A Skycare Air Ambulance Swearingen SA226-AT (C-GCPX) from Sioux Lookout, ON (CYXL) to Round Lake (Weagamow Lake), ON (CZRJ) was observed on radar at FL180 without clearance. When queried, the pilot descended to their cleared altitude of A170. No impact to operations.

Aircraft re-cleared for approach to Runway 25. Aircraft landed at 0407Z. No impact to operations.

Incident at SIOUX LOOKOUT ON (CYXL)

The pilot of a Skycare Air Ambulance Swearingen SA226-AT (C-GCPX) from Thunder Bay, ON (CYQT) to Sioux Lookout, ON (CYXL) landing on Runway 16 had difficulty controlling the aircraft on landing and subsequently damaged runway edge light. No other impact to operations.
UPDATE TSB Report #A20C0007: C-GCPX, a Swearingen SA-226-AT operated by Skycare Air Ambulance was enroute from Thunder Bay (CYQT), ON to Sioux Lookout (CYXL), ON with two flight crew members on board. During the landing roll on Runway 16 at CYXL, the crew pulled the power levers in an asymmetric configuration and the aircraft veered off to the right side of the runway. While regaining directional control of the aircraft, the left propeller struck a runway light. The aircraft then taxied to the ramp without further incident. An inspection of the aircraft by the operators maintenance revealed damage to one of the blades of the left propeller.

Swearingen SA226-AT (C-GCPX) on a flight from Sioux Lookout, ON (CYXL) to Cat Lake, ON (CYAC) was given and read back instructions to maintain 4000 feet on the standard instrument departure (SID) (which is normally 5000 feet) and was observed on surveillance climbing through 4000 feet. A 2080061 Ontario Inc. Swearingen SA226-TC (C-GKPX) on a flight from Sioux Lookout, ON (CYXL) to Thunder Bay, ON (CYQT). They departed Runway 16 at CYXL at 0839Z and immediately advised returning to CYXL to land and no assistance required. The aircraft landed Runway 16 without incident at 0844Z.

Incident at SIOUX LOOKOUT ON (CYXL) (Aerodrome - power failure, Aerodrome - visual aids)

At 1222Z, at Sioux Lookout, ON (CYXL), a power bump occurred, and all approach and runway lighting were lost while a Skycare Air Ambulance Swearingen SA226-AT (C-GCPX) from Sault Ste. Marie, ON (CYAM) to Sioux Lookout, ON (CYXL) was in the final landing phase. Aircraft landed, and held position on runway until temporary power was restored approximately one minute later. Aircraft was able to taxi while runway and taxiway lights surged between high and low.

Incident at 50NM SE THOMPSON MB (CYTH)

A Perimeter Aviation Swearingen SA226-TC (C-GIQK/PAG13) on a flight from Thompson, MB (CYTH) reported smoke in the cockpit and requested returning to CYTH. Aircraft Rescue and Fire Fighting (ARFF) assistance was offered and declined. The aircraft landed safely. No impact to operations.
Update TSB Report #2020C0048: C-GIQK, a Swearingen SA226-TC operated by Perimeter Aviation LP., was conducting flight PAG13 from Thompson (CYTH), MB to Oxford House (CYOH), MB. When the flight was approximately 50 nm southeast of CYTH, the flight crew noticed a haze of smoke in the cockpit. The flight crew donned oxygen masks and carried out QRH functions for smoke in the flight deck. The flight crew notified ATC and elected to return to CYTH. An emergency was not declared and the flight crew declined ARFF services. The subsequent landing was uneventful. The operators maintenance personnel found no evidence of a fire or source of smoke. Further inspection found the environmental system water-separator housing full of water. A drain tube was replaced. The #1 and #2 cooling turbine water separators were removed, cleaned, and re-installed.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The flight crew of C-GIQK reported smoke in the cockpit and returned to CYTH. Maintenance found no evidence of smoke or fire. After further inspection, maintenance found the #2 drain port blocked and the water separator housing full of water. Drain port tube replaced and water removed. Also, the #1 and #2 cooling turbine water separators were removed, cleaned and re-installed.

