Incident at N VILRO intersection

A weak, constant emergency locator transmitter (ELT) signal was heard for over 10 minutes and reported by 2 aircraft in the same location, one at FL220 (a Jazz de Havilland (JZA8781) at 0940Z) and the other at FL240 (a Jazz de Havilland (JZA8945) at 1002Z). The Joint Rescue Coordination Centre (JRCC) in Trenton was notified. The source was identified on the ground as the signal from a Syncrude Canada Ltd. Cessna 560 (C-FSNC) heading to St-Hyacinthe, QC (CSU3). No impact.
UPDATE AOR 263176-V3: Aircraft registration information for ELT source changed to a privately registered Piper PA-32R-300. Narrative updated to read: "The source was identified on the ground as the signal from a privately registered Piper PA-32R-300 heading to St-Hyacinthe, QC (CSU3)."

The signal strength for an emergency locator transmitter (ELT) on 121.5 from 0743Z-0747Z was strong. The Moncton, NB (CZQM) shift manager was advised. Halifax Joint Rescue Coordination Centre (JRCC) was advised by the Air Traffic Operations Specialist (ATOS). The issue was resolved with the airline for a Jazz de Havilland DHC-8-402 (C-FSRW/ JZA8945) from Saint John, NB (CYSJ) to Toronto, ON (CYYZ) on the ground.

Due to an issue at the area control centre (ACC), communications and surveillance were conducted using secondary systems. Impact to operations involved a Jazz Aviation de Havilland DHC-8-400 (JZA8945) from Saint John, NB (CYSJ) to Toronto/Lester B. Pearson, ON (CYYZ) and an Air Canada Airbus A320 (ACA603) from Halifax/Stanfield, NS (CYHZ) to Toronto/Lester B. Pearson, ON (CYYZ) which were left on standard instrument departures (SID) for 20-30NM before being cleared enroute. A WestJet Boeing 737-700 (WJA392) from Edmonton, AB (CYEG) to Halifax/Stanfield, NS (CYHZ) performed a 360 at destination (CYHZ) until communications were re-established.

The Flight Service Station (FSS) was advised by airport staff that there was a fuel spill around a Jazz Aviation DeHavilland DHC 8 402 (JZA8945) scheduled from Saint John (CYSJ), NB to Toronto (CYYZ), ON. Flight was cancelled and clean up initiated. Further update at approximately 1400Z from airport staff surmised leak was occurring through the night and only found when ground crew started preparations. Leak was on going and mechanics were on site to repair aircraft. No other impact to operations.

UPDATE Supplementary information received from TSB Daily Report [A12O108]: Jazz flight JZA945, a de Havilland DHC-8-400 aircraft, was concluding a scheduled IFR flight from St John's to Toronto LBPIA, was on the UDNOX1 star for Runway 24L and was level at 7000 feet. Air Canada flight ACA683, an Embraer ERJ-190-100-IGW aircraft, was concluding a scheduled IFR flight from Deer Lake to Toronto LBPIA, was on the EMEBA1 Star for Runway 23 and cleared to descend to 4000 feet. A loss of separation occurred when JZA945 was cleared to 6000 feet. The separation was reduced to 2.2nm laterally and 600 feet vertically in airspace that requires 5 nm or 1000 feet of separation

The controller issued overshoot instructions to JZA8945 while the aircraft was on approach to runway 05 due to decreasing spacing on final approach behind JZA7702, which was operating at a reduced speed due to the weather conditions (snow). The (Air Canada) Jazz Air Limited Partnership Bombardier CL-600-2B19 (RJ100) Regional Jet aircraft (operating as flight JZA8945) was concluding a scheduled IFR flight from Saint John International Airport (CYSJ) to Toronto (LBPIA) (CYYZ). The flight crew reported multiple bird strikes upon landing on runway 24L at 1034Z. Airfield Maintenance staff found the remains of two (2) plovers. Ops. impact -- arrivals were transferred to runway 24R while departures were delayed. Runway 24L was returned to service at 1040Z.

JZA8945 had received a pre-taxi clearance that included a requirement for the pilot to read back "Do not depart until departure approval received". Preliminary investigation shows that clearance was issued and read back correctly; and the pilot did not request the departure approval. Nil TSB.

MUpdate #1: The crew reported an indication regarding the door latch in the passenger cabin. The Dorval maintenance personnel found that the proximity door sensor No. PS6MB was defective, and it was replaced. The aircraft was returned to service. **************** * ************* ise à jour #1: L'équipage a rapporté une indication de blocage de porte dans la cabine des passagers. C-FWJF, a Regional Jet operated by Air Canada Jazz as flight number JZA8945, was conducting an instrument flight rules (IFR) flight from Saint-John (CYSJ) to Toronto (CYYZ). When it was in the vicinity of the Dorval (YUL) VHF omnidirectional range (VOR), following an indication that a door was not closed properly, the crew requested to descend from FL340 to 10 000 ft, and was given a more direct route to its destination. No emergency was declared or priority requested. The aircraft landed without incident at Toronto at 1739Z.

The flight crew of a (Air Canada) Jazz Air Limited Partnership Bombardier Regional Jet aircraft (operating as JZA8945, completing a scheduled IFR flight from St. John's International Airport (CYSJ) to Toronto (LBPIA) (CYYZ)) reported a bird on the runway after landing on runway 05. The runway was closed for an inspection and the remains were removed from the runway. The runway was returned to service at 1650Z. Ops. impact -- two (2) following flights (JZA8935 and JZA7808) had to overshoot to accommodate the runway closure.

Air Canada Jazz Beech Airline JZA8945 struck a bird upon landing. A dead gull was found and removed upon inspection. No damage to the aircraft was reported. An American Eagle¿s takeoff was slightly delayed. JZA8945, un Beech Airline d'Air Canada Jazz, a frappé un oiseau lors de l'atterrissage. Un goéland mort a été trouvé lors de l'inspection et il a été enlevé. Aucun dommage n'a été rapporté à l'appareil et un appareil d'American Eagle a subi un léger retard avant de décoller.
