Incident at 15 NM South of CYCH (Conflict - loss of separation, ATS operating irregularity)

NWA66, Airbus 330-300 enroute from Detroit (KDTW) to Rome (LIRF) at 37,000 ft. proceeding MIILS COLOR. NWA66 was cleared at 0132Z to climb to 39,000 ft. due to turbulence. BAW183, Boeing 747-400, enroute from London (EGLL) to New York (KJFK) proceeding CYAY TOPPS at 38,000 ft. At 1035Z, separation between the aircraft deteriorated to 600 feet and 3 NM. Separation was reestablished at 0135:30Z and controller reported incident and was removed from control duties. Preliminary investigation is continuing. TSB Case Closed.

Incident at over Lake Huron

The aircraft (operating as NWA66) was on a scheduled IFR flight from Detroit Metropolitan (Wayne County) International Airport, MI (U.S.A.) (KDTW) to Rome (Fiumicino) International Airport (Italy) (LFCO). Shortly after departure, the flight crew reported electrical problems and elected to return to Detroit. Fuel was dumped over Lake Huron prior to the landing. Ops. impact -- unknown.

NWA66 DC10 enroute Detroit to Rome diverted at 47N/50W to Gander due Passenger Medical emergency. Female passenger with heart attack. Landed Gander 0437z. Nil TSB

February 8, 2001


NWA66 DC10 enroute Detroit to Rome at position south west of Sydney aircraft declared a medical emergency ( 43 year old female flight attendant ) Flight diverted to Halifax. RCC advised. Priority handling and ambulance requested, all agencies advised. Aircraft landed at 0135Z, Rnwy 06. TSB Case Closed with more to follow.

Incident at Gander Oceanic (CYQX)

NWA66 DC10 FL 330 enroute Detroit to Rome experienced Pressurization problems and requested descent to FL250. Unable to issue descent clearance due westbound traffic at FL 320. Pilot carried out contingencies and descended to FL250, requested and was cleared direct Boston. No assistance required. Nil TSB

COA84 B772 FL340 enroute Newark to Tel-Aviv ETA 44N050W 0045Z. NWA66 DC10 FL340 44N050W ETA 0052Z. Separation required was 10 minutes 7 was achieved. The aircraft had speed and time restrictions imposed on the ocean to regain the required spacing TSB Case Closed TSB file # A00A0086. Nav Canada will be conducting a Fact Finding Board.
