A Provincial Airlines de Havilland DHC-8-311 (C-GVPA/PVL901) from St. John's, NL (CYYT) to Deer Lake, NL (CYDF) conducted a missed approach Runway 07 due to a flight management system (FMS) indication at 1120Z. Switched back to Gander, NL (QX) air traffic control (ATC). Flight arrived at CYDF Runway 07 at 1205Z.

Incident at DEER LAKE NL (CYDF)

A Provincial Airlines de Havilland DHC-8-315 (C-FPAE/PVL901), from St. John's, NL (CYYT) to Deer Lake, NL (CYDF) cancelled IFR at 1058Z before entering the control zone and advised they were staying to the southeast over Grand Lake at A012 to check on an issue. No assistance was required. At 1103Z, the aircraft advised that the issue had been resolved and subsequently landed Runway 07 without incident at 1110Z.

Incident at WABUSH NL (CYWK)

A Provincial Airlines de Havilland DHC-8-311 (C-GVPA/PVL901) from Goose Bay, NL (CYYR), to Wabush, NL (CYWK), landed safely at 1355Z. The pilots went into the flight service station (FSS) at 1510Z and advised of a flat tire. A vehicle (ST22) did a complete runway check and advised no foreign object debris (FOD) on runway at 1534Z.

Incident at WABUSH NL (CYWK) (FOD (foreign object debris))

At 1435Z, Flight Service Station (FSS) received phone call from Provincial Airlines staff requesting Transport Canada perform a runway check for foreign object debris (FOD). The caller advised a North Cariboo Flying Service Beech 1900D (NCB903) from Charlo, NB (CYCL) to Wabush, NL (CYWK) discovered a loose flap after arrival at Wabush, NL at 1422Z. Transport Canada completed a runway check and advised no FOD on runway. A PAL Airlines de Havilland DHC-8-102 (PVL901) from Goose Bay, NL (CYYR) to Wabush, NL (CYWK) arrival was delayed approximately 3 minutes to accommodate runway check.

Incident at WABUSH NL (CYWK) (FOD (foreign object debris))

A Provincial Airlines de Havilland DHC-8-102 (C-FDND/PVL901) from Goose Bay, NL (CYVR) to Wabush, NL (CYWK) reported striking several small birds when passing over the threshold for landing on Runway 36. The remains of fourteen birds were removed from the runway near the threshold. No impact on operations.

A Provincial Airlines de Havilland DHC-8-315 (C-FPAE / PVL901) from St. John's, NL (CYYT) to Deer Lake, NL (CYDF) was restricted to hold short of Taxiway ?F" and was observed shortly after moving onto Taxiway ?F" and then crossing Runway 02/20. No impact to operations.

Per Airport Duty Manager, departures from Terminal were held due to a security matter. Normal operations resumed at 0950Z. The following aircraft were affected: WestJet (WJA659) Proposed 0920Z / Departed 1040Z. PAL Airlines (PVL901) Proposed 0930Z / Departed 1003Z. Air Canada (ACA691) Proposed 0930Z / Departed 1005Z.

Incident at ST. JOHN'S INTL NL (CYYT) (Aerodrome - visual aids)

Failure of all aerodrome lighting from 0932Z-1000Z. Low Visibility Operational Plan (LVOP) were in effect (visibility 1/4SM fog, VV001, some runway visual ranges (RVRs) below 1200). Impact: WestJet (WJA659), PAL Airlines (PVL901), Air Canada (ACA691), Jazz (JZA8991) all delayed approximately 30 minutes. Cougar Helicopters (CHI31) and PAL Aerospace (SPR03) shut down, delayed indefinitely. Lights restored at 1000Z but CAT II still out of tolerance. A WestJet Boeing 737 700 (WJA164) from Stephenville, NL (CYJT) to St John's, NL (CYYT) missed approach on CAT I approach runway 29 at 1032Z.

