Incident at OTTAWA / GATINEAU QC (CYND)

C-GOXI, a privately owned Piper PA3, was on an IFR flight from Qu├ębec/Jean-Lesage (CYQB) to Gatineau (CYND). During RNAV approach for Runway 27, the aircraft was observed at an altitude of 2 000 ft while the allowed altitude was 3 100 ft. No impact on ATC operations.

Incident at TOFINO / LONG BEACH BC (CYAZ)

The Pilot of Orca Airways Flight LZ107, a Piper PA3 (C-FRDN) IFR Vancouver to Tofino, reported a bird strike over the threshold upon landing Runway 29 at Tofino. There was no damage and staff were unable to locate remains.
UPDATE from Aerodrome Safety: No further action required. The Tofino Airport has a wildlife program in place meeting the regulatory requirement of CAR 302.

Incident at 30 NM Northwest of Vancouver

Canadian Air Charters Flight AX422, a Piper PA3 IFR Prince George to Vancouver, was cleared to and read back 9,000 ft on descent 30 NM Northwest of Vancouver, and descended to 8,000 ft in a 9,000 ft vectoring area. The aircraft was re-cleared to 9,000 ft and told to expedite the climb.

UPDATE / Add Info from TSB: A08P0056: An Integra Ops (dba Canadian Air Charters) PA31, Flight AX422, was on an IFR flight from Prince George to Nanaimo. At 30 NM NW of Vancouver the aircraft was issued and read back a clearance to descend to 9,000 ft. The aircraft was observed descending to 8,000 ft in a 9,000 ft vectoring area. The aircraft was re-cleared to 9,000 ft and told to expedite the climb.

April 1, 1995

Incident at St. Andrews, Manitoba

C-FXVW, PA3, was flying circuits at St. Andrews. During a full stop landing on Runway 36L, the aircraft ran off the east side of the runway. No injuries, no damage.

Accident at PRIVATE STRIP

PA3 CUB TRAINER C-FKKZ - WHILE TRYING TO LAND THE SKI EQUIPPED A/C ON A SNOW COVERED PRIVATE STRIP NEAR ORILLIA UNDER OVERCAST AND LIGHT SNOW WX CONDITIONS, THE PILOT MOMENTARILY LOST VISUAL REFERENCE AND MADE A HARD LANDING. THE RIGHT MAIN GEAR COLLAPSED AS A RESULT OF THE CROSS BRACE BREAKING AND THE A/C CAME TO REST UPRIGHT WITH THE LANDING GEAR, PROP, AND RIGHT WING DAMAGED. TIME = 2200Z. ACTION = PAH, PAR, TSB.
