A SkyWest Airlines, Inc Canadair RJ200 (SKW5296) from San Francisco, CA (KSFO) to Victoria, BC (CYYJ) reported a 20 kt tailwind on approach for Runway 09 and executed a missed approach. The aircraft commenced an approach for Runway 27 and landed safely. No operational impact.

FAA Report September 18: A Jazz Canadair RJ200 Regional Jet (JZA8315) from Calgary, AB (CYYC) to Portland, OR (KPDX) and a Horizon de Havilland DHC-8 (QXE644) came within 500 feet vertical, 0.36 mile lateral separation in the vicinity of Portland, OR (KPDX). JZA8315 responded to a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) and descended to avoid further loss of separation.

Incident at 75 miles S WINNIPEG INTL MB (CYWG)

A SkyWest Airlines Bombardier CL600 2B19 (SKW5238) from Winnipeg, MB (CYWG) to Denver, CO (KDEN) reported a flap failure and advised returning to Winnipeg, MB. No emergency declared. Aircraft rescue and fire fighting (ARFF) on standby as a precaution. SKW5238 landed safely at 1919Z. No impact on operations.
UPDATE: TSB Report#A14C0050: The crew of SkyWest Airlines flight SKW5238, a Canadair CL65 2B19 RJ200 registration N494CA reported that the flaps would not retract from the takeoff setting of 20 degrees after becoming airborne. The crew elected to return to Winnipeg and landed safely with ARFF on standby. Once on the ground the flaps were reset as per the flight information manual and recycled. The aircraft was returned to service per AD2009-06-12 for 10 flight cycles prior to next inspection.

At 0315Z the terminal advised a Jazz Aviation LP Canadair RJ200 Regional Jet (JZA8281) from Prince Albert, BC (CYPR) to Vancouver, BC (CYVR) that they may have a possible blown tire on the right main gear. The aircraft landed safely on runway 26R at 0346Z. Upon runway inspection no debris was found on the runway. Runway 26R was re-opened at 0350Z.

Incident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG)

A Skywest Airlines Bombardier CL 600 2B19 (N477CA/ SKW4479) from Minneapolis/ St Paul, MN (KMSP) to Winnipeg, MB (CYWG) five miles final runway 36, advised tower of a nose gear indication and initiated overshoot. SKW4479 executed missed approach and switched back to arrival. Response equipment was requested. SKW4479 landed safely at 1742Z. No other impact on operations.
UPDATE TSB A13C0115: A Canadair RJ200 (N477CA, operating as SkyWest 4479) was on approach to Winnipeg International Airport. When the crew extended the landing gear they observed a gear disagreement warning which indicated that the nose gear had not extended and was indicating UP. They requested a go-around to troubleshoot. The QRH items were carried out and the alternate gear extension procedure was required to lower the landing gear, after which they had an indication of all wheels down and locked. They then carried out an uneventful landing and taxied to the gate. An emergency was not declared, however ARFF vehicles were standing by as a precaution The systwm was rebooted during a subsequent inspection with gear indications showing normal. The aircraft was returned to service.

A Jazz Aviation Bombardier CL600 (operating as JZA8557) on a IFR flight from Calgary (CYYC) to Victoria (CYYJ) departed runway 28 and at approximately 1 mile off the end reported to Tower that they hit something with their main gear just past runway 34 intersection. A Jazz Aviation De Havilland DHC 8 (operating as JZA8429) on a IFR flight from Calgary (CYYC) to Kamloops (CYKA) was instructed to abort take off runway 28. Staff vehicle inspected runway 28 and found remains of a coyote. Runway cleaned up and re-opened at 0313Z.
UPDATE TSB A13W0021: The Jazz LLP (DBA Air Canada Express) Bombardier CL-600-2B19 (RJ200), C-GJZZ, was operating as flight JZA8557 from Calgary, AB (CYYC) to Victoria, BC (CYYJ). During the take-off roll on Runway 28 the aircraft collided with a coyote just west of the intersection of Runway 34. There was no damage to the aircraft.

The first Westjet Boeing 737 700 aircraft (operating as WJA1228) was on a scheduled IFR flight from Toronto (CYYZ) to Montego Bay (JM) (MKJS). The second Westjet Boeing 737 700 aircraft (operating as WJA1116) was on a scheduled IFR flight from Toronto to Orlando (US) (KMCO). The Jazz Air LP Canadair RJ200 Regional Jet aircraft (operating as Air Canada Express flight JZA7920) was on a scheduled IFR flight from Toronto to Charlotte (US) (KCLT). The Sunwing Airlines Inc. Boeing 737 700 aircraft (operating as SWG420) was on a scheduled IFR flight from Toronto to Punta Cana (MX) (MDPC). While all these aircraft were taxiing in the south half of CYYZ, the ground controller mistook the positions of WJA1228 and WJA1116. This led to the ground controller giving taxi instructions that caused JZA7920 and SWG420 to cut off WJA1228 and cause a risk of collision on the ground. Ops. impact -- none.

