A Government of Quebec Canadair CL-600-2B16 (QUE10) on a MEDEVAC flight from Bagotville, QC (CYBG) to Quebec/Jean Lesage, QC (CYQB) was assigned the runway in use, namely Runway 29, by the en route controller. QUE10 flew the standard terminal arrival (STAR) of SIMTO 4 for Runway 24 without approval. No impact on operations.

Incident at 5NM N FLEUR intersection

Despite having been authorized and correctly reading back FLEUR direct CYGV, a Gouvernement du Québec Bombardier CL-600-2B16 (QUE10) on a MEDEVAC flight from Quebec/Jean Lesage Intl, QC (CYQB) to Havre St-Pierre, QC (CYGV) flew FLEUR direct ML. Air traffic services (ATS) corrected the situation. No impact to operations.

Incident at 6NM QUÉBEC / JEAN LESAGE INTL QC (CYQB)

Following takeoff from Quebec/Jean Lesage, QC (CYQB), the pilot of a Government of Quebec Canadair CL600-2B16 (C-GURG/QUE10) on a MEDEVAC flight from Quebec/Jean Lesage, QC (CYQB) to Val-dOr, QC (CYVO) reported a problem with the landing gear system. The aircraft made the decision to return to CYQB, where vehicles had been deployed as a precaution. QUE10 landed at 0248Z without incident, no impact on operations.
UPDATE TSB Report #A20Q0069: C-GURG, a Canadair CL60 operated by the Quebec Government Air Service, which operated flight QUE10 under instrument flight rules, had just taken off from Quebec City/Jean-Lesage, QC (CYQB) for a flight to Val dOr, QC (CYVO). When selecting the landing gear in the retracted position, the nose wheel did not retract. The Quick Reference Manual (QRH) checklist had been completed and the landing gear was successfully deployed. The crew decided to return to CYQB and, as a precaution, requested that the Aircraft Rescue and Fire Fighting Services (ARFF) unit be deployed. The aircraft landed on the runway without incident. An inspection revealed the presence of a white cloth, which masked the gear position sensors.

A Government of Quebec Canadair CL600-2B16 (series 601-3R) (C-GURG/QUE10) from Mont-Joli, QC (CYYY) to Quebec/Jean Lesage, QC (CYQB) conducted a missed approach on Runway 24 due to an instrument problem. The aircraft received and correctly read back the instruction to turn left on a heading of 150 and to climb to 3 000 ft. Impact: separation was reduced to 2.7 NM and 800 ft with the aircraft that preceded it due to late execution. No other impact on operations.
UPDATE TSB #A20Q0019: C-GURG, a Service aérien gouvernemental Canadair CL-600-2B16 (QUE10), was on an instrument flight rules (IFR) flight from Mont-Joli, QC (CYYY) to Jean-Lesage, QC (CYQB). C-GDTL, a Panorama Fixed Wings Pilatus (NDL543), was on an instrument flight rules (IFR) flight from CYQB to Wabush, NL (CYWK). QUE10 initiated a missed approach RWY 24 due to an instrument problem. It received and correctly read back the instruction to turn left on a heading of 150 degrees and to climb to 3 000 feet. When it reached 2 900 feet in altitude, separation with NDL543, which had taken off before it, was lost. Separation was reduced to 2.7 nautical miles (NM) horizontally and 800 feet vertically, while 3 NM horizontally or 1 000 feet vertically was required between the 2 aircraft. No injuries or damage.

Incident at 65NM SW PUVIRNITUQ QC (CYPX)

An Air Inuit Ltd. Boeing 737-2Q2C (C-GMAI/AIE704) from La Grande-Rivière, QC (CYGL) to Puvirnituq, QC (CYPX) level at FL280 was cleared to leave controlled airspace below FL180 and was instructed to confirm that it left FL180. AIE704 reported having left FL180. The air traffic controller (ATC) contacted AIE704 to confirm that the aircraft was going to leave FL180, and the pilot confirmed. ATC cleared a Government of Quebec Canadair CL600-2B16 (Series 601-3R) (C-GURG/QUE10) from Puvirnituq, QC (CYPX) to Montreal/Trudeau, QC (CYUL) to climb to FL240 (aircraft travelling in the opposite direction) with an information exchange on the aircraft. At that moment, AIE704 said that it was leaving FL280. ATC corrected the situation and gave new clearance to QUE10 to stop its climb to 17 000 ft. No loss of separation.

