Incident at N VILRO intersection

A weak, constant emergency locator transmitter (ELT) signal was heard for over 10 minutes and reported by 2 aircraft in the same location, one at FL220 (a Jazz de Havilland (JZA8781) at 0940Z) and the other at FL240 (a Jazz de Havilland (JZA8945) at 1002Z). The Joint Rescue Coordination Centre (JRCC) in Trenton was notified. The source was identified on the ground as the signal from a Syncrude Canada Ltd. Cessna 560 (C-FSNC) heading to St-Hyacinthe, QC (CSU3). No impact.
UPDATE AOR 263176-V3: Aircraft registration information for ELT source changed to a privately registered Piper PA-32R-300. Narrative updated to read: "The source was identified on the ground as the signal from a privately registered Piper PA-32R-300 heading to St-Hyacinthe, QC (CSU3)."

An Air Inuit de Havilland DHC-8102 (C-FDAO/AIE842) from Kuujjuaq, QC (CYVP) to Puvirnituq, QC (CYPX) received and read back clearance to descend to FL200. Air traffic services (ATS) entered FL220 by mistake. When the controller was switched, and when AIE842 was asked again, AIE842 confirmed it was at FL200, not at FL220 as indicated by ATS. AIE842 was cleared again for FL200. No impact on operations.

A Sky Jet M.G. An Leeward Islands Air Transport Aerospatiale ATR 42 (V2LIA/LIA991) from Quebec/Jean Lesage, QC (CYQB) to Fort Lauderdale, FL (KFXE) received and read back FL200 but was seen climbing at FL204. When questioned, the aircraft reported that it was climbing to FL220, but the flight data entry (FDE) indicated FL200. LIA991 was cleared for FL220, and there was no impact on operations.

Incident at 5411N / 12643W (Aerodrome, runway or taxiway shutdown)

A North Cariboo Flying Service Beech 200 (NCB911) on a flight from Edmonton Intl, AB (CYEG) to Terrace, BC (CYXT) at FL220 declared a MAYDAY at 2003Z approximately 40NM SSE of Smithers, BC (CYYD). The pilot reported low oil pressure and engine surging and shutting down the engine, and requested clearance to CYYD. The aircraft had 1600 lbs of fuel and 4 souls on board (SOB). No assistance was requested and the aircraft landed safely at CYYD at 2021Z but was disabled on the runway. Runway re-opened at approximately 2040Z. (Duplicate AOR 258923-V1 deleted).
UPDATE TSB Report #A19P0169: C-FCGC, a Beech Aircraft Corporation King Air 200 aircraft operated by North Cariboo Flying Service Ltd., was conducting flight NCB911 from Edmonton Intl (CYEG), AB, to Terrace (CYXT), BC with 2 crewmembers and 2 passengers on board. During the initial approach to CYXT, the number 2 engine (Pratt & Whitney Canada PT6A-41 serial #PCE85035) fuel flow gauge indicated a drop followed by a spike and then began fluctuating. The number 2 oil pressure master warning light then illuminated followed by an oil pressure drop and fluctuation. Concurrently, the number 2 engine surged with torque fluctuations. Following SOPs and the QRH, the crew shut down the number 2 engine and initiated a diversion to Smithers (CYYD), BC. Vancouver Center was advised and the crew declared a Mayday. The crew did not request ARFF but the CYYD FSS ordered the emergency vehicles to standby. The aircraft landed safely, came to a stop, and was towed off the runway. Maintenance conducted troubleshooting and replaced the #2 engine FCU and high-pressure fuel pump prior to the test flight.

Incident at 42NM N VANCOUVER INTL BC (CYVR)

TSB Report #A19P0022: C-FSRY, a Bombardier DHC-8-400 aircraft operated by Jazz Aviation LP, was conducting flight JZA8185 from Vancouver Intl (CYVR), BC to Fort St. John (CYXJ), BC with 4 crew members and 62 passengers on board. As the aircraft was climbing through FL220 after the departure from CYVR, the flight crew reported a hissing noise in the flight deck, simultaneously receiving a cabin pressure caution indication. As the cabin pressure began to climb, the flight crew donned their oxygen masks. An emergency descent was executed, and the aircraft returned to CYVR where it landed without further incident. A few passengers reported ear discomfort during the descent. The operator’s maintenance inspection revealed that the belly drain under the cockpit was leaking air. The valve and the cabin pressure controller were replaced. The aircraft was returned to service.