Incident at 75NM SE THOMPSON MB (CYTH)

A Perimeter Aviation Swearingen SA227-AC (PAG13) from Oxford House, MB (CYOH) to Thompson, MB (CYTH) reported a wing overheat indication and declared a PAN PAN. Pilot declined assistance at destination and landed without further incident at 1800Z. No impact to operations.
UPDATE TSB Report #A20C0043: C-GIQK, a Swearingen SA226-TC aircraft operated by Perimeter Aviation LP., was conducting medevac flight PAG13 from Oxford House, MB (CYOH) to Thompson, MB (CYTH). While in cruise to CYTH, the left-hand wing-overheat warning light illuminated. The crew performed the checklist items in the AFM and notified ATC of the issue. Following the checklist the wing overheat light extinguished and did not re-illuminate for the remainder of the flight to CYTH. The flight crew informed ATC of the event, declared a PAN PAN and landed the aircraft safely. The operators maintenance discovered a bleed air leak at the bleed air line/ turbine cooling fan attach point. The aircraft was repaired and returned to service.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. C-GIQK flying from Oxford House, MB (CYOH) to Thompson, MB (CYTH) on a MEDEVAC flight had a left-hand (L/H) wing overheat indication. The flight crew turned off the bleed air as per the Aircraft Flight Manual (AFM) checklist, and the overheat caution light extinguished. The aircraft landed uneventfully into CYTH. Maintenance discovered that the bleed pipe connection at the cooling air turbine was leaking. The pipe connection was re-secured and the aircraft was returned to service.

A Perimeter Aviation Swearingen SA226-TC (C-FJNW/PAG03) from Thompson, MB (CYTH) to Thompson, MB (CYTH) returned to the field shortly after take-off due to a maintenance issue. No assistance required. No impact on operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. C-FJNW conducted an air turn back in Thompson, MB (CYTH) due to the landing gear failing to retract. Upon "gear up" selection, gear would not retract. 3 red "in transit" remained upon selection of "gear down". Gear indicated 3 green. Found normal/emergency selector towards the emergency position. Selector was re-positioned to normal. Gear swings carried out in accordance with (IAW) maintenance manual (MM) 32-30-00. Checked serviceable.

A Perimeter Aviation Swearingen SA226-TC (PAG02) from Cross Lake, MB (CYCR) to Thompson, MB (CYTH) declared PAN PAN while inbound from the south and indicated they are flying single pilot as the captain needed to tend to a medical passenger in the back. Passenger was assisted and the captain re-joined the cab and landed without incident. No emergency declared, no impact on operations.
Update TSB Report #A20C0010: C-GIQG, a Swearingen SA 226-TC operated by Perimeter Aviation PAG02, was conducting a medevac flight from Cross Lake (CYCR), MB to Thompson (CYTH), MB. During the descent into CYTH, the paramedic attending the patient required assistance. The first officer left the cockpit to assist and the Captain made a PAN PAN indicating single pilot operations. Prior to landing, the first officer returned to the cockpit and resumed normal duties. The aircraft subsequently landed safely at CYTH.

Incident at 5NM S THOMPSON MB (CYTH)

A Perimeter Aviation Swearingen SA226-TC (C-FSLZ/PAG08) from Thompson, MB (CYTH) to Cross Lake, MB (CYCR) departed Runway 24 at 0106Z. Once 5NM South, PAG08 advised that their landing gear was locked down and they were returning to CYTH. Landed on Runway 24 at 0121Z, no impact on operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. C-FSLZ had an air return to Thomson, MB (CYTH) due to the landing gear failing to retract. Maintenance carried out the following and returned the aircraft to service. Landing gear selector valve and Left-Hand (L/H) main landing gear (MLG) O/B Squat switch inspected for condition and security with no faults found. Wiring and Canon plugs inspected in accordance with (IAW) SA226 maintenance manual (MM) 24-00-00 with no faults found. Hydraulic accumulator serviced IAW SA226 MM 29-10-04. Nose and both main landing gears cleaned and lubricated IAW SA226 MM 12-20-10. Landing gear function checks completed IAW SA226 MM 32-00-00. Aircraft found to be serviceable.

UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. C-GKKC had no right-hand (R/H) green down gear light on approach to Calgary, AB (CYYC), crew requested a go around and were vectored to an area where they could troubleshoot. Flight crew swapped gear bulbs and confirmed it was just a blown bulb and returned to land. Maintenance replaced all the gear bulbs as a precaution.

UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. C-GQAJ had a rejected takeoff at Thompson, MB (CYTH) due to the #2 propeller not coming off of the locks. The corrective actions were taken as follows: #2 Spinner removed and snow and ice was found. Removed snow and ice and dried start lock. Tested start locks in accordance with (IAW) SA226 MM 61-10-00. Function checked. Serviceable.

The Island Lake, MB (CYIV) runway edge lights failed at 0104Z. 2 aircraft were delayed and one aircraft diverted. A Perimeter Aviation Swearingen SA226-TC (C-GIQF/PAG12) on a flight from Island Lake, MB (CYIV) to Winnipeg/James Armstrong Richardson Intl, MB (CYWG) was unable to turn runway lights on using aircraft radio control of aerodrome lighting (ARCAL) at 0104Z. PAG12s departure was delayed by 30 minutes. A Beaver Air Services Beech 200 (C-FICU/MA04) on a flight from Winnipeg/James Armstrong Richardson Intl, MB (CYWG) to Island Lake, MB (CYIV) diverted to St. Theresa Point, MB (CYST). A Skynorth Air Beech E90 (C-GSNM/SN696) on a flight form Winnipeg/James Armstrong Richardson Intl, MB (CYWG) to Island Lake, MB (CYIV) was delayed landing for 10 minutes. Lights reset by airport staff at 0138Z.

When queried, the pilot advised "levelling 6000 ft". Radar playback indicates maximum altitude achieved was 6600 ft before descending to 6000 ft. Incident at ST. THERESA POINT MB (CYST)

Aviation Incident Report #16146: The pilot of a Perimeter Aviation Swearingen SA226-TC (C-GQAJ) saw a flock of seagulls during landing on Runway 04 at St. Theresa Point, MB (CYST). They landed as usual however after parking and shutting of the engines smoke was noticed from the left engine. A fire extinguisher was immediately used, there was no visible flame. Pilots indicated they did not notice hitting a bird however after inspection small traces of blood were discovered on the propeller.

No emergency was declared and assistance was not required. PAG09 flew an extended downwind for Runway 24 to run checks and landed without incident at 0310Z. No impact on operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Perimeter Aviation aircraft (C-FFDB) had an air return, because the landing gear was unable to retract. Aircraft returned to base and maintenance replaced the left hand (L/H) gear safety switch.

Incident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG)

Shortly after departure, a Perimeter Aviation flight (PAG11) reported an equipment issue and advised returning to Winnipeg/James Armstrong Richardson Intl, MB (CYWG). The aircraft landed at 0136Z. No impact to operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. A Perimeter Aviation Swearingen SA226-TC Medevac (C-FBTL/PAG11) had an air turnback due to a #2 engine intake heat annunciator that would not illuminate. Annunciator light would test. Aircraft returned and maintenance replaced the #2 engine anti-ice valve.

Incident at NORWAY HOUSE MB (CYNE) (Communication error, Aerodrome - other)

Norway House, MB (CYNE) maintenance vehicle (LO220) was authorized by the flight service station (FSS) to work on Runway 06/24. A Perimeter Aviation Swearingen SA226-TC (C-FSLZ/PAG08) was IFR and inbound from Thompson, MB (CYTH) to Norway House, MB (CYNE). LO220 was instructed by the FSS to exit the runway but did not respond to multiple calls. The FSS then tried to phone airport staff but there was no answer on any of the numbers called. PAG08 elected to do a low pass prior to landing to ensure the runway was clear at 1819Z. PAG08 landed safely at 1826Z. No other impact on operations.
