continued on to Sioux Lookout Airport (CYXL). WSG1400 went into a hold over Pickle Lake Airport for approximately 20 minutes then eventually requested ATC clearance to Sioux Lookout Airport at approximately 0031Z. WSG126 attempted to leave Pickle Lake Airport at 0017Z but was unable to do so. The Pickle Lake Airport Manager installed flare pots by 0050Z. A NOTAM was issued at 0050Z and Winnipeg ACC staff were notified. A voice NOTAM regarding the runway edge light problem was provided until then. Winnipeg ACC staff were advised of the situation at approximately 0015Z. Ops. impact -- no other known operational impact.

Incident at SIOUX LOOKOUT ON (CYXL) (Aerodrome, runway or taxiway shutdown, Aerodrome - operations)

The Wasaya Airways Ltd. Beech 1900D aircraft (operating as WSG913) was concluding an IFR flight from Pickle Lake Airport (CYPL) to Sioux Lookout Airport (CYXL). The flight crew advised that the aircraft may have hit a seagull on landing on runway 34. Airport staff found the remains of one seagull in pieces and advised the flight crew. Ops. impact - unknown.

Incident at THUNDER BAY ON (CYQT) (Other operational incident, Communication navigation surveillance/air traffic system, Aerodrome - power failure, ATM - other)

NAV CANADA staff at Thunder Bay Airport (CYQT) F.S.S. reported that their facility experienced a power failure lasting three (3) minutes. At the time, WSG913 was on final at Pickle Lake Airport (CYPL) and C-FORS was three (3) NM northwest. There was no reported traffic at Island Lake Airport (CYIV) or St. Theresa Point Airport (CYST). The pilot of C-FORS reported that both aircraft were down and clear at Pickle Lake Airport when power was restored. Ops. impact -- unknown.

Incident at PICKLE LAKE ON (CYPL) (Aerodrome, runway or taxiway shutdown, Aerodrome - operations)

The Lockhart Air Services Ltd. Cessna C-402 aircraft (C-GOGP) was concluding a VFR flight from Sachigo Lake Airport (CZPB) to Pickle Lake Airport (CYPL). At 2221Z, the pilot reported that the aircraft was stuck on the runway with flat tire. At 2250Z, the pilot reported that the aircraft was now 10 feet off of the runway, however, an inbound aircraft (WSG913) was unable to land and diverted to Sioux Lookout Airport (CYXL). A NOTAM was issued for the closure of the runway. A second inbound aircraft (WSG1507) was forced to divert to Sioux Lookout Airport (CYXL) as well. The pilot reported that the aircraft was clear of the runway at 2316Z and the runway was re-opened. Ops. impact -- unknown.

Incident at SIOUX LOOKOUT ON (CYXL)

UPDATE Follow-up information received from Maintenance & Manufacturing [2009/05/05]: A Civil Aviation Safety Inspector (CASI) spoke with the operator's DoM about this occurrence. The CASI was advised that maintenance staff investigated the aircraft (C-FWAU) and could not find a problem nor could they duplicate the problem under normal conditions. It is suspected that the pilot still had the Yaw Damper engaged causing (the stiffness in the rudder pedals).
The Wasaya Airways Limited Partnership Beech 1900 aircraft (operating as flight WSG913) was on an IFR flight from Pickle Lake (CYPL) to Sioux Lookout Airport (CYXL). WSG913 landed CYXL @2313 and was un-able to taxi clear of the runway in a timely fashion due power steering problem. After 7 minute delay they were able to taxi off runway 16/34 at taxiway B. Airport staff and Cessna 180 advised on the situation. 1 IFR outbound delayed aprox. 7 minutes and 1 VFR inbound delayed aprox. five minutes.
