Incident at In the vicinity of: NORET intersection

A WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3150) from Kelowna, BC (CYLW) to Calgary Intl, AB (CYYC) was following a Morningstar Air Express Inc. Boeing 757-2B7 (MAL7062) from Vancouver Intl, BC (CYVR) to Calgary Intl, AB (CYYC) into Calgary, in the vicinity of the NORET intersection. WEN3150 was 4NM behind and 3000 feet lower than MAL7062, and lateral separation was increasing. MAL7062 received a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) and levelled at F240 for a minute, and WEN3150 received a TCAS. There was no other known traffic in the area and no impact to operations.

A Morningstar Air Express Inc. Boeing 757 200 (MAL7062) from Calgary, AB (CYYC) to Toronto, ON (CYYZ) initiated an overshoot for Runway 24R citing landing gear issue. The aircraft returned and landed onto Runway 24R at 0931Z without incident. No operational impact.

Incident at 145NM NW TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

A Morningstar Air Express Boeing 757-200 (MAL7062) from Calgary, AB (CYYC) to Toronto, ON (CYYZ) declared a "Pan Pan" due to a leading edge symmetry warning. Aircraft Rescue and Fire Fighting (ARFF) requested. The aircraft landed without incident at 0848Z.
UPDATE: TSB Report#A17O0033: C-FMEU, a Boeing 757-200 aircraft operated by Morningstar Air Express, was conducting flight MAL7062 from Calgary Intl, AB (CYYC) to Toronto/Lester B. Pearson Intl, ON (CYYZ). During cruise flight at FL390, the flight crew received a Master Caution warning and a LE SLAT ASSYM EICAS message. The flight crew verified that the flap handle was in the fully up position and also checked that there was no other indications related to the flaps/slats. The QRH was actioned, a PAN PAN was declared as a precaution and the flight continued to CYYZ where it landed without further event with ARFF standing by. The operator?s maintenance replaced two slat position indicators and the aircraft was returned to service.

A Morningstar Air Express Inc. Boeing 757 200 (MAL7062) from Calgary, AB (CYYC) to Toronto, ON (CYYZ) initiated 2 missed approaches for Runway 23 due to not receiving landing clearance. The terminal control unit (TCU) coordinated landing clearance with the Tower. MAL0742 landed on Runway 23 on the third attempt at 0742Z. There was no operational impact.

A Morningstar Air Express Boeing 757 200 (MAL7062) from Vancouver, BC (CYVR) to Calgary, AB (CYYC) reported a possible bird strike landing Runway 17R. Staff (APRT1) inspected. An owl was found, however when he went to pick it up, it flew away. Minimal impact to operation.

Incident at CALGARY INTL AB (CYYC)

A WestJet Boeing 737 700 (WJA533) from Hamilton, ON (CYHM) to Calgary, AB (CYYC) was inbound to runway 17L from the East and a Morningstar Air Express Boeing 757 200 (MAL7062) from Vancouver, BC (CYVR) to Calgary, AB (CYYC) was inbound to runway 17R from the west. Traffic was passed and WJA533 reported MAL7062 in sight, then reported a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) and climbed 100 feet. TCAS RA quickly resolved and WJA533 continued to airport to land. No impact to operations.

Incident at CALGARY INTL AB (CYYC)

Aviation Incident Report#13558: On arrival in Calgary, AB (CYYC) maintenance discovered Foreign Object Debris (FOD) damage to the #3 main landing gear tire of the Morningstar Air Express Inc Boeing 757-2B7 (C-FMFG / MAL7062). The damage was a cylindrical cut, down to the chord layer. Possibly occurred on departure from Vancouver, BC (CYVR) or arrival in Calgary, AB (CYYC). The tire was replaced with a 14 minute delay.

A Morningstar Air Express Boeing 757 200 (MAL7062) advised the Vancouver (CYVR) ground controller that they had an indication of a possible fire on board the aircraft. Ground initiated a call to Emergency Response Services (ERS). False alarm. No impact to operations.

A Westjet Boeing 737 600 (WJA203) from Regina, SK (CYQR) to Calgary, AB (CYYC) cleared ILS approach runway 29. The aircraft was sequenced behind a heavier Morningstar Air Express Boeing 757 200 (MAL7062) from Vancouver, BC (CYVR) to Calgary, AB (CYYC) on approach. Tower controller pulled WJA203 off the approach due to eroding separation. No other impact on operations.

