Despite having been authorized and correctly re-reading 7000ft, an American, privately registered Cirrus SR22 from Quebec/Jean Lesage, QC (CYQB) to Kuujjuaq, QC (CYVP) continued to climb up to 7400ft before the intervention of air traffic control (ATC). Aircraft indicated autopilot trouble. No impact.

The aircraft was observed on radar climbing to 2000 in Class C airspace without authorization from the CYMX Tower. No impact on operations.

Incident at VANCOUVER / BOUNDARY BAY BC (CZBB)

At 2315Z, the Kamloops flight information centre (FIC) received a flight plan, filed through the Internet, for a Cirrus Aviation Services Cirrus SR22 (C-GEHK) from Victoria, BC (CYYJ) to Vancouver/Boundary Bay, BC (CZBB). The departure time was 2115Z. The Flight Service Station Information Management System (FIMS) interpreted the flight plan for the next day. Alerting position called the pilots cell phone to confirm the time was correct and left a message. At 0100Z, the pilot called back to discuss the error, noting that they were on the ground at CZBB and had already completed the flight without alerting, and asked the flight plan be cancelled.

A privately registered Cirrus SR22 from Toronto/Oshawa, ON (CYOO) to Kitchener/Waterloo ON (CYKF) departed Runway 30 and reported a bird strike on climb out. The aircraft circled for the circuit and came back in to land. The aircraft taxied in with no issues. A foreign object debris (FOD) check of the runway was completed, and the bird (sparrow) was retrieved. No impact on operations.

Accident at 56.6NM N SEPT-ÎLES QC (CYZV)

An American, corporately operated Cirrus SR22 (N699RS) from Wabush, NL (CYWK) to Quebec/Jean Lesage, QC (CYQB) declared an emergency and requested to divert to Sept-Îles, QC (CYZV). En route to CYZV, the aircraft made an emergency landing using its parachute: 510072N/0670097W. The emergency locator transmitter (ELT) signal went off. Joint Rescue Coordination Centre (JRCC) Trenton was notified. Thanks to the help of aircraft in the area, the crash site was located. There was one person on board, who was safe and sound.
Update TSB #A19Q0119: N699RS, a privately operated SR22, was conducting an IFR flight from Wabush Airport, NL, to Quebec City/Jean-Lesage Intl, QC, at an altitude of 8,000 feet ASL. The pilot, who was alone on board, noted a drop in engine oil pressure and high oil temperature. The pilot declared an emergency and requested a diversion to Sept-Îles Airport (CYZV), QC. Approximately 60 NM north of CYZV, the engine (TELEDYNE CONTINENTAL MOTORS IO-550) stopped. The pilot glided the aircraft down to approximately 4,000 feet before deploying the aircraft’s parachute system. The aircraft crashed into trees about 56 miles north of CYZV and sustained significant damage. The pilot was not injured. The pilot activated the SOS mode of his Garmin InReach hand-held GPS. He was rescued about five hours after the crash by Canadian Armed Forces Search and Rescue.

Once it was in position, the aircraft requested to go back to the hangar. Before it could clear the runway, the aircraft stopped due to a blown tire, then asked to be towed off the runway. Impact: two other aircraft (CRQ606 and QUE228) had to fly holding patterns for 15 minutes.

A 608369 BC Ltd. Cirrus SR22 (C-GBCE) from Kamloops, BC (CYKA) to Abbotsford, BC (CYXX) was disabled on Runway 19 until 2050Z. Several aircraft were re-taxied for Runway 25 with the wind 190/10-15. Operations continued on Runway 25, however the tower was unable to approve some requests for circuits until the problem was resolved.

Incident at VERNON BC (CYVK)

The pilot of a 1072422 B.C. Ltd. Cirrus SR22 (C-GMEM) from Kamloops, BC (CYKA) to Vernon, BC (CYVK) called just before 2300Z to close their VFR flight plan from CYKA into CYVK . No flight plan was on file at the flight information centre (FIC). The pilot apparently misfiled online as an IFR and somehow during the changeover from an IFR to a VFR flight plan, their alerting was closed. The FIC was never informed of the flight.

Incident at PETERBOROUGH ON (CYPQ)

Aviation incident report #16168: At approximately 1520Z, A Seneca College of Applied Arts and Technology Cessna 172S (C-GSCS) was established on final for landing on Runway 09, sequenced after an Exact Air Cessna R182 (C-GSCG) who was departing Runway 09 from Hotel intersection. Immediately after C-GSCG became airborne, C-GSCS (at approximately 500 above ground level (AGL) on final) observed a privately registered Cirrus SR22, taking position onto the runway. Upon seeing a potential conflict, C-GSCS re-broadcasted its position on short final for Runway 09 in an effort to alert the Cirrus SR22 of its current proximity to landing. At this point, the Cirrus SR22 proceeded to takeoff Runway 09 with minimal separation behind the departed aircraft in front (C-GSCR). C-GSCS initiated the go-around (at approximately 1521Z) as the runway was unclear for landing approaching 200 above ground level (AGL). During the go-around, C-GSCS witnessed the Cirrus SR22 perform a low level left-hand turn departing to the North soon after it took off. It was noted that C-GSCS was at approximately 300 AGL when the Cirrus SR22 began turning left from an altitude that was lower than C-GSCS. At 1522Z, C-GSCS broadcasted on the ATF frequency asking the Cirrus SR22 what his/her intention was, to which there was no response. No radio call was heard from the Cirrus SR22 from that point on. This created a dangerous situation. The circuit was extremely busy at this time due to an airport event at Peterborough, ON (CYPQ).
