Incident at PRINCE ALBERT (GLASS FIELD) SK (CYPA)

After landing Runway 08 at 1411Z, a West Wind Aviation Aerospatiale ATR 42-300 (C-GWWR/WEW481) from Saskatoon/Diefenbaker, SK (CYXE) to Prince Albert (Glass Field), SK (CYPA) advised that exiting the runway would be slow due to a hydraulic issue. Assistance was offered and declined. One aircraft was affected and delayed approximately 3 minutes while a vehicle (ST31) completed a foreign object debris (FOD) check.

A Mitchinson Flying Service Ltd. Cessna 172P (C-GMRB) from Saskatoon/Diefenbaker, SK (CYXE) to Saskatoon/Diefenbaker, SK (CYXE), inbound to Prince Albert, SK (CYPA) for a low approach to Runway 26, made its initial call when 4NM Southeast of CYPA, while a West Wind Aviation Aerospatiale ATR 42-300 (C-GWWC/WEW481) from La Ronge (Baker Field), SK (CYVC) to Prince Albert (Glass Field), SK (CYPA) was on right base. WEW481 extended to the East to join a long final.

Incident at LA RONGE (BARBER FIELD) SK (CYVC) (FOD (foreign object debris))

A West Wind Aviation Aerospatiale ATR 42-320 (C-GLDE/WEW481) from La Ronge (Barber Field), SK (CYVC) to Points North Landing, SK (CYNL) reported a bird strike on departure. Two small birds were found upon runway inspection. The aircraft returned without incident. No operational impact.

Assistance was offered and declined. The aircraft landed Runway 26 at 2032Z. No impact to operations.

At 1637Z, a West Wind Aviation Aerospatiale ATR 42-300 (C-GWWC/WEW481) on a flight from La Ronge (Barber Field), SK (CYV) to Point North Landing, SK (CYNL) reported a medical emergency on board and advised returning to CYVC. Emergency medical services (EMS) advised. No impact to operations.

Incident at 38NM LA RONGE (BARBER FIELD) SK (CYVC)

UPDATE: TSB REPORT#A17C0052: C-GWWV, a Beech 200 King Air aircraft operated by West Wind Aviation, was conducting flight WEW250 from La Ronge (Barber Field), SK (CYVC) to Wollaston Lake, SK (CZWL). After the departure from CYVC, WEW250 was cleared to FL190. 38 nautical miles North of CYVC, WEW250 was observed on radar at FL194 as was asked by ATC to confirm the altitude. WEW250 then descended back to FL190. This altitude excursion created a conflict with C-GLDE, an Aerospatiale ATR 42-320 aircraft operated by West Wind Aviation, who was conducting flight WEW481 from CYVC to Prince Albert/Glass Field, SK (CYPA). A loss of separation occurred when the vertical distance between the two aircraft eroded to 600 feet with a lateral distance of 1.2 nautical miles, where 1000 feet or 5 nautical miles is required.
A West Wind Aviation Limited Partnership Beech 200 (WEW250) was cleared to 19 000 feet by the Winnipeg area control centre (ACC) controller and read back the clearance correctly. The aircraft was observed on radar at 19 400 feet by the controller and was asked to confirm its altitude. The flight was descended to 19 000 feet. The aircraft was within 600 feet and 1.2 miles of traffic, a West Wind Aviation Aerospatiale ATR-42-300 (WEW481). No other impact to operations.

Incident at LA RONGE (BARBER FIELD) SK (CYVC) (Aerodrome, runway or taxiway shutdown)

A Transwest Air Limited Partnership Beech B200 (C-GKHI/TW01-T) on a local flight from La Ronge, SK (CYVC) reported a possible gear indication problem and declared an emergency. Emergency services were advised. The aircraft landed without incident at 1921Z. Runway 18/36 was closed for approximately 30 minutes via voice NOTAM. Impact on operations: a West Wind Aviation Aerospatiale ATR-42-300 (WEW481) was delayed on departure.
UPDATE: TSB REPORT#A17C0042: C-GKHI, a Beech B200 aircraft operated by Transwest Air, was conducting local training flight TW01-T from La Ronge (Barber Field), SK (CYVC). During the approach to Runway 36, the landing gear selector was moved to the "DOWN" position. The right main landing gear did not indicate down and locked and the red "Gear Unsafe" light in the landing gear handle remained illuminated. The flight crew performed a go-around and orbited to the east to troubleshoot. QRH actions were carried out and the landing gear cycled without success. Maintenance personnel were consulted and an emergency landing gear extension was performed; however, the landing gear indications remained the same. The flight crew declared an emergency and orbited to burn off fuel. Immediately prior to touchdown, the fight crew shutdown both engines and feathered the propellers. The landing was without incident and the aircraft was brought to a stop on the runway. The operator's maintenance replaced the right main landing gear down-lock switch.

