Incident at TROIS-RIVIÈRES QC (CYRQ)

Aviation incident report #15220: A Flexjet Embraer EMB-545 (N413FX/LXJ413) from Memphis, TN (KMEM) to Trois-Rivières, QC (CYRQ) landed without contacting Universal Communications (UNICOM) on mandatory frequency (MF) 123.00 in IFR conditions (SPECI CYRQ 152038Z AUTO 00000KT 3/4SM BR OVC003 00/M01 A2950 RMK SLP994=). Number of passengers unknown. There were no vehicles on the runway during landing, but several were taxiing on the service roads.

Incident at SUDBURY ON (CYSB)

A Government of Canada, Royal Canadian Mounted Police, Aerospatiale AS350 B3 (C-GMPK) on a flight to Sudbury, ON (CYSB), made its initial call 4 miles SE. Weather was below VFR. Ceiling OVC003 2SM -DZ BR. SVFR was given to the aircraft and it landed without incident. No impact on operations.

Incident at ST. JOHN'S INTL NL (CYYT)

A Government of Newfoundland and Labrador Beech B300 Medevac (C-GNLF) on a flight from Goose Bay, NL (CYYR) to St. John's, NL (CYYT) went into the missed approach for Runway 11 due to being unable to pick up the glide slope. A subsequent arrival reported no error with the glide slope. C-GNLF returned and landed safely Runway 11 on their second approach at 1829Z. Weather at the time of the missed approach was 3NM Visibility, -RA BR OVC003. All runway visual ranges (RVRs) greater than 6000.

Incident at CALGARY INTL AB (CYYC)

Update Maintenance & Manufacturing: The overshoot was due to the pilots getting behind the aircraft on the approach. The approach was apparently down to minimums and they decided to overshoot. There was not mechanical failures or problems with the aircraft. The aircraft registration is C-GXHG.
A Bar XH Air Beech 200 (BXH353M) from CYXH to CYYC overshot runway 16 over the threshold at 0254Z due to an issue inside the aircraft. WX 2SM -SN BR OVC003. No ARFF requested. BXH353M landed runway 16 at 0307Z. No impact on operations.

Incident at YELLOWKNIFE NT (CYZF) (Aerodrome - visual aids)

An Air Tindi aircraft, flight AT801, DeHavilland DHC 7 on an IFR flight from Diavik to Yellowknife was 1nm on final for runway 34, with tower VSBY 1sm -SN ceiling OVC003. Airport power switched from back-up to regular power and all runway and approach lights failed for 3-5 seconds. Lighting returned to normal when the aircraft was within a half mile final.

Incident at SEPT-ÎLES QC (CYZV) (Aerodrome, runway or taxiway shutdown)

C-GQZE, a Groupe Normandin Piper Navajo, was on an IFR flight from Québec/Jean-Lesage (CYQB) to Sept-Îles (CYZV). Upon landing on Runway 09, the aircraft went off the runway. The aircraft was not damaged and no one was injured. Runway 09-27 was closed until an inspection could be done.
Update #1: Event time specified. According to information from NAV CANADA: Upon landing on Runway 09, the aircraft went approximately 150 ft off the end of the runway and the front wheel got stuck. The runway was closed for 28 min, causing arrival delays for three aircraft and departure delays for two aircraft. The Transportation Safety Board was notified. Update #2: The TSB occurrence number and class of investigation were added. Event "Weather - Visibility" added. According to TSB report #A11Q0071: A privately owned Piper Navajo PA-31, registered C-GQZE, was on an IFR flight between Québec/Jean-Lesage (CYQB) and Sept-Îles (CYZV). Upon landing on Runway 09, the aircraft went approximately 150 ft off the end of the runway and the front wheel got stuck. The aircraft was not damaged and no one was injured. At the time of the event, the visibility varied from de ¼ to ½ mi. in the fog.

The flight climbed to 2,200 ft. and held between EMOSI and EMPEN. The pilot subsequently advised of attempting another AR10 approach and at time, 15:00Z, advised in another missed approach. The pilot of NRL52, a Convair operated by Nolinor conducting an IFR flight from Mirabel (CYMX) to Val d'Or (CYVO), reported that he had the Val d'Or runway in sight and it was 10 NM on final at approximately 2 500 ft when the automated weather observation system (formerly AWOS) reported a visibility of 3 1/2 SM and a cloud-base height of 300 ft (SPECI CYVO 030806z AUTO 19003kt 3 1/2sm OVC003 A2990).

QUE10, un Challenger du Service aérien gouvernemental du Québec, effectuait un vol d'évacuation médicale depuis Québec/Jean-Lesage (CYQB) à destination de Kuujjuaq (CYVP). En raison des conditions météorologiques, l'équipage a effectué une première approche interrompue pour la piste 07 à 0327Z ainsi que deux autres pour la piste 31 à 0336Z et 0346Z avant de repartir pour Québec/Jean-Lesage. Le système d'atterrissage aux instruments (ILS) de la piste 07 était hors de service lors de l'événement. Incident at FORT MCMURRAY AB (CYMM) (ATM - weather observation systems, ATM - inaccurate aeronautical information)

Peace River FSS reported that an aircraft taxied for departure at Fort McMurray while the AWOS was reporting 1/8 SM BKN001 OVC003 OVC090. However, FSS received several reports that the AWOS was reporting inaccurately, that the sky was clear and there was 5-9 miles visibility. Additionally, Special VFR was approved. The pilots of aircraft on the ground and in the air reported conditions to be VFR.
