Incident at IQALUIT NU (CYFB)

KBA 311, a DHC-6 Twin Otter operated by Kenn Borek Air, departed Iqaluit at 1308z on a VFR flight to Kimmirut on a company note. FSS gave an advisory to the crew which included a ceiling of 600 feet, visibility 20 SM. Within approximately one mile of the airport after departure, the aircraft was observed climbing through cloud in the left turnout. There was no conflicting IFR traffic at the time. Upon return from Kimmirut, the aircraft landed on Runway 35 and the flight crew was again given the visibility and low ceiling. The crew used a simulated ILS approach to Runway 35 and landed at 1504z. Incident at ROUYN-NORANDA QC (CYUY)

Un Helio Courrier de propriété privée effectuait un vol selon les règles de vol à vue (VFR) depuis le lac Joncas à destination du lac Osisko. Malgré avoir été informé par le spécialiste d'information de vol (FSS) que les conditions météorologiques étaient inférieures aux minimums permettant le vol selon les règles de vol à vue, l'appareil est quand même entré dans la zone de Rouyn-Noranda (CYUY) sans autorisation spéciale VFR. Il n'y a eu aucun impact sur les opérations ATC. Les conditions météorologiques présentent à Rouyn-Noranda à 1422Z étaient: Vents du nord - nord-ouest à six noeuds, visibilité quinze milles terrestres, plafond fragmenté à 500 pieds, couvert à 1,200 pieds. Remarque: stratus fractus 7/8, strato-cumulus 1/8. (SPECI CYUY 311422Z 34006KT 15SM BKN005 OVC012 RMK SF7SC1)*** ** *** A privately-owned Helio Courrier was conducting a visual flight rules (VFR) flight from lac Joncas to lac Osisko. Despite being informed by the flight service specialist that the weather conditions were below the minima allowed for VFR flight, the aircraft still entered the Rouyn-Noranda (CYUY) area without special VFR authorization. There was no impact on ATC operations. The weather conditions at Rouyn-Noranda at 1422Z were: winds from the north-northwest at 6 kt, visibility 15 SM, broken ceiling at 500 ft, covered at 1 200 ft. Incident at GANDER INTL NL (CYQX) (Aerodrome, runway or taxiway shutdown)

UPDATE TSB: A07A0135: The Volga-Dnepr Antonov AN-124, operating as VDA4308, landed on Runway 03 at Gander International Airport, NF at 0222 NST. Weather at the time was reported to be rain and drizzle with patchy fog. Approximately 30 minutes after the aircraft taxied to the ramp airport maintenance personnel advised that there were three runway edge lights broken in the vicinity of taxiway Alpha. The Airport Duty manager that the left main landing gear contacted the runway edge lights however the aircraft did not sustain any damage. A NOTAM was issued and Runway 03/21 was closed to facilitate repair of the lights; the runway was reopened at 04:35 NST.
At 05:52Z, VDA4308, Antonov AN-124, enroute from Philadelphia (KPHL) to Gander (CYQX), landed on Runway 03. At 06:28Z, airport maintenance advised the tower that there were three runway edge lights broken. Runway 03/21 was closed under a NOTAM. Airport maintenance fixed the runway lighting and reopened the runway at 08:05Z. The airport duty manager advised that the aircraft was inspected and there was no damage found. TSB Update Dec 19, 2001. A01A0147: EIA6535, A BOEING 747-200 EN ROUTE FROM HELSINKI TO GANDER, LANDED ON RUNWAY 31 AFTER COMPLETING A LOCALIZER BACK COURSE. THE AIRCRAFT TOUCHED DOWN AT 12:24 NST NEAR THE INTERSECTION OF RUNWAYS 31 AND 22, WITH THE NOSE WHEEL TOUCHING DOWN NEAR TAXIWAY ECHO. THE AIRCRAFT WEIGHT WAS APPROXIMATELY 600,000 LBS, NEAR THE MAXIMUM LANDING WEIGHT. THE CREW HAD SELECTED WHEEL BRAKING TO MINIMUM FOR LANDING, AND THRUST REVERSERS WERE DEPLOYED AFTER TOUCHDOWN. INITIAL STOPPING PERFORMANCE WAS GOOD, AND THE REVERSERS WERE STOWED AT 80 KTS AS PER COMPANY SOP. DURING THE LAST PORTION OF THE ROLL THE CREW SENSED THAT STOPPING PERFORMANCE HAD DETERIORATED SIGNIFICANTLY. THRUST REVERSERS WERE RE-DEPLOYED, AND MAXIMUM BRAKING WAS APPLIED. THE AIRCRAFT CAME TO A FULL STOP ON THE BLAST PAD OF THE BUTTON OF RWY 13, WITH THE NOSE GEAR JUST CLEAR OF THE CLEARED PORTION. Incident at Halifax Intl

On landing at Halifax, ICN 2212 FA28 IFR Ottawa to Halifax, the pilot reported that the aircraft exited Runway 33 in the vicinity of taxiway Bravo. The aircraft re-entered the runway under its own power and taxiied to the ramp.The pilot later advised that an inspection of the aircraft revealed no damage. The airport foreman advises that there was no damage to property on the ground, and no debris. TSB evaluating. #A99A0057 Update: THE CREW OF ICN2212 WAS FLYING A VECTORED BACK COURSE APPROACH TO RUNWAY 33 WITH THE FIRST OFFICER AT THE CONTROLS. WINDS WERE GUSTY AND HEADING CHANGES FROM TEN DEGREES RIGHT TO FIVE DEGREES LEFT WERE REQUIRED FOR CORRECTION. AT MDA AND 2 DME THE CAPTAIN CALLED THE FIELD IN SIGHT.
