Incident at GOOSE BAY NL (CYYR)

An Air Labrador de Havilland DHC 6 Series 300 (C-GNFZ/ LAL200) from Goose Bay, NL (CYYR) to Rigolet, NL (CCZ2) had been cleared for takeoff runway 34 and just as they started to roll the aircraft had a bird strike (gull hit left wing). Pilot aborted takeoff, aircraft exited Alpha and returned to the terminal and dropped off passengers. The aircraft proceed to operations for check, returned to terminal and departed again 1048Z. No damage to aircraft.

Incident at GOOSE BAY NL (CYYR)

LAL200, DHC 6 300, was given taxi instructions for Runway 26 in Goose Bay (CYYR) via Taxiway Bravo and to hold short of Runway 26. The pilot acknowledged the taxi instructions and read back "Hold short Runway 26". The pilot contacted the tower for takeoff clearance having already crossed the Hold Short line. As there was no other traffic at the time LAL200 was given takeoff clearance.

Incident at 70NM North of CYYR (Conflict - potential, Conflict - IFR & VFR, ATM - other)

SPR410 DH82 enroute from Goose Bay (CYYR) to Voisey's Bay (CVB2) and LAL 200 DHC6 enroute from Nain (CYPD) to Goose Bay (CYYR), Report filed on behalf of YYR RATCON, Aircraft routing: CYYR direct to CVB2 (Voisey's Bay) at 15000 feet. The pilot reported a TCAS Alert and deviated to the right and then continued on to destination. The only known traffic was a LAL200 operating VFR from CYDP to CYYR at 8500 feet. The LAL200 had its transponder set to 1200 and was mode C equipped but the Mode C was unserviceable. Nil TSB

Incident at MAKKOVIK NL (CYFT)

The Lab Air DHC6 (LAL200) was northbound on a DVFR flight plan from Goose Bay NL with numerous scheduled stops along the Labrador Coast to as far north as Nain and return. The pilot closed the flight plan at CYFT due to weather. The closed flight plan was not re-activated nor was a new flight plan filed for the continuation of the DVFR flight to Nain and return to Goose Bay. The aircraft was operated in the ADIZ without an active flight plan. Nil TSB

LAL200 DHC6 departing Goose Bay for Nain at 1323Z was reported by Halifax FIC to have had penetrated the CADIZ without filing a flight plan. There was also no VFR Alerting Service provided by Halifax FIC for this flight. Nil TSB

Incident at GOOSE BAY

DH6 TWIN OTTER LAL200 C-GNOY - A/C DECLARED EMERGENCY WITH GOOSE BAY TOWER. THE LEFT ENGINE FAILED DUE TO LOSS OF OIL PRESSURE. A/C LANDED SAFELY AT 1410Z. WX VFR. IT WAS DISCOVERED THAT THE FILLER CAP WAS LOOSE. TIME = 1405Z. ACTION = MAH.
