Emergency services were advised. No damage to the aircraft. Impact: Runway was closed from 1457Z to 1515Z and delays of 1 to 5 minutes occurred for several aircraft.

Accident at TORONTO / BUTTONVILLE MUNICIPAL ON (CYKZ) (Aerodrome, runway or taxiway shutdown)

The airport manager at the Toronto/Buttonville, ON (CYKZ) aerodrome called to inform the London flight information centre (FIC) that a Toronto Automart Ltd. Cirrus SR20 (C-FACJ) from Toronto/Buttonville, ON (CYKZ) to Toronto/Buttonville, ON (CYKZ) had gone off the end of Runway 15, and that both Runway 03/21 and 15/33 would be closed until 0000Z.
Aviation Incident Report #15934: At approximately 1747Z, a Toronto Automart Ltd. Cirrus SR20 (C-FACJ) from Toronto/Buttonville, ON (CYKZ) to Toronto/Buttonville, ON (CYKZ) was observed to stall, first noticed north of CYKZ Taxiway Alpha, as it attempted to takeoff but was failing to lift. The aircraft then attempted a second time to establish lift by pulling up and obtaining approximately 10 feet, then stalling and landing on its main gear, dragging the tail. The aircraft then proceeded to attempt another climb that was unsuccessful, with a steep right bank, and embarked past Hangar 9. The aircraft overshot the Runway 15 threshold. The aircraft failed to lift and went through the fence and crashed to the North of 16th Avenue.
Update TSB Report #A19O0028: A privately registered Cirrus SR20 aircraft, was rented by a commercially licensed pilot for the purpose of building flight time. As the pilot had limited experience on this aircraft type, in addition to no recent flying, the pilot hired an instructor to provide some training. The instructor was a Class 1 instructor, and a Transport Canada accredited Pilot Examiner. On 12 March 2019 at 1342 Eastern Daylight Time (EDT), the aircraft departed from Runway 33 at Toronto/Buttonville Municipal (CYKZ), ON, with the commercial pilot and the instructor on board, with the intention to conduct some circuits at CYKZ. The wind was light and variable, so the aircraft joined the left downwind leg of the circuit for Runway 33, and planned for a touch-and-go. At 1348 EDT on the initial touchdown, the aircraft bounced and full power was added to commence the takeoff portion of the touch-and-go. The nose of the aircraft was raised, the aircraft became briefly airborne, before returning to the surface abruptly. The nose was raised again, and the tail of the aircraft struck the runway as the aircraft became airborne again, albeit briefly. As the aircraft contacted the ground the third time, the outer right flap hinge and right wheel contacted the ground outside of the runway surface laterally, approximately 1000 feet from the runway end. The nose was raised again, and the aircraft continued the take-off roll. As the aircraft left the runway surface longitudinally, still on the ground, it struck a 4 foot high fence, located 60 feet from the runway end. Past this fence, the terrain slopes down toward 16th Avenue, which crosses the departure path perpendicularly. The aircraft crossed 16th Avenue in the air, in a right bank, and dragging the right wing tip. The aircraft passed directly in front of a tow truck that was in the westbound lane, and the driver of the vehicle captured the event on dash-camera. North of 16th Avenue, the terrain rises rapidly; the aircraft collided with the ground, and came to a stop approximately 250 feet passed the end of Runway 33. The two occupants were wearing 4-point safety belts and received no significant injuries. The aircraft received significant damage to the wings, nose, tail, landing gear, and propeller. The aircraft was equipped with an Avidyne FlightMax Entegra flight display that recorded engine and GPS data. The unit recorded data at 10hz, or one data point every 6 seconds. This data was acquired and analyzed by the TSB following the accident. It was noticed that, following the initial touchdown on Runway 33, the engine RPM and manifold pressure increased to full, followed by a significant reduction, and then a return to full. During the touch-and-go attempt, the recorded ground speed steadily decreased from 61 kts at the initial touchdown point, to 52 kts shortly before impact.

A Calgary Flying Club Cirrus SR20 (C-FGXK) from Calgary/Springbank, AB (CYBW) to Calgary/Springbank, AB (CYBW) was sequenced to follow a Springbank Air Training College Ltd. Cessna 172N (C-FIAH) from Calgary/Springbank, AB (CYBW) to Calgary/Springbank, AB (CYBW) ahead downwind, and instead turned base and descended, cutting off traffic on final.
UPDATE from Flight Operations: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The rental pilot of C-FGXK was debriefed by the Chief Flight Instructor (CFI) regarding this incident. They were reminded to slow down prior to entering the circuit and were also reminded to advise the tower that they didn’t have their traffic and have them call their base turn if required.

Incident at TROIS-RIVIÈRES QC (CYRQ) (FOD (foreign object debris), Aerodrome, runway or taxiway shutdown)

Aviation incident report #15529: There was radio contact from a Parachutisme Adrénaline Inc. Cessna 208B (C-FDJN) to report that a main parachute (canopy only) had detached from a parachutist during a jump. The canopy was found in the middle of the threshold of Runway 23, and the parachutist landed normally, in the usual landing site and with an emergency parachute. The aircraft specified the location of the parachute on the ground. The owner of the parachuting company phoned to request that someone pick up the parachute so that it could be kept intact and so that the cause of the problem could be verified. After the canopy had been inspected, the owner picked up the parachute. Radio communication was established with an Aviation CVO Inc. Cirrus SR20 (C-GVEO), which was conducting touch-and-goes at that time. Everything was back to normal when the runway was cleared 4 to 5 minutes after the first radio contact. Impact on operations: extension of the circuit for C-GEVOs touch-and-goes.

Aviation Incident Report #15415: A Toronto Automart Ltd. Cirrus SR20 (C-FACJ) on a flight part of the 99s Poker Run departing Toronto/Buttonville Municipal, ON (CYKZ) and landing at Toronto/Buttonville Municipal, ON (CYKZ) with en route stops at CYOO, CNF4, CYQA and CNY3 was directed by the Controller at Toronto/Oshawa, ON (CYOO) to extend their downwind. The Controller instructed that they would call base. The aircraft turned base upon being instructed and turned final at the appropriate time to line up with the runway centre line. As the aircraft was on final, they noticed another aircraft; a Borden Flying Club Cessna 177B (C-GPDW) a few hundred feet away that was just in the process of turning final. After the loss of separation occurred, the Controller instructed C-FACJ to turn left.

The aircraft decided to continue landing and after touchdown went onto the closed portion of Runway 12 hitting several cones before coming to a stop. The aircraft backtracked to Delta and taxied to the apron. No impact to operations.
