Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

A WestJet Boeing 737-7CT (C-GWJT/WJA534) from Winnipeg/Richardson, MB (CYWG) to Toronto/Lester B. Pearson, ON (CYYZ) overshot Runway 15R at 1659Z due to wind shear on approach. WJA534 landed at 1711Z and taxied to the gate without further incident (WOFI). No operational impact.

Incident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG) (FOD (foreign object debris))

A WestJet Boeing 737-6CT (C-GWSB/WJA534) from Winnipeg/Richardson, MB (CYWG) to Toronto/Pearson, ON (CYYZ) reported striking a bird after rotation on Runway 36. Winnipeg Airport Authority (WAA) found a small barn swallow. No impact to operations.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

A WestJet Boeing 737-8CT (C-FYPB/WJA534) from Winnipeg/James Armstrong Richardson International, MB (CYWG) to Toronto/Pearson International, ON (CYYZ) was instructed to Go-Around due to a previous arrival, a Flightpath Charter Airways Inc. Beech 200 (C-FYPB/WJA534) from T. F. Green International, RI (KPVD) to Toronto/Pearson International, ON (CYYZ), that was still on Runway 33L. WJA534 landed on Runway 33L at 1618Z without further incident.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

Aviation Operations update: After exiting the runway via A2, and receiving and reading back an instruction to taxi via A and A3 and then hold short of Runway 24L on Taxiway A3, an I.M.P. Group Cessna 750 (C-GTCI) came to a stop on Taxiway A3 once it passed the Runway 24L protected area.

Group Cessna 750 (C-GTCI), from Great Falls (KGTF), MT, to Montréal (CYUL), QC, came to a stop on Taxiway A3 once past the Runway 24L protected area. Takeoff authorization for a WestJet Boeing 737 700 (WJA534), from Montréal (CYUL), QC, to Vancouver (CYVR), BC, had to be cancelled. No other impact on operations.

Incident at 4507N / 08127W

UPDATE: TSB Report#A17O0126: C-FWSX, a Boeing 737-700 aircraft operated by WestJet, was conducting flight WJA534 from Winnipeg/James Armstrong Richardson Intl, MB (CYWG) to Toronto/Lester B. Pearson Intl, ON (CYYZ). Just prior to the descent into CYYZ, the AUTO FAIL caution warning light illuminated on the pressurization controller panel. The flight crew actioned the AUTO FAIL Non-Normal Checklist (NNC) and selected the alternate controller. The AUTO FAIL light remained illuminated with the controller selected to alternate. In accordance with the NNC, the flight crew selected manual control of the outflow valves and began a 1000 feet per minute descent. During the descent, the pressurization in manual mode became unmanageable, which resulted in a CABIN ALTITUDE warning and horn. The flight crew donned their oxygen masks and completed the CABIN ALTITUDE or RAPID DEPRESSURIZATION QRH procedure. The cabin altitude reached 14 000 feet and the cabin masks deployed automatically. An emergency was declared and a rapid descent was executed. The aircraft descended to 10 000 feet and continued to CYYZ for a normal landing. The operator's maintenance discovered that the number 2 Cabin Pressure Controller would not test and therefore replaced it. The number 1 Cabin Pressure Controller was also replaced as a precaution.
A WestJet Boeing 737-7CT (C-FWSX/WJA534) on a flight from Winnipeg/James Armstrong Richardson, MB (CYWG) to Toronto/Lester B. Pearson, ON (CYYZ) requested immediate descent due to depressurization while descending inbound to CYYZ. The aircraft landed safely on Runway 15R at 1603Z. Aircraft Rescue and Fire Fighting (ARFF) was on standby.

Incident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG) (Aerodrome, runway or taxiway shutdown)

A WestJet Boeing 737-7CT (C-FXWJ/WJA534) on a flight from Winnipeg/James Armstrong Richardson, MB (CYWG) to Toronto/Lester B. Pearson, ON (CYYZ) reported striking a bird while departing Runway 18. The Winnipeg Airport Authority (WAA) inspected the runway and no remains found. An inbound Kississing Lake Lodge Ltd. Beech 200 (C-FAFT) changed to Runway 13. A SkyWest Bombardier CL-600-2B19's (SKW5974) departure was delayed about 2 minutes waiting for the runway inspection. No other impact to operations. (Duplicate AOR 218684-V1 deleted).

Incident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG)

A WestJet Boeing 737-7CT (C-FIBW/WJA534) from Winnipeg / James Armstrong Richardson, MB (CYWG) to Toronto / Lester B. Pearson (CYYZ) was instructed to hold short of Runway 18 at Papa Taxiway and read it back. WJA534 was observed crossing Runway 18 at Papa with a Jazz Bombardier CL-600-2D15 (C-GPJZ/JZA8525) from Ottawa, ON (CYOW) to Winnipeg / James Armstrong Richardson, MB (CYWG) approximately 2 miles final cleared to land Runway 18. JZA8525 was told to continue with the landing clearance and informed of WJA534 crossing.

Incident at 16 SW WIARTON ON (CYVV)

A WestJet Boeing 737 700 (WJA534) from Winnipeg, MB (CYWG) to Toronto, ON (CYYZ) was cleared to descend to FL250 and received a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) through FL260. Traffic was a US Army Beech 200 (PAT544) from Concord, NH (KCON) to Gaylord, MI (KGLR) level at FL240.

A WestJet Boeing 737 700 (WJA534) from Winnipeg, MB (CYWG) to Toronto, ON (CYYZ) reported a bird strike as they departed runway 13. Departing aircraft Keystone Air Service (KEE208), Perimeter Aviation (PAG203), WestJet Encore (WEN3275) and Keystone Air Service (KEE205) were all delayed for a runway inspection. No remains found. No additional impact on operations.

Incident at Kelowna (TCAS alert)

Update from ANS&A: This incident has been investigated by the TSB and no further action is required
Westjet flight WJA534 a Boeing 737 reported a TCAS RA 5 miles southwest of Kelowna ( CYLW). Traffic was C-GZOR a private registered Piper PA-34-200T. Traffic was passed prior to the resolution advisory.
Update from TSB: A03P0142 - The WestJet B737 (WJA534) was at 2,800 feet, arriving at Kelowna from Calgary when the pilots received a TCAS TA, and an RA to climb, on a Piper PA-34 (C-GZOR) inbound from the south. As a result, WJA534 climbed. Although ATC had passed traffic to the WJA534 pilots, they did not see the other aircraft.
Update from the TSB: A03P0141 - The WestJet B737 (WJA065) departed runway 33 at Kelowna for Vancouver and proceeded on the SID towards the LW NDB, at the same time as a Beechcraft was holding at the NDB. The B737 TCAS alerted the pilots with a TA on the Beech, then an RA to reduce climb. The pilot complied with the RA and after the TCAS issued a ""clear of conflict"" message, they resumed the climb to FL240 without further event.
