A KLM Royal Dutch Airlines Boeing 747 400 (KLM661) from St John's, NL (CYYT) to Houston, TX (KIAH) taxied and departed Runway 29, closing the terminal ramp and all manoeuvring areas. 2 Aircraft outbound, an Air Saint-Pierre Cessna F406 Medevac (SPM7447-M) from St John's, NL (CYYT) to St-Pierre (LFVP) and an Air Canada Embraer ERJ 190-100 IGW (C-FLWH/ ACA667) and 2 Aircraft inbound, a Jazz de Havilland DHC-8-402 (C-GGOK/ JZA8992) from Halifax, NS (CYHZ) to St John's, NL (CYYZ) and a Provincial Aerospace Beech 200 (C-GTJZ/ SPR04) on a local flight from St John's, NL (CYYT) were delayed, waiting for the foreign object debris (FOD) to be removed. Surfaces were closed at 1610Z when KLM661 commenced taxi, and all surfaces were re-opened at 1800Z.

UPDATE: TSB Report#A15A0026: The Jazz Bombardier CRJ 705 (registration C-GDJZ) aeroplane enroute from Halifax (CYHZ) to St John's (CYYT) was on approach to landing when, on gear down selection, all hydraulic pressure/quantity was lost on the # 3 system. The crew conducted a go around with the gear down and declared an emergency due to lack of nose wheel steering and loss of some braking. The aircraft landed safely, stopped on the runway and was towed to the gate. There was a loss of fluid on the runway which was closed until it was cleaned up. Maintenance replaced a broken hydraulic hose.
At 1540Z, a Jazz Bombardier CL-600-2D15 (C-GDJZ / JZA8992) from Halifax, NS (CYHZ) to St. John's, NL (CYYT) conducted a missed approach indicating hydraulic and landing gear issues. Flight declared an emergency and requested Aircraft Rescue and Fire Fighting (ARFF) standing by. The aircraft's hydraulic problem caused a failure of the landing gear to deploy and subsequently had to be manually pumped into position. In addition, braking ability of aircraft may have been affected; pilot indicated that half of braking capability was available. The aircraft landed at 1600Z and had to be towed off runway due to lack of steering and braking. The resulting loss of hydraulic fluid on the runway caused Runway 11/29 to be closed until the runway surface could be cleaned.

The air traffic controller in Montreal/Trudeau immediately corrected the situation. Porter Airlines Dash 8 POE407 was on an instrument flight rules (IFR) flight from Montréal/Trudeau (CYUL) to Halifax (CYHZ). Air Canada Jazz Regional Jet JZA8992 conducted an IFR from Montreal/Trudeau to Moncton (CYQM). An operating irregularity occurred 46 nautical miles southeast of Montreal/Trudeau when separation between the two aircraft was not assured. Incident at GREATER MONCTON ROMEO LEBLANC INTERNATIONAL AIRPORT (Overshoot/missed approach, Engine oil problem)

UPDATE Maintenance & Manufacturing: The oil pressure was low due to the winter plate on the oil cooler. The student pilot wanted to return to MFC & there was a misunderstanding in conversation with controller.
C-GHTO, Diamond DA 20 C1, taxied to position on Runway 29. JZA8992 was approximately 5 miles final. C-GHTO reported low pressure and asked to vacate Runway 29 and return to Moncton Flight College. Since three aircraft blocked Taxiway ALPHA, C-GHTO taxied Runway 29 to Taxiway BRAVO. JZA8992 was instructed to pull up and go around in a right hand circuit for Runway 29. JZA8992 landed without further delay. Nil TSB.

Update #1: The TSB occurrence number and class of investigation were added. According to the TSB report A06Q0156: Upon departure from Dorval (CYUL), the CRJ2 operating as flight number JZA8992 was authorized to take off from Runway 24L while an A330-300, operating as flight number ACA415, was authorized to take off from Runway 24R. The latter took off when the CRJ2 was in initial climb at 400 ft. A loss of separation lasted for nearly two minutes until the CRJ2 turned left. During that time, the lateral separation between the two aircraft fluctuated between ¾ mile and 1 ½ miles horizontally, with less than 1 000 ft vertically, even though the required separation is 3 miles and 1 000 ft.
JZA8992, a Regional Jet operated by Air Canada Jazz, was authorized to take off from Runway 24L at Dorval (CYUL) for an instrument flight rules (IFR) flight to Moncton (CYQM). ACA415, an Airbus 330-300 operated by Air Canada, was authorized to take off from Runway 24R at Dorval for an IFR flight to Toronto (CYYZ). This situation caused a loss of separation because there was only 1.4 NM that separated the two departing aircraft.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

During take-off from Dorval (CYUL) for a flight to Moncton (CYQM), the crew of AIr Canada Jazz Regional Jet JZA8992 had technical problems and asked to level off at 7000 feet and to be directed back to Dorval. No emergency was declared. The crew made some checks. At 1838Z the crew advised that it was proceeding to Moncton. The aircraft had interrupted take-off from rwy 06L a few minutes earlier. No impact on ATC operations.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

Air Canada Jazz Regional Jet JZA8992 was preparing to fly IFR to Moncton (CYQM). For some reason the crew aborted take-off on rwy 06 at Dorval (CYUL). The aircraft took off again for Moncton eighteen minutes later. No impact on ATC operations.
Update #1: The registration was added. During the take-off run, the flight attendant reported that he could see daylight in the upper part of the main cabin door. The aircraft returned to the gate. Dorval maintenance personnel and Bombardier personnel found that the striker on the bottom of the door was defective. It was repaired and no similar incidents have occurred since then. ****** ** ******* Mise à jour #1: Ajout de l'immatriculation. Lors de la course au décollage, l'agent de bord a rapporté qu'il pouvait voir la lumière du jour dans la partie supérieure de la porte principale de la cabine. L'appareil est retourné à la barrière. Le personnel du département de maintenance de Dorval ainsi que le personnel de Bombardier ont déterminé que c'était le percuteur du bas de la porte qui faisait défaut. Ce dernier a été réparé et aucun autre incident similaire n'a été constaté depuis.

JZA8992, CRJ2, enroute from Montreal (CYUL) to Moncton (CYQM) (IFR), was cleared for a visual approach to Runway 06. C-GMFC, Piper PA-44, (VFR) was holding at Riverview NDB at 3,000 ft. C-GMFC was instructed to continue outbound reference the inbound traffic. Traffic was passed to both aircraft. When they were 2 miles apart, C-GMFC had visual contact with JZA8992. When JZA8992 turned final he advised they had a TCAS and had to maneuver. TSB Evaluating.
