Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Communication navigation surveillance/air traffic system)

Upon landing on Runway 24L, a Jazz de Havilland DHC-8-402 (C-GGNW/JZA8949) from Saint John, NB (CYSJ) to Toronto/Lester B. Pearson, ON (CYYZ) was cleared to taxi via Taxiway D2 and cross Runway 24R. Separation was not assured with a departing Air Canada Embraer ERJ 190-100 IGW (C-FMZB/ACA462) from Toronto/Lester B. Pearson, ON (CYYZ) to Ottawa/Macdonald-Cartier, ON (CYOW). Transportation Safety Board of Canada (TSB) advised.

UPDATE TSB Report #A20O0026: Jazz Aviation LP, flight JZA949, a De Havilland DHC-8-400 (registration C-GGNW) aircraft, landed at Toronto / LBPIA (CYYZ) on Runway 24L and was instructed by ATC to take a right turn onto D2 and then cross Runway 24R. When the aircraft was approaching the holding position for Runway 24R, the flight crew observed an Air Canada ERJ 190-100 (flight AC462, registration C-FMZB) departing runway 24R in front of them. AC462 had received clearance from ATC to depart Runway 24R, and was on the take-off roll. The flight crew of JZA949 queried ATC when they saw AC462 passing in front. NAV Canada is conducting an internal investigation.

Pearson, ON (CYYZ), on initial check in with the Moncton (QM) Area Control Centre (ACC) after departure, indicated a possible gear issue and requested to level off at 5000ft to execute their checklists. The aircraft returned to CYSJ and landed at 2308Z.

A WestJet Boeing 737-76N (C-GRWS/WJA1219) from New York/LaGuardia, NY (KLGA) to Toronto/Lester B. Pearson, ON (CYYZ) was 1000 feet below and 5.1NM ahead. No impact to operations.

Aviation Incident Report#13969: A Jazz de Havilland DHC-8-400 (JZA8949) from Saint John, NB (CYSJ) to Toronto, ON (CYYZ) was at CYSJ when an aircraft refuelling company started to fuel (automatic fuelling method) aircraft #403 with a fuel request of 11724 and checked the high level shut off which stopped the flow. Then when tank 2 reached 5800 lbs of fuel burped out of the wing vent onto the ground. The passengers and crew were deplaned and the spill was contained and cleaned up. There was a delay of departure.

A Jazz Bombardier CL600 2B19 (JZA8949), from Saint John, NB (CYSJ) to Toronto, ON (CYYZ), began take off roll on Runway 23 at 2305Z. The aircraft slowed shortly thereafter and the crew reported a rejected take off due to bird activity. The crew didn't believe they struck any birds and did not see any while examining the runway on their taxi back to the ramp. RED1 inspected the runway and did not find any dead birds. JZA8949 departed for CYYZ off Runway 23 at 2334Z. No impact on operations.

The Air Canada Airbus A319 112 aircraft (operating as ACA691) was completing a scheduled IFR flight from St. John's (CYYT) to Toronto (CYYZ). The Jazz Air LP Canadair RJ200 Regional Jet aircraft (operating as Air Canada Express flight JZA8949) was completing a scheduled IFR flight from Saint John (CYSJ) to Toronto. While on a category 3 approach to runway 06L, the flight crew of ACA were instructed to carry out a go-around as the previous landing aircraft, JZA8949, had not yet cleared the ILS critical area. The (Air Canada) Jazz Air Limited Partnership Bombardier CL-600-2B19 (RJ100) Regional Jet aircraft (operating at JZA8949) was concluding a scheduled IFR flight from Saint John Airport (CYSJ) to Toronto (LBPIA) (CYYZ). The aircraft landed on runway 23 at 0157Z and the flight crew reported a possible rabbit strike. The runway was closed for an inspection and the remains of a rabbit were recovered. Ops. impact -- unknown.

JZA8946, RJ100, enroute from Toronto (CYYZ) to Saint John (CYSJ), cancelled IFR. A Piper PA34, enroute from Saint John to Iles-de-la-Madeleine (CYGR) received a departure clearance off Runway 14. JZA8946 was on final when the PA34 finished reading back the clearance. FSS advised the PA34 of JZA8946 position as three miles final on Runway 23. The PA34 departed and JZA8946 broke off the approach and made a right turn to circle for landing on Runway 23. JZA8949 landed at 20:10. Nil TSB.

JZA8949, enroute from Fredericton (CYFC) to Toronto (CYYZ), advised that it might have hit 2 small snow buntings (snowbirds) with the landing gear on departure from Runway 27. The field foreman found two birds on the runway and the pilot was advised. Nil TSB.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Conflict - potential, Conflict - loss of separation)

The (Air Canada) Jazz Air Limited Partnership Bombardier CL-600-2B19 (RJ100) Regional Jet aircraft (operating as JZA8949) was on a scheduled IFR flight from Saint John International Airport (CYSJ) to Toronto (LBPIA) (YYZ). The corporately-operated North West Company Inc. Cessna C-441 aircraft (C-FNWC) was on an IFR flight from Rouyn-Noranda Airport (CYUY) to Toronto (LBPIA) (CYYZ). A loss of separation occurred when JZA8949 (on the Arrival Controller frequency at 10,000 feet) was descended to 8,000 feet with less than the required separation with a same direction turbo-prop aircraft (C-FNWC) at 9,000 feet. JZA8949 overtook C-FNWC and was approximately 2.1 miles in front of C-FNWC and 300 feet above it when, upon instruction from the Arrival Controller, JZA8949 commenced a climb to 10,000 feet.

At 2340Z Shuttle America flight TCF7598, a Embraer 170, was cleared for takeoff runway 25 Ottawa (CYOW) with Air Canada JAZZ flight JZA8949, A dehavilland Dash 8, on final. TCF7598 was slow to roll. To preseve separation, takeoff clearance was cancelled and JZA8949 was overshot. JZA8949 landed runway 25 at 2253Z. No other impact.