The aircraft did not declare an emergency but did request that emergency services be ready to intervene. The aircraft landed without incident at 1840Z. No impact on operations.
Update: TSB Report #A19Q0200: C-GURG, a Service aérien gouvernemental du Québec CL60 (QUE10 MEDEVAC), had just taken off from Chibougamau/Chapais, QC (CYMT) for Val d’Or, QC (CYVO) when the flight crew received a FLAP FAIL indication and noticed that the flaps were stuck at 20 degrees. The flight crew requested to be diverted to Quebec/Jean Lesage, QC (CYQB) and requested the presence of emergency services for its landing. The landing took place without incident. Maintenance staff reinitialized the flap system and the aircraft was returned to service.

TSB Report #A19Q0004: C-GURG, a Service aérien gouvernemental du Québec Bombardier CL600-2B16 Challenger 601-3R, was on medical evacuation (MEDEVAC) flight QUE10 from Quebec/Jean Lesage Intl, QC (CYQB) to Gaspé (Michel Pouliot), QC (CYGP) with 2 crew members and 2 passengers on board. When the up position of the landing gear was selected after departure from CYQB, a WOW I/FAIL message appeared in the cockpit and the nose gear did not retract. An emergency was declared and the aircraft returned to CYQB and landed without incident. The maintenance crew returned the aircraft to service after they had replaced an electrical harness on the nose gear.

A Government of Quebec Canadair CL600-2B16 Series 601-3R (C-GURG/QUE10) from Chibougamau/Chapais, QC (CYMT) to Montréal/Pierre-Elliott-Trudeau, QC (CYUL) at 6000 feet, had to climb to 6300 feet to avoid the aircraft squawking transponder code 1200 at 5600 feet. The air traffic service (ATS) exchanged information on the aircraft several times with QUE10.
TSB Update #A18Q0165: C-GURG, a Service aérien gouvernemental du Québec Bombardier CL600-2B16 Challenger 601-3R, was on IFR flight QUE10 from Chibougamau/Chapais, QC (CYMT) to Montreal/Pierre-Elliott-Trudeau, QC (CYUL). While the aircraft was near UFX (Saint-Félix de Valois, QC) with an altitude restriction of 6 000 ft while passing UFX, the crew received a Traffic Alert and Collision Avoidance System (TCAS) advisory, which indicated an aircraft flying at an altitude of 5 000 ft and climbing. A few seconds later, a resolution advisory (RA) was issued; it advised the crew to climb. The crew followed the directions of the TCAS and immediately initiated a climb to an altitude of 6 500 ft. A traffic advisory had been transmitted by air traffic control (ATC) for traffic on a VFR flight at an altitude of 5 600 ft.

QUE10 declared an emergency. Coordination provided. QUE10 landed safely. Impact: Site managers of CYQB, CYUL and Ottawa/MacDonald-Cartier Intl (CYOW), ON, notified, and a Jazz de Havilland DHC-8-402 (JZA7962) and a WestJet Encore de Havilland DHC-8-402 (WEN3524) did circuits before landing. National Operations Centre (NOC) and Aviation Operations Centre notified.

TSB Report No. A18Q0049: C-GURG, a Bombardier CL600-2B16 Challenger 601 aircraft operated by the Government of Quebec Air Service, flying QUE10 under instrument flight rules from Montréal/Pierre Elliott Trudeau Intl, QC (CYUL) for a Medevac flight to Quebec/Jean Lesage Intl, QC (CYQB). During the initial climb after take-off, a muffled noise was heard, accompanied by a significant and abnormal vibration of the right engine, followed by an indication of internal turbine temperature exceeding limits. The crew closed the engine (General Electric CF34-3A1) and returned to CYUL. The aircraft landed without issue with Aircraft Rescue and Fire Fighting (ARFF) on alert. There were no injuries and the aircraft was not damaged.