AWI3706, RJ200, was on the taxiway in Halifax (CYHZ) after exiting the arrival runway and advised that they were stopping due to a passenger that had not remained seated. The aircraft was able to resume taxiing after a couple of minutes. One other aircraft was provided an alternate taxi route as a result.

Incident at CHARLOTTETOWN AIRPORT (ATM - ILS irregularity)

FLG4292, RJ200, enroute from New York (KJFK) to Charlottetown (CYYG), attempted two approaches on Runway 21. On both attempts the flight conducted missed approaches approximately five nautical miles final. When the Area Control Centre (ACC) vectored the aircraft for the second approach, the flight crew reported that they had issues with the localizer and they were not going to try another approach. There was some convective activity southwest of the airport along with a few flashes of lightning. There was no other operational impact.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ)

JZA822, RJ200, enroute from Ottawa (CYOW) to Halifax (CYHZ), was vectored for the CAT II ILS on Runway 23. The flight executed a missed approach and rather than carrying out the published missed approach procedures, the aircraft turned to the right. When queried, the pilot reported that the aircraft turned right when he selected the go around sequence in the flight management system. Although the weather was poor at the time, the flight conducted the missed approach due to problems with the aircraft?s navigation system. There was no other traffic in the area at the time. The flight crew elected to divert to Fredericton (CYDF). There was no impact on operations.

JZA805, DHC 8 102, enroute from Sydney (CYQY) to Halifax (CYHZ), was cleared for a RNAV approach to Runway 32. JZA631, RJ200, enroute from St. John?s (CYYT) to Halifax, was cleared for an ILS approach for Runway 23. JZA805 conducted a missed approach to Runway 32. Separation between the two aircraft was reduced to 2.1 nautical miles before vertical separation was established.

Incident at VANCOUVER INTL BC (CYVR) (Conflict - potential, Conflict - loss of separation, Communication error, ATS operating irregularity)

UPDATE from TSB; A11P0140: CYVR Vancouver - JZA440 a CRJ2, was on a visual approach to 26R behind PAL106 a A343. The pilot of JZA440 switched to tower frequency before being instructed to. The controller tried to contact JZA440 to reduce speed as it was faster than the aircraft ahead. Before contact was established JZA440 entered the 6 nm wake turbulence protected area. The controller did not advise the JZA440 of possible wake turbulence once contact was made. The aircraft did not encounter wake turbulence.
UPDATE from Vancouver ACC: JZA440 was turned final for an ILS runway 26R approach 7nm in trail of PAL106. The controller attempted to issue speed reductions to JZA440 to maintain separation. However, JZA440 had changed to the tower frequency on his own. Separation behind PAL106 reduced to 4.5 nm at touchdown.
The Air Canada Jazz Canadair RJ200 operating flight JZA440, IFR Whitehorse (CYXY) to Vancouver (CYVR), was turned final for an Instrument Landing System (ILS) approach runway 26R 7nm in trail of the Philippine Air Lines Boeing 747-400 flight PAL106, IFR Manilla (RPLL) to CYVR. As separation was starting to reduce the controller gave JZA440 speed reductions to maintain separation. JZA440 had left the frequency on his own and separation behind PAL106 reduced to 4.5 nm at touchdown.

UPDATE from NavCanada Wake Vortex Encounter Report: Vertical Spacing 1,000', Horizontal Spacing 2.5 nm. The Air Canada Jazz Canadair RJ200 (CRJ2) was on a visual departure behind the Alaska Airlines Boeing 737 800. The pilot requested a turn to get out of the bumps. The contoller turned the CRJ2 10 degrees left and the pilot was happy.
Air Canada Jazz, Canadair RJ200, operating flight JZA209, IFR Vancouver (CYVR) to Prince Georger (CYXS), departed Runway 08R on a visual VANCOUVER SIX DEP behind Alaska Airlines flight ASA706, a Boeing 737 800, IFR CYVR to Los Angeles, CA USA (KLAX), and reported wake turbulence from the traffic ahead. JZA209 was given a 10 degree turn at 3,000 feet to avoid the turbulence. No other traffic was affected.
