An Etihad Airways Airbus A380-800 (ETD11B) from Abu Dhabi, United Arab Emirates (OMAA) to New York/John F. Kennedy, NY (KJFK) experienced severe turbulence at FL400. Aircraft was descended to FL380 and again experienced severe turbulence. Aircraft was further descended to FL360 and reported smooth. No emergency, no injuries or impact on operation.

TSB Report #A20F0053: C-GYUK a Boeing 737-8K5 operated by Sunwing SWG596 from Cancun Intl. (MMUN), Mexico to Calgary Intl. (CYYC) AB, declared an emergency due to smoke in the cabin while in US airspace at FL360. The aircraft diverted to Memphis International Airport (KMEM), TN and landed without incident. Suspected cause of smoke is a battery from an e-cigarette.

Incident at Mammoth Yosemite Airport / Mammoth Lakes (KMMH)

TSB Report #A20F0025: C-GBHM, an Air Canada Airbus 319-100, was conducting flight AC1047 from Palm Springs Intl. (KSPS), CA to Vancouver Intl. (CYVR), BC. During cruise at FL360 in vicinity of Mammoth Yosemite (KMMH), CA, Flight crew reported severe wind shear of +/- 30 knots and +/- 2000-3000 FPM with an altitude change of +/- 500ft, classified associated turbulence as moderate/severe. Flight continued without further incident. DFDR (digital flight data recorder) download showed vertical accelerations at 1.75G and lateral accelerations -.2G, within limit of Aircraft maintenance manual.

A Societe Air France Boeing 777-300 (AFR084) on a flight from Paris/Charles de Gaulle, France (LFPG) to San Francisco Intl, CA (KSFO) readback the instruction and climbed to FL360. No impact to the operation.

A Lufthansa Airlines aircraft (DLH460) reported severe turbulence at FL360. Aircraft did not vacate flight level. The Halifax Flight Information Centre (FIC) was notified. At 1918Z, a Kenya Airways Boeing 787-8 (KQA003) also reported severe turbulence at FL370 in the same vicinity. The FIC was notified again.

Incident at 20NM NE CHARLO NB NDB (CL)

A Blue Panorama Airlines Boeing 767-323 (EIGEP/BPA1712) from Milan/Malpensa, Italy (LIMC) to Havana/Jose Marti Intl, Cuba (MUHA) climbed 300 feet above the assigned altitude of FL360. Controller noticed immediately and advised pilot. Aircraft descended initially to 200 feet above, then climbed again to 300 feet above briefly before correcting to 200 feet below FL360. After a number of corrections, aircraft levelled at FL360. Boston (BOS) air route traffic control center (ARTCC) was advised.

The aircraft entered Moncton (QM) airspace at FL280 and landed Halifax/Stanfield Intl, NS (CYHZ) at 0336Z without incident.

A United Airlines Boeing 777-200 (UAL56) from Paris, France (LFPG) to Newark, NJ (KEWR) and an Air Canada Boeing 787-9 (ACA879) from Zurich, Switzerland (LSZH) to Toronto/Lester B. Pearson, ON (CYYZ) had less than required separation after air traffic control (ATC) issued a climb clearance to UAL56.
TSB #A19Q0054: N788UA, a United Airlines Boeing 777-200, was on instrument flight rules (IFR) flight UAL56 from Paris/Charles de Gaulle, France (LFPG) to Newark/Liberty Intl, NJ (KEWR). C-FRTU, an Air Canada Boeing 787-9, was on instrument flight rules (IFR) flight ACA879 from Zürich-Kloten, Switzerland (LSZH) to Toronto/Lester B. Pearson Intl, ON (CYYZ). UAL56 was level at FL360, while ACA879 was also level at FL380. The two aircraft followed headings that were similar, but divergent. Approximately 30 NM southwest of Sept-Îles, QC (CYZV), UAL56 was cleared to climb to FL380. There was a loss of separation when UAL56 established itself at FL380 and the horizontal distance between the 2 aircraft was reduced to 8.8 NM. The required separation was 10 NM horizontally, or 1 000 ft vertically.

