Incident at TORONTO CENTRE / ACC DE TORONTO

A Sky Regional Airlines Embraer ERJ 170-200 SU (SKV7665) from Toronto/Lester B. Pearson, ON (CYYZ) to Dallas, TX (KDFW) returned to CYYZ with engine vibrations. Aircraft landed safely at 1734Z. No impact to operation.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Incursion - manoeuvring area)

TSB Report #A19O0006: C-FEJB, an Embraer ERJ 170-200 SU, (operated by Sky Regional Airlines, flight 7665) was departing from runway 06L at Toronto/LBPIA (CYYZ) during snowy weather conditions. At the same time, a convoy of snow plows /sweeper vehicles were operating north of runway 06L and proceeding southbound on taxiway Echo. ATC instructed the vehicles to turn left on to taxiway Charlie (east), however the vehicles turned left on Charlie 2 (C2) towards Runway 06L and continued past the hold short position. Ground control instructed the vehicles to STOP, and they came to rest just prior to 06L. The tower controller instructed C-FEJB to abort; the flight crew rejected the take-off and the aircraft came to rest just prior to C2.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Incursion - runway - vehicle, Aerodrome, runway or taxiway shutdown)

At the Toronto/Lester B. Pearson Intl, ON (CYYZ) aerodrome, a snow plow (PL862) was the lead vehicle for a roving taxiway team that was removing snow from the taxiways. The team was instructed to turn left on Taxiway Charlie at 1847Z. At 1848Z, other snow plows (PL170 and BL186) were observed crossing the stop bar on Charlie 2. Ground control instructed the team to stop. The south tower instructed a Sky Regional Airlines Inc. Embraer ERJ 170-200 SU (SKV7665) from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Dallas/Fort Worth Intl, TX (KDFW) to abort takeoff at 1848Z. After the rejected takeoff, SKV7665 was disabled on Runway 06L due to stuck reversers. This closed the runway for approximately 45 minutes and required a ground stop and significant airborne holding.

Incident at Dallas/Fort Worth International Airport, TX (KDFW)

UPDATE from Maintenance: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The CASI contacted the operator involved for more information and was provided with additional details of this occurrence. Torque tube off of Right Hand (R/H) #3 flap actuator was found. Torque tube reinstalled. The CASI was satisfied with the information provided and advised that no further follow-up action by Transport Canada was required at this time.
TSB#A16F0021: The Sky Regional Embraer ERJ 170-200SU, C-FEIQ, operating as flight SKV7665 from Toronto/LBPIA, ON (CYYZ) to Dallas/Fort Worth Intl, TX (KDFW), was on the visual approach for Runway 35C at KDFW when a Flap Fail CAS message was received. The QRH checklist was completed and a PAN PAN declared. The aircraft landed uneventfully.