A few moments later, the aircraft requested to be redirected with priority to Val-d'Or, QC (CYVO), QC, due to the patient?s worsening condition. The aircraft landed without incident. No impact.
UPDATE from the operator: The operator confirmed that the aircraft experienced a flap failure and diverted to Val-d'Or, QC (CYVO). There was no medical issue reported with the Medevac patient.

At 1445Z, a Government of Quebec Canadair CL-600-2B16 Medevac (C-GURG/QUE10) on a flight from Quebec/Jean Lesage, QC (CYQB) to Puvirnituq, QC (CYPX) declared an emergency after takeoff due to a gear issue. The aircraft returned and landed safely in CYQB at 1456Z. No impact on operations.
UPDATE: BST#A17Q0175: C-GURG, a Service aérien gouvernemental du Québec CL-600-2B16 (601-3R series), was on instrument flight rules (IFR) flight QUE10 from Quebec/Jean Lesage, QC (CYQB) to Puvirnituq, QC (CYPX). During the initial climb and after the landing gear was retracted, the green light for the left main landing gear stayed on, indicating a lowered and locked wheel. The flight crew declared an emergency and returned to land at CYQB, where emergency vehicles were at the scene. Maintenance workers from the operator replaced the two lock indicators, which displayed a lowered position, of the hydraulic brakes that activate the main landing gear.

A WestJet Encore de Havilland DHC-8-402 (C-GWEF/WEN3411), from Québec/Jean Lesage (CYQB), QC, to Toronto/Lester B. Pearson (CYYZ), ON, reported that it struck a bird on takeoff from Runway 29. The aircraft rescue and firefighting (ARFF) service was notified. A seagull carcass was found on the runway. A Quebec government Bombardier CL-600-2B16 (QUE10) on arrival and an Air Inuit de Havilland DHC-8-300 (AIE833) on departure were slightly delayed.

Shortly after takeoff from CYQB, the ?Outboard Anti-Skid? message appeared. The crew conducted the emergency checklist, prepared the cabin and declared an emergency. After burning excess fuel in order to obtain the maximum landing weight, the aircraft landed without incident on Runway 29 and came to a stop to enable firefighters to verify the brakes. The aircraft then taxied to the hangar with firefighter escort.
After taking off from Québec/Jean Lesage (CYQB), QC, a Quebec government Canadair Cl-600-2B16 (C-GURG/QUE10), from Québec/Jean Lesage (CYQB), QC, to Rouyn-Noranda (CYUY), QC, declared that it wanted to return to CYQB because of a brake problem. Emergency declared and assistance requested. Impact: Given that there was only one runway in service at CYQB, departures from CYQB were halted between 2242Z and 2335Z, which delayed four aircraft by about 36 min. QUE10 landed without incident at 2327Z. No other impact on operations.

The aircraft conducted a low altitude pass near the tower for a check and returned to land without incident on Runway 24 at 0859Z. No impact on operations.
Update TSB #A17Q0011: A Quebec government Challenger 600 (C-GURG/ QUE10) was on a MEDEVAC flight from Bagotville (CYBG), QC, to Québec/Jean Lesage Intl Airport (CYQB). During the final approach, the crew declared an emergency due to an improper landing gear indication. The crew conducted a pass near the tower for verification and returned to land without incident.

A Quebec government Canadair CL600-2B16 (C-GURG/QUE10) MEDEVAC, from Québec (CYQB), QC, to CYVB, and from CYVB to Rimouski (CYXK), QC, had to call on the ground to close its flight plan. On takeoff, QUE10 had to contact the Montréal centre in flight to obtain its IFR authorization.

A Quebec government Service aérien gouvernemental Canadair CL-600-2B16 MEDEVAC flight (QUE10), from Québec (CYQB), QC, to Kuujjuaq (CYVP), QC, declared an emergency because of a landing gear problem and asked to return to CYQB. Emergency measures were deployed. QUE10 conducted a low altitude pass over Runway 29 and later landed without incident on Runway 24 at 0320Z. The aviation operations centre was notified. No impact.
Update (Maintenance): A civil aviation safety inspector (CASI) looked into this incident. The CASI communicated with the operator to obtain more information. The problem was caused by a plunger that remained stuck. The CASI was satisfied with the information provided and indicated that no other follow-up by Transport Canada was required for the time being.
