Accident at CLARENVILLE NL (CCZ3)

Update Dec 24, 2001 Comercial and Business Aviation. Company advises incident was a result of pilot error. No further action required.
C-GCHA HB4 helicopter was being used to assist in the stringing of Lightning Wires on a Hydro transmission line. While maneuvering, the pilot lost sight of the wires. The Main Rotor struck and parted a 1 1/4 inch main power line. The Helicopter was landed without further incident. There was impact damage to the Rotor. The helicopter was fitted with a wire strike protection system (WSPS) but due to the flight profile at the time (Descending and Banking at the time of impact) the WSPS did not engage the Wire. Accident at 40 NM SW Wabush NF (CYWK)

CFMUO HB4 on a flight itinerary from Fermont to an unmanned camp crashed unroute. MAYDAY was received 1812z. 3 SOB. No injuries reported and no report of aircraft damage. The 3 SOB were picked up by a CAF Labrador Helicopter and transported to Wabush. Note Occurrence date July 12, 2000

Accident at Mud Lake, near Goose Bay

GNLK HB4 helicopter departed Goose Bay with only the pilot on board to pick up a passenger at Mud Lake. Arriving at Mud Lake the pilot landed the helicopter on an Easterly heading in a small field which had a foot paths coming in from the east and west. After the Helicopter touched down, the pilot, who was unsure of what direction the passenger would enter from decided to turn the Helicopter 90 degrees so that he could see down both paths. He picked up the helicopter into a 4 foot hover, turned 90 degrees and started to lower the helicopter. As he was setting down, the tail rotor struck a snow bank and deflected the rotor back into the tail boom. The pilot rolled off the throttle and landed the helicopter without further incident. Accident at Grand lake Lab.

C-GJBC HB4 was on a fire fighting flight when the pilot heard loud noises from the engine (Allison C20R). Shortly after, the engine lost power and the pilot autorotated the helicopter to the shoreline of Grand Lake. The helicopter was substantially damaged as a result of the hard landing on the rocky shoreline. Pilot and passengers were not injured. A TSB investigator is enroute to the scene. Classification of the occurrence will be determined after a preliminary examination of the helicopter. Bell helicopters and Allison engine have been notified of the accident. Incident at Rouyn

SCHOONER 06, a Challenger CL60 was in position on runway 08 for takeoff when GBNP, an HB4 on the ramp, advised he was departing to the south. Rouyn FSS immediately told him of the traffic (CL60 taking off on runway 08). The pilot of the Jetranger acknowledged and started his climb to the south. The CL60 had to abort its takeoff because of the helicopter.

AFTER PAX RETURNED, PILOT ATTEMPTED 'NO HOVER' TAKE-OFF. HE WAS CONCERNED THAT IF HE USED A 'NO HOVER' TAKE-OFF, TAIL MIGHT STRIKE NEARBY TALL GRASS. AS PILOT APPLIED COLLECTIVE, A/C STARTED TO ROLL TO LEFT. PILOT REDUCED POWER AND LOWERED COLLECTIVE. A/C SETTLED ON UNEVEN GROUND, BENDING RIGHT REAR EMERGENCY POPOUT FLOAT. PILOT INSPECTED A/C AND FOUND LEFT FORWARD SKID HAD SNAGGED A TREE STUMP HIDDEN IN SNOW. PILOT TOOK-OFF SAFELY AND FLEW A/C TO GIMLI FOR INSPECTION AND THEM CONTINUED ON TO WINNIPEG. HB4 JET RANGER CANADIAN HELI LTD. C-GGOZ - HELICOPTER WAS BEING USED IN A BIRD SURVEY. PILOT HAD LANDED THE A/C AND 3 PAX HAD DISEMBARKED TO SET UP BIRD NETS. PILOT THEN MANOUVERED THE HELICOPTER TO HERD THE BIRD TO THE NETS. WHILE HOVERING IN REARWARDS FLIGHT, THE STINGER CONTACTED THE GROUND AND BROKE OFF AND THE TAIL ROTOR BLADE GRAZED THE MOSS COVERED SURFACE. THE PILOT LANDED IMMEDIATELY AND THERE WAS NO FURTHER DAMAGE. AN OVERFLYING RCMP TWIN OTTER WAS CONTACTED AND A PICKUP OF THE PILOT AND PAX WAS ARRANGED. TIME = 1900Z.

HB4 C-GSHJ - BELL 206B HELICOPTER WAS ON CONTRACT & WAS BEING USED TO TRANSPORT PERSONNEL CONDUCTING WATER-LEVEL SURVEYS. PILOT COULD NOT USE HIS NORMAL LANDING SITE BECAUSE THE LEVEL OF THE WATER IN THE LAKE WAS HIGHER THAN NORMAL. PILOT WAS IN PARTIAL HOVER, WITH TOES OF A/C SHID GEAR RESTING ON SHORELINE OF LAKE. 1 PAX REMAINED SEATED IN THE FRONT OF THE HELI. AS A SECOND PAX WAS RETRIEVING A PORTABLE COMPUTER FROM THE BACK OF THE MACHINE. SUDDENLY THE PILOT SENSED THAT THE ANTI-TORQUE ROTOR BECAME INEFFECTIVE. AS THE PILOT THROTTLED BACK, THE HELI. ROTATED THROUGH 360 DEGREES THEN CAME TO REST IN AN UPRIGHT ATTITUDE ON THE SHORE LINE. NO INJURIES. SUSPECTED DAMAGE. TIME - 2020Z. ACTION - NIL.
