Incident at 45N30W

When an Iberia Air Lines of Spain Airbus A340-600 (ECJFX/IBE6251) from Madrid (LEMD) to New York (JFK), NY reported 45N30W they confirmed the wrong coordinate at 50W. The flight was subsequently re-cleared to 46N50W which was the fix approved with Shanwick and Santa Maria. The incorrect coordinate was 47N50W and traffic at that point was an Air Canada Rouge Boeing 767 300 (ROU1915) from Barcelona (LEBL), Spain to Toronto (CYYZ), ON.

Incident at CYD734, CYD738 (ATM - other)

Multiple aircraft filed flight plans that infringed upon active danger areas south of Halifax. All were identified and rerouted by controllers and/or specialists to avoid the airspace. CYD734-738 were reported active by NOTAM 120652 and CYD703, 739, 740, 741, 742, 743, 744, 745 & 746 were reported active by NOTAM 120645. The flights involved were: QTR083, BAW179, RCH418, UAL115, BAW215, AFR026, UAL111, ELY841, SIA26, DLH420, DAL12, UAL63, AFR688, UAL55, ETH501, KLM255, IBE6257, IBE6253, DAL83, KLM643, IBE6251, DAL161, AFR688, AFR332, AAL65, DAL83 and AAL171.

Incident at 5023N 03301W

IBE6251, Airbus A340-600, enroute from Madrid (LEMD) to New York (KJFK), operating between 50/30 and 51/40 at 34,000 ft. at 15:18Z, advised that the flight was experiencing heavy turbulence and had to climb 300 feet without ATC clearance. The flight further advised that the turbulence lasted between 45 seconds and one minute and that the aircraft had returned to 34,000 ft. Nil TSB.

Incident at Egle Intersetion (ATS operating irregularity)

Iberian IBE6251, an Aairbus 340 was on an IFR flight from Madrid to New York via EGLE, WHALE to Kennedy. Royal Air Maroc RAM200 a Boeing 747 was flying westbound in New York Oceanic airspace via route 41N60W DOVEY, (entirely within New York's airspace, departure and destination unknown). Both aircraft were at FL350. RAM200 deviated 35 miles north of track and Penetrated protected airspace for IBE6251 without ATC clearance. When Moncton ACC observed an unidentified radar target at FL350 the Moncton Controller issued a climb clearance to FL360 to IBE6251, and contacted New York Center to identify the unknown target. After several minutes, New York established radar identification and communication with RAM200. On initial radar contact, the two aircraft were approximately 40 miles apart in Airspace where 15 minutes longitudinal separation was required. New York Centre office of Quality Assurance is conducting an investigation.
TSB Update - 2003-09-08 - A03A0105: Iberia Airlines, IBE6251, an Airbus A340 aircraft, was en route from Madrid to New York via route EGLE, WHALE, JFK. Royal Air Maroc, RAM200, a Boeing 747-200 aircraft was flying westbound in New York Oceanic airspace via route 41N/60W, Dovey. Both aircraft were at FL350. RAM200, deviated 35 miles north of track and penetrated airspace protected for IBE6251 without ATC clearance. When the Moncton ACC controller observed an unidentified radar target at FL350, he issued a climb clearance to FL360 to IBE6251 and contacted New York centre to identify the unknown target. After several minutes, New York centre established radar identification and communications with RAM200. On initial Moncton ACC radar contact, the two aircraft were approximately 40 miles apart in airspace where 15 minutes of longitudinal separation was required. The FAA is investigating.

Incident at 5030N 04543W

The crew of the B747 contacted Gander Domestic Control to inform them that a female passenger had passed away at 2045z. The woman was an American citizen travelling with her spouse. TSB no involvement.
