Incident at 40 W TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

A Jazz de Havilland DHC 8 102 (JZA7720) from London, ON (CYXU) to Toronto, ON (CYYZ) at 40nm west CYYZ at 9000ft was hit by lightning. Navigation electronics failed. JZA7720 declared emergency. Aircraft rescue and fire fighting (ARFF) advised. JZA7720 landed Runway 24R at 0043Z.
UPDATE: TSB Report#A14O0238: The Jazz Aviation LP De Havilland DHC-8-100 aircraft (JZA7720, C-GJMI) was enroute when the right wing was struck by lightning. All navigation screens and radios were temporarily lost but regained power after an estimated ten seconds. Interphone communication between the flight deck and cabin crew also failed for the remainder of the flight. An emergency was declared and the aircraft landed uneventfully with ARFF standing by. The aircraft did not stop on the runway but continued to the gate with ARFF following. Maintenance found the right hand aileron assembly damaged from the lightning strike. The aileron assembly was replaced and balanced IAW AMM 27-12-11. Maintenance also replaced the aileron position indicator rod that was also damaged from the strike.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (FOD (foreign object debris), Aerodrome, runway or taxiway shutdown)

After landing runway 24R, a Jazz Aviation de Havilland DHC 8 400 (JZA7720) from London, ON (CYXU) to Toronto, ON (CYYZ) advised the ground controller that a foreign object debris (FOD) was observed on taxiway C2. MTC25 found exposed wires on the taxiway. Operations on runway 24R ceased pending GTAA approval.

The (Air Canada) Jazz Air Limited Partnership de Havilland DHC-8-400 series aircraft (operating as flight JZA7720) was on a scheduled IFR flight from London (CYXU) to Toronto (CYYZ). A US corporation owned Beechcraft Hawker 900XP (N80E) was on an IFR flight from Toronto (CYYZ) to Pittsburgh (KPIT). When JZA7720 was 7 west of YYZ at 8,000 feet they advised of a TCAS RA with traffic visual. The traffic was N80E in an opposite direction climbing to 7,000 feet. The traffic had been given to JZA7720. No operational impact.

Incident at LONDON ON (CYXU) (Aerodrome, runway or taxiway shutdown)

The Jazz Aviation Limited Partnership de Havilland DHC-8-402 aircraft (C-GGND, operating as Air Canada Express flight JZA7720) was departing on a scheduled IFR flight from London International Airport (CYXU) to Toronto (LBPIA) (CYYZ). NAV CANADA staff at London Tower advised that the flight crew reported a birdstrike on departure off of runway 33. The runway was closed for an inspection and Airfield Maintenance staff recovered a bird carcass from the runway. No species indication was provided.

Incident at LONDON ON (CYXU) (Communication navigation surveillance/air traffic system)

UPDATE Follow-up information received from Safety Oversight-Operations [2011/08/10]: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The pilot was a licensed, rental pilot. The CASI advised that the company has submitted an acceptable corrective action plan (CAP) and no further action is required by this office.
The St. Catharines Flying Club Piper PA28-161 aircraft (C-GQXP) was preparing to depart on a VFR flight from London International Airport (CYXU) to St. Catharines/Niagara District Airport (CYSN). NAV CANADA staff at London Tower instructed the pilot to hold short of runway 33. The pilot was then observed taxiing onto runway 33. When instructed to stop, the pilot indicated that he thought he had a take-off clearance. The aircraft was subsequently cleared to take off runway 33. Ops. impact - minor delay for JZA7720 departing from runway 09

The Air Canada Jazz de Havilland DHC-8-102 aircraft (operating as JZA7720) was concluding a scheduled IFR flight from London International Airport (CYXU) to Toronto (LBPIA) (CYYZ). The V. Kelner Pilatus Center Inc. Pilatus PC-12-45 aircraft (operating as FKL193) was concluding an IFR flight from Cochrane Airport (CYCN) to Toronto (LBPIA) (CYYZ). Toronto Tower staff issued overshoot instructions to JZA7720 on the approach to runway 05 to maintain spacing with the preceding arrival FKL193. Ops. impact - unknown.

The Air Canada Jazz de Havilland DHC-8-102 aircraft (operating as JZA7720) was concluding a scheduled IFR flight from London International Airport (CYXU) to Toronto (LBPIA) (CYYZ). The flight crew overshoot their approach to runway 24L due to a faulty landing gear indication at 2258Z. The aircraft subsequently landed on runway 24L at 2308Z without incident. Ops. impact - none.

An Air Canada Jazz flight JZA7720 a De Havilland Dash 8 - 100 reported a TCAS RA 6 SW of Toronto (CYYZ) at 8000 feet while joining the downwind for Runway 24L. JZA7720 reported traffic in sight which was a United Airlines 737-500 UAL111 departing Runway 23 climbing to 7000 feet. No loss of separation.

Incident at LONDON ON (CYXU) (Aerodrome, runway or taxiway shutdown, Aerodrome - operations)

The (Air Canada) Jazz Air Inc. de Havilland DHC-8 turbopropeller aircraft (operating as JZA7720) was departing on a scheduled IFR flight from London International Airport (CYXU) to Toronto (LBPIA) (CYYZ). The flight crew reported flying through a flock of geese on departure from runway 09 at 800 feet AGL. The flight crew estimated that the aircraft had struck four to five geese. The flight continued to Toronto (LBPIA). Runway 09 was closed for an inspection but nothing was found. Ops. impact -- unknown.
Follow-up information received from Aerodrome Safety [2005/01/28]: A Civil Aviation Safety Inspector (CASI) met with the airport operator about this occurrence. The CASI was informed that the operator had nothing further to report, as presumably the geese were off airport property (given the altitude the aircraft was at).

Incident at LONDON ON (CYXU)

London (CYXU) - Air Canada Regional flight 7720 (JZA7720), C-GVON, departed runway 15 and reported engine failure immediately after departure. Aircraft did wide circuit for landing runway 15 at 2025Z with left engine shut down.
From TSB Notification - A02O0166: Climbing through 200 feet and after the landing gear had been selected up, the #1 propeller went into feather and the engine torque climbed to 130%. The captain shut down the #1 engine, climbed to 1500 feet, and joined a wide downwind to return to land on runway 15. The propeller has been replaced and the engine borescoped with no damage found. The malfunction remains under investigation by the operator. See also CADORS 2002O0687.