Incident at CALGARY INTL AB (CYYC)

MAL7062 was inbound to Calgary on the HANDA SIX arrival for runway 34. The aircraft crossed MESKA at 6100ASL instead of the published crossing restriction of 7500ASL or above. The controller instructed the pilot to maintain 5400ASL as it was observed that the aircraft had also descended to 5400 ASL prior to reaching GABOL. GABOL crossing restriction is 6500ASL or above. MAL70262 landed runway 34 without further incident

Incident at VANCOUVER INTL BC (CYVR) (Aerodrome, runway or taxiway shutdown)

On departure runway 08R, MAL7062, a Morningstar Express Boeing 757-200, IFR Vancouver (CYVR) to Calgary (CYYC), reported bird strike. Runway inspection found nothing. Runway 08R returned to service at 0206Z with minimal delays to other airport operators
UPDATE from Aerodrome Safety: No further action required. The Vancouver airport has a wildlife program in place meeting the regulatory requirement of CAR 302.

The Morningstar Air Express Inc. Boeing 727-200 series aircraft (operating as flight MAL7062) was concluding an IFR flight from Calgary International Airport (CYYC) to Toronto (LBPIA) (CYYZ). The aircraft was on approach to runway 23 when the flight crew reported that a birdstrike had occurred at 2,000 feet. The First Officer's window was obscured. The aircraft landed without incident on runway 23 at 0724Z.

Incident at 40NM north of Hamilton

The Morningstar Air Express Inc. Boeing 727-22C aircraft (operating as MAL7062) was on an IFR flight from Calgary International Airport (CYYC) to Hamilton (John C. Munro) International Airport (CYHM). When the aircraft was 40NM north of Hamilton, the flight crew reported a fire warning indication and declared an emergency. ARFF services stood by as the aircraft landed straight in on runway 12 at 0739Z without further incident. The problem appears to be a faulty indication.
Supplemental information from T.S.B. Initial Notification (#A04O0007): The Morningstar Air Express Inc. Boeing 727-22C cargo aircraft, operating as flight MAL7062, was en-route to Hamilton airport when the engine fire bell activated. There were no other indications of fire from any of the three engines. The flight crew was unable to cancel the bell and as a precaution requested ARFF to be on standby for the uneventful landing. After the landing, the bell deactivated and maintenance was unable to duplicate the fault. As a precaution, the main fire detection box and the bell were replaced. Further components to be replaced are the individual fire warning modules for each of the three engines. The operator will be issuing an SDR on this event.

Incident at VANCOUVER INTL BC (CYVR)

Number 3 engine of flight MAL7062 was shutdown after departure. CFR standby. The aircraft landed safely.
A01P0303: During the initial climb on departure from Vancouver the crew of Morningstar Air Express Boeing 727-100 (N124FE) observed the #3 engine (P&W JT8D-1) filter bypass light illuminate. The crew completed the supplementary procedure and completed the engine shutdown checklist. An emergency was declared to air traffic control and the aircraft returned to Vancouver and landed without further incident. Maintenance personnel in Vancouver changed the filter, inspected the system and returned the aircraft to service; no contamination was found in the system.

Morningstar Air Express flight MAL7062 enroute from Calgary to Hamilton, reported smoke in the cabin while on the apron at Hamilton International Airport. The crew requested ERS to respond. The aircraft then turned off their engines. The smoke dissipated. ERS stood down at 0800z. Ops Impact: No delays. SOB: unknown OPI: PAX Supplemental TSB Information (A99O0098): After landing at Hamilton, the Boeing 727 was taxiing to the ramp when the flight crew observed the right air conditioning pack overheat light illuminate and the lavatory smoke detector activate. CFR was requested as a precaution. There was no fire and CFR was stood down. Maintenance replaced the right AC pack cooling fan and the aircraft was returned to service. Three (3) S.O.B. - 3 Crew.

Incident at Moncton NB

MAL7062 B727 departing Moncton initiated a turn after incorrectly reading back the Controller's instructions. The Controller did not detect the Readback error and a loss of separation occurred with another aircraft in the circuit. Traffic was passed. The aircraft were 1.56 miles apart same altitude where 3 miles lateral or 1000 feet vertical is required. Weather was VFR. Nav Canada investigating. TSB case closed. File # A99A0059

Incident at Moncton NB (Aerodrome - runway or taxiway surface condition)

MAL7062 B727 landing Moncton was concerned about the currency of the Runway Condition Report which was issued 0808z. The latest weather sequence reported freezing fog which would indicate the possibility of Hoar Frost on the runways. At aprx 1206z the Airport Maintenance foreman inspected the runway and the flight landed without incident. Total delay time was aprx 20 minutes. Nil TSB