Incident at VANCOUVER INTL BC (CYVR)

The terminal was advised that an Air Canada Airbus A320-211 (C-FFWN/ACA538) from Vancouver, BC (CYVR) to Anchorage, AK (PANC) returning to CYVR with an engine out. Tower initiated stand by. ACA538 landed without incident at 2137Z.
Update TSB #A19P0081: C-FFWN, an Airbus 320-200 aircraft operated by Air Canada, was conducting flight ACA538 from Vancouver Intl (CYVR), BC to Anchorage/Ted Stevens Intl (PANC), AK with 5 crew members and 106 passengers on board. During cruise flight at FL360 approximately 158 nm north west of CYVR, the flight crew received an ENG 2 FAIL ECAM message. The flight crew completed the associated checklist, secured the engine (CFM56-5A1), declared a MAYDAY, and requested a return to CYVR where the aircraft landed without further incident. The operators maintenance found metal contamination in the fuel pump, Hydro Mechanical Unit (HMU), fuel-oil heat exchanger, fuel flow transmitter, and servo fuel heater. The engine was changed, and the aircraft was returned to service.

An Air Canada Airbus A320-214 (C-FZUB/ACA253) from Edmonton, AB (CYEG) to Vancouver, BC (CYVR) declared PAN PAN due to a hydraulic issue and reported shutting down the #1 engine. Aircraft landed safely on Runway 26R and taxied off the runway.
Update TSB #A19P0055: C-FZUB, an Airbus 320-200 aircraft operated by Air Canada, was conducting flight ACA253 from Edmonton Intl (CYEG), AB to Vancouver Intl (CYVR), BC with 5 crew members and 128 passengers on board. During cruise flight at FL360, the flight crew observed a GREEN low hydraulic fluid pressure on the ECAM, followed by a hydraulic green number 1 engine low pressure. The flight crew turned off the Engine Driven Pump (EDP) and the Power Transfer Unit (PTU), and declared a PAN PAN. The landing gear was manually extended, and the aircraft landed at CYVR on Runway 26R without further incident, with ARFF standing by. The operator’s maintenance found that the number 5 right-hand spoiler servo was leaking, and replaced it.

TSB Report #A19F0266: C-FDBD, a Boeing 737-800 aircraft operated by Sunwing Airlines Inc., was conducting flight TFL194, Kos Island Intl (LGKO), Greece to Amsterdam/Schiphol (EHAM), Netherlands with 6 crew members and 183 passengers on board. During the climb through FL300, ATC asked the flight crew if they could climb to FL360 and how long it would take. The flight crew replied in the affirmative and indicated it would take 4 minutes. While climbing the flight crew informed ATC that they would be unable to make FL360 within the 4 minutes. Leveling off at FL360 a TCAS RA (Monitor vertical speed) was triggered. No action was required, the flight continued uneventfully.

Pearson Intl (CYYZ), ON with 6 crew members and 200 passengers on board. During cruise flight at FL360 in the vicinity of Scranton, PA, the flight crew reported a collision with what is believed to be a weather balloon. The flight continued to destination and landed without further incident. The operator’s maintenance conducted a visual external inspection of the aircraft, and no damage was found. The aircraft was returned to service.

Incident at 250NM N MUCC - Jardines del Rey Intl - Cayo Coco

TSB Report #A19F0054: C-GPVS, a Boeing 737-800 aircraft operated by Sunwing Airlines, was conducting flight SWG158 from Fredericton Intl (CYFC), NB to Cayo Coco/Jardines del Rey Intl (MUCC), Cuba with 6 crew members and 181 passengers on board. During cruise flight at FL360, approximately 250 nm north of MUCC, one of the flight crew members began to feel unwell after consuming a meal. Within the next 10 minutes, the flight crew member started to feel ok and was able to resume duties of Pilot Monitoring (PM). The aircraft landed at MUCC without further incident. In case of a re-occurrence during the return portion (SWG159) from MUCC to CYFC, it was decided that the affected flight crew member would carry out the same duties (PM). The affected flight crew member felt unwell again for a brief period during the return flight, which resulted in a state of fatigue. Controlled rest was used, in conjunction with careful hydration. Prior to the arrival at CYFC, the affected flight crew member assumed PM duties, and the flight landed without further incident.