Incident at DEER LAKE NL (CYDF)

UPDATE: TSB Report#A14A0046: The Provincial Airlines Dash-8 (C-GYCV) aircraft was taxiing for departure and was asked to hold short the active runway. The Skylink Express B1900 (C-GSKN) aircraft was on approach to landing. The Dash-8 rolled across the hold short line on Taxiway A and stopped after being advised by ATC of the aircraft on final. The landing aircraft landed without incident.
A PAL Airlines de Havilland DHC 8 300 (PVL901) from Deer Lake, NL (CYDF) to Goose Bay, NL (CYYR) was given a departure advisory for runway 07. The aircraft was requested to hold short as a Beech 1900 was on final runway 07. The aircraft was observed taxiing onto taxiway A and proceeding past the hold short line. The pilot was then advised to hold short due to aircraft on final. The aircraft stopped past the hold short line. Skylink Express (SLQ601) landed runway 07 at 1057Z, without incident. PVL901 departed at 1059Z.

A truck (Grader150) was stuck on runway 29/34 intersection from 0930Z-0938Z. Cargojet (CJT620) on approach runway 29, was held at TESOX for 8 minutes with Gander, NL (CYQX) area control centre (ACC). PAL Airlines (PVL901) and WestJet (WJA435) were delayed 8 minutes on departure.

Incident at DEER LAKE NL (CYDF)

UPDATE: TSB REPORT#A13A0083: The Provincial Airlines DHC-8 (registration C-FPAE) aeroplane was cleared an Instrument Flight Rules (IFR) approach at Deer Lake (CYDF). At the same time an EVAS BE1900 was departing for St John's (CYYT). The EVAS flight was cleared a Visual Flight Rules (VFR) departure. On descent in cloud the DHC-8 received a TCAS "Traffic Traffic" advisory" and deviated to the right for the outbound traffic and continued descent to ensure adequate separation. IFR was cancelled once below cloud and the flight landed safely.
A PAL Airlines Ltd. de Havilland DHC-8-315 (C-FPAE / PVL901) was inbound to Deer Lake (CYDF) from St. John's, NL (CYYT) when it recieved and followed a resolution advisory. Traffic was an outbound Exploits Valley Air Services Ltd. Beech 1900, from CYDF to CYYT, that departed and was operating under visual flight rules. There was no further impact and PVL901 cancelled IFR at 1019Z.

Incident at ST. JOHN'S INTL NL (CYYT) (Weather - visibility, ATM - NAVAIDS/radar)

Low visibility operational Plan (LVOP) was not authorized, airport surface detection equipment (ASDE) was unserviceable. Marginal IFR limits. Aircraft movements were restricted to one aircraft at a time on the manoeuvring areas, which results in numerous delays 5-25 min. Affected flights Air Canada (ACA629), Westjet (WJA659), PAL Airlines (PVL913), Air Canada (ACA691), Jazz Aviation (JZA8991), Jazz Aviation (JZA8829), PAL Airlines (PVL901), PAL Airlines (PVL301), Westjet (WJA547), Air Canada (ACA113).

Incident at ST. JOHN'S INTL NL (CYYT)

UPDATE TSB: A12A0123: The Provincial Airlines Saab 340A aircraft (registration C-FPAG), was operating as PVL901 on a flight from St. John's, NL (CYYT) to Deer Lake, NL (CYDF). While en-route to CYDF, the crew noticed the #2 engine oil temperature indicating higher than normal. The crew continued to monitor the oil temperature and elected to shut down the engine (General Electric CT7-5A2) when the oil temperature indication increased into the red arc. The crew shut down the engine in accordance with checklist procedures and declared an emergency with Gander Centre. The aircraft landed at CYYT on runway 11 without further incident and with airport emergency services standing by. Company maintenance inspected the aircraft and found the engine oil temp sensor probe to be faulty. The sensor was replaced and the aircraft returned to service after ground runs were successfully completed.
Just after departure, PVL901, Saab SF 340A, enroute from St. John?s (CYYT) to Deer Lake (CYDF), requested, and was cleared, to return to St. John?s. No assistance was required. Shortly afterwards the pilot declared an emergency due to the shutdown of the number-2 engine and requested Airport Rescue and Fire Fighting (ARFF) services. The aircraft landed without incident at 11:01Z. TSB Case Closed.
