Incident at GANDER CENTRE / ACC DE GANDER (Communication navigation surveillance/air traffic system)

A flight plan for a United Airlines Boeing 767-300 (UAL929) from London/Heathrow Int'l, UK (EGLL) to Chicago/O'Hare Int'l, IL (KORD) was received at time 0229Z and had a routing of YVO BURWA ODAXY. At time 0550Z, a change message with a modification to the route, YVO ODAXY, was received but was not processed at the Air Traffic Operations Specialist (ATOS) position. The aircraft flew modified route, the Canadian Automated Air Traffic System (CAATS) profile was based on the original flight plan. Incorrect routing was transferred to Montreal Center.

At 1259Z, Air Traffic Control (ATC) issued a clearance to a United Airlines Boeing 767 300 (N649UA/UAL929) from London, England (EGLL) to Chicago, IL (KORD) to proceed direct Sault Ste. Marie, MI (SSM). There was no read back from the flight. The flight proceeded via the flight plan route, CUDDY - JOVIE - GRAMP - SSM. Separation was maintained with other flights at all times.

UAL929, a United Airlines Boeing 777-200, was conducting an instrument flight rules (IFR) flight from London (EGLL) to Chicago (KORD). Incident at Intersection ROBDO (Conflict - loss of separation, ATS operating irregularity)

There was a loss of separation between NWA67, a Northwest Airlines Airbus 330, and UAL929, a United Airlines Boeing 777, when NWA67 was authorized to climb from FL 350 to FL 370 while UAL929 was at FL 360 with less than the required longitudinal separation. The separation was based on 10 minutes between the two aircraft at the same point, while the aircraft had 16 minutes on a converging path, but not at the same point.
Update #1: The TSB investigation class and number were added. The TSB reports that the Airbus 330-300 operated by Northwest Airlines and the Boeing 777-200 operated by United Airlines were both bound for Detroit, inbound from Europe. The two aircraft were on converging paths at the RODBO intersection in MNPS/RNPC transition airspace. The Airbus 330 was authorized to climb from FL 350 to FL 370, while the Boeing 777 was at FL 360. There was a loss of separation when the Airbus climbed through FL 360, since the aircraft were 10 minutes apart, while the standard was 15 minutes for converging paths at the same altitude.

UAL 929, B-777, declared a medical emergency due to a 61 yr old male with a suspected heart attack. The pilot requested a diversion to Gander and was cleared Carpe YAY YQX. UAL 929 had to dump fuel prior to landing. A/C landed Gander at 1501Z. Nil TSB

Incident at Entre les points de compte rendu PEPKI et LOPVI (Conflict - loss of separation, ATS operating irregularity)

Un manque de séparation est survenu lorsqu`AAL87, un Boeing 777 d`American Airlines, a été autorisé du niveau de vol (FL) 350 au FL370 sans que l`espacement longitudinal de 10 minutes n`ait été assuré avec UAL929, un Boeing 727 de United Air Lines, évoluant au FL370, derrière AAL87. Les deux aéronefs se sont rapprochés à 8 minutes sur la même trajectoire, en palier au FL370. A loss of separation occurred when American Airlines Boeing 777 AAL87 was cleared to climb from FL350 to FL370 without verifying the 10-minute longitudinal separation with United Airlines Boeing 727 UAL929, which was at FL370 behind flight AAL87. The two aircraft approached to within 8 minutes of each other on the same path, in level flight at FL370.

Incident at 45 mi. sud-est de Val-D'or

UPDATE #1: Modified aircraft information to amateur built and changed occurrence type to Accident.
The Trenton Search & Rescue Centre informed the Rouyn FSS that an ELT signal had been received by satellite. A MAYDAY call was also picked up by UAL929, and the information was relayed to the Montreal Control Centre. FBVN informed the Rouyn FSS that it was in contact with the pilot, that there were no injuries and that the aircraft was between Dorval Lodge and Lac Cambrai. The aircraft was found by CAF75, and the two passengers were taken to Val D'Or. Minor damage. Update: The ultralight was at 2,300 feet when the pilot became slightly ill. He decided to land the ultralight on a lake. On landing, one of the skis dug into the snow and broke. The ultralight nosed over. The pilot activated the emergency locator transmitter. They were helped by Search & Rescue about 5 hours later. No injuries. Damage to the propeller and windshield. Le Centre de Recherches & Sauvetages de Trenton a avisé la FSS de Rouyn d'un signal ELT capté par satellite. Un appel "MAYDAY" a également été capté par UAL929 et l'information a été passée au Centre de contr,le de Montréal. FBVN a avisé la FSS de Rouyn qu'il était en contact avec le pilote, qu'il n'y avait aucun blessé et que l'aéronef était entre Dorval Lodge et le Lac Cambrai. L'aéronef a été retrouvé par CAF75 et les deux passagers ont été transportés à Val D'Or. Dommages légers. Mise à jour: L'aéronef était à 2300 pieds lorsque le pilote a senti un léger malaise. Il a décidé de poser l'aéronef sur un lac. Lors de l'atterrissage, un des ski s'est enfoncé dans la neige et a cassé. L'aéronef a culbuté. Le pilote a activé la radiobalise de secours. Ils ont été secourus par Recherches et Sauvetage environ 5 heures plus tard. Aucun blessé. Dommages à l'hélice et au pare-brise.

Incident at Gander/Montreal FIR

The United Airlines, UAL929, was on a flight from Heathrow to Chicago when at 61N060W the crew advised of a medical emergency. A request was made for a clearance to Goose Bay. The ACC could not provide clearance at the time. At 1610 the was aircraft contacted and cleared directly to Goose Bay at FL370. TSB no involvement.