Incident at 84NM SW COLORADO SPRINGS, CO (KCOS)

TSB Report #A19F0017: C-FLSS, an Airbus 320-200 aircraft operated by Air Canada, was conducting flight ACA795 from Toronto/Lester B. Pearson Intl (CYYZ), ON to Los Angeles Intl (KLAX), CA with 5 crew members and 146 passengers on board. During cruise flight at FL360, approximately 85 nm south west of Colorado Springs/Municipal (KCOS), CO, the aircraft passed through a mountain wave and the number 1 engine (CFM56-5A1) rolled back from 90% to 60% N1. The aircraft started to lose speed, and started to descend. The flight crew notified ATC and requested FL300. There was a slight vibration on the number 1 engine, however there were no ECAM warnings. Shortly thereafter, the engine recovered and the flight landed without further incident. No emergency was declared. The operator is troubleshooting to find the cause.

Incident at 570NM W SEATTLE-TACOMA INTL - SEATTLE (KSEA)

TSB Report #A19F0005: C-FSNQ, a Boeing 737-8 MAX aircraft operated by Air Canada, was conducting flight ACA517 from Vancouver Intl (CYVR), BC to Honolulu/Daniel K Inouye Intl (PHNL), HI. During cruise flight, continuous light turbulence was encountered as expected. Shortly after, flight conditions worsened to continuous moderate turbulence at FL360. As the flight subsequently encountered severe turbulence, a PAN PAN was declared, and the flight crew requested a lower altitude via Controller-Pilot Data Link Communications (CPDLC). The autopilot could not maintain the selected altitude or speed, as there were variations of at least +/- M.03 and +/- 500 feet observed. The aircraft descended to FL300 and continued to destination without further incident.

The pilot of a Horizon Air Industries Embraer ERJ 170-200 SU (QXE2535) on a flight from Edmonton Intl, AB (CYEG) to Seattle/Tacoma Intl, WA (KSEA) reported a computer error that showed fluctuating air pressure. The aircraft was climbing through FL310 to FL360. The aircraft requested descent to FL300. Once the aircraft levelled at FL300, the pilot reported that the indicators returned to normal. No impact to operations.

A Qatar Airways Boeing 777-200 (A7BBG/QTR739) on a flight from Doha, Qatar (OTHH) to Los Angeles Intl, CA (KLAX) was transitioning from westbound (W/B) to eastbound (E/B) direction of flight. The aircraft was asked "What time able FL370?" and it replied that it was able to get to FL370 now. Approximately 10 seconds later, the aircraft advised that it was leaving FL360 and climbing to FL370. No climb clearance had been issued. No operational impact.

Radar mode C indicated that the aircraft descended without clearance to FL356. The pilot stated that mountain wave action rendered them unable to maintain the assigned altitude. After establishing contact with the Winnipeg Area Control Centre (ACC), a SkyWest Airlines Bombardier CL-600-2D24 (Series 900) (SKW4559) from Minneapolis/St. Paul International, MN (KMSP) to Winnipeg/Richardson International, MB (CYWG) was cleared to continue on the NORAK TWO arrival into CYWG, and was subsequently observed beginning their descent out of FL360 without a descent clearance. After a query from the controller, the aircraft was cleared to FL290. No operational impact.

Incident at BUDUM intersection (Communication navigation surveillance/air traffic system)

An Etihad Airways P.J.S.C. Boeing 777 300 (ETD89Q) from Abu Dhabi Intl Airport, UAE (OMAA) to Los Angeles Intl Airport, CA (KLAX) requested FL360 through Rekjavik. FL360 was approved by Edmonton area control centre (ACC) and not updated in Canadian Automated Air Traffic System (CAATS). The aircraft entered Edmonton (EG) airspace at FL360 when CAATS data showed at FL340. No loss of separation and no impact to operations.

SIGMET G1 issued after Halifax (HZ) Flight Service Station (FSS) was advised of each of the reports. Aircraft continued on route as filed.