A West Wind Aviation Aerospatiale ATR 42-300 (C-GWWR/ WEW481) from La Ronge, SK (CYVC) to Prince Albert, SK (CYPA) received an Air Traffic Control (ATC) clearance to Prince Albert, SK that included a standard instrument departure (SID) restriction. The flight departed at 2022Z and did not conform to the SID restriction.

Incident at PRINCE ALBERT (GLASS FIELD) SK (CYPA)

At 1401Z, a West Wind Aviation Aerospatiale ATR 42-320 (C-GWEA/WEW481) from Saskatoon, SK (CYXE) to Prince Albert, SK (CYPA) initiated a missed approach while on the ILS08 due to aircraft equipment issues. WEW481 landed safely at 1412Z. 2 inbound aircraft and 1 outbound aircraft were delayed 10-15 minutes.

Incident at LA RONGE (BARBER FIELD) SK (CYVC)

A West Wind Aviation Limited Aerospatiale ATR-42-300 (WEW442) from La Ronge, SK (CYVC) to Cigar Lake, SK (CJW7) was issued a departure clearance from La Ronge via the CYVC Standard Instrument Departure (SID). Shortly after departure, WEW442 was radar observed turning off the SID and was vectored to maintain separation with another West Wind Aviation Limited Aerospatiale ATR-42-300 (WEW481) from Points North Landing, SK (CYNL) that was on approach into CYVC. WEW442 was subsequently instructed to proceed to destination. No impact on operations.

Incident at PRINCE ALBERT (GLASS FIELD) SK (CYPA)

Update TSB Report #A14C0133: The Cessna A185F amphibious aircraft, registration C-FNWU, departed Prince Albert Airport with one pilot on board. During the initial climb, the aircraft lost all engine power (Continental IO-520). The aircraft force-landed 5.2 nautical miles SE of the airport with the landing gear retracted. There were no injuries. The aircraft sustained minor damage to the engine cowling due to the uncontained engine failure.
At 1947Z, a Silvernagle Air Operations Cessna A185F (C-FNWU), departed Prince Albert (Glass Field), SK (CYPA) on a VFR flight to Yorkton, SK (CYQV). At 1950Z, reported clear of the zone. At 1951Z, advised of mechanical issue and was going to make a forced landing. Initial intention was to land on North Saskatchewan River as aircraft is amphibious. Unable to land on river and made a forced landing on a field. N530832 W1052941 (approximately 9 SE YPA). West Wind Aviation (WEW481) was in vicinity and was requested to assist to verify location and condition of aircraft and passengers. WEW431 provided location and information that pilot was walking around outside of aircraft. Emergency Rescue Services (ERS) notified and enroute.

Incident at 60 miles E SASKATOON / JOHN G. DIEFENBAKER INTL SK (CYXE)

A Jazz Aviation Bombardier CL600 2D24 (RJ900 (JZA8841) was inbound to Saskatoon, SK (CYXE) from Toronto, ON (CYYZ) and was instructed to descend to FL240. A Province of Saskatchewan Beech 200 (SLG3) from Saskatoon, SK (CYXE) to Winnipeg, MB (CYWG) had departed CYXE and was eastbound at FL230. Traffic was passed between both aircraft. At 1936Z, JZA8841 was observed descending below FL240 without a clearance and was queried as to their altitude. JZA8841 believed that they had been cleared to 7,000 feet. Moments before JZA8841 commenced their descent, an uninvolved aircraft on the frequency, a West Wind Aviation Aerospatiale ATR 42-300 (WEW481) from La Ronge, SK (CYVC) to Prince Albert, SK (CYPA), was cleared to descend to 7,000 in the vicinity of CYPA. At this time JZA8841 and SLG3 were observed on radar as having passed each other and that radar separation did exist and JZA8841 was cleared to 7,000 feet.