A WestJet Encore Ltd. de Havilland DHC-8-402 (C-FSWE/WEN3109) from Calgary International, AB (CYYC) to Nanaimo, BC (CYCD), at FL220, responded to a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) that referred to an American, corporately registered Piaggio P-180 Avanti (N990RS) from Spokane International, WA (KGEG) to Kamloops, BC (CYKA) at FL220. Both aircraft were radar identified, and traffic was not passed to either. WEN3109 was observed responding to a TCAS descent within 2.5NM of N990RS on a crossing track. Both aircraft reported the other aircraft in sight.
Update TSB Report #A19P0002: C-FSWE, a Bombardier DHC-8-400 aircraft operated by WestJet Encore, was conducting flight WEN3109 from Calgary Intl (CYYC), AB to Nanaimo (CYCD), BC. N990RS, a Piaggio P.180 aircraft operated by Rainbow Sandals Retail Inc, was conducting a flight from Spokane Intl (KGEG), WA to Kamloops (CYKA), BC. A loss of separation and risk of collision occurred near Kelowna (CYLW), BC, while both aircraft were cruising at FL220. The separation between the 2 aircraft reduced to approximately 500 feet vertically and ½ nm laterally, when the required separation minima was 1000 feet or 5 nm.

Incident at West Palm Beach FL

TSB #A19F0090: C-GJWN, an Airbus 321-200 aircraft operated by Air Canada, was conducting flight ACA921 from West Palm Beach Intl (KPBI), FL to Toronto/Lester B. Pearson Intl (CYYZ), ON with 6 crew members and 16 passengers on board. Passing FL220 during the climb to cruising altitude after the departure from KPBI, the aircraft experienced a Green Hydraulic System fluid loss. The flight crew contacted the operator’s Maintenance and Operational Control Centres, and the decision to return to KPBI was made. No emergency was declared, and the aircraft landed on Runway 10L without further incident. The aircraft was then towed to the gate. The operator’s maintenance found a leak in the inboard lockstay actuator hose. Both lockstay actuator hoses were replaced in accordance with the Aircraft Maintenance Manual (AMM) and, following functional checks, the aircraft was returned to service.
UPDATE Aviation Incident Report: ACA921 returned to land on West Palm Beach, FL (KPBI) Runway 10L after reporting a hydraulic failure. Aircraft Rescue and Fire Fighting (ARFF) unit responded and the aircraft was towed to the gate with passengers on board.

A WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3122) from Thunder Bay, ON (CYQT) to Toronto/Pearson Intl, ON (CYYZ) was instructed to fly heading 140 and climb to FL210, and the pilot read back the instruction correctly. The aircraft was subsequently observed climbing through FL220. When queried, the pilot indicated being confused between the heading and altitude. No other aircraft were in the vicinity and there was no impact to operations.

Incident at 40NM N IQALUIT NU (CYFB) (Communication navigation surveillance/air traffic system)

While inbound for Iqaluit, NU (CYFB), a First Air Aerospatiale ATR 42-500 (FAB883) from Artic Bay, NU (CYAB) to Iqaluit, NU (CYFB) was cleared for FL230 but read it back wrong and it was not corrected. Impact: loss of separation with an Aerospatiale ATR 42-500 (FAB871) from Iqaluit, NU (CYFB) to Igloolik, NU (CYGT) climbing to FL220 in the opposite direction.
Update: TSB Report A19Q0191: C-FTIQ, a First Air ATR-42-500 (FAB883), was on an IFR flight from Arctic Bay, NU (CYAB) to Iqaluit, NU (CYFB) with 8 passengers and 3 crew members on board. There was a misunderstanding with respect to descent clearance at flight level (FL) 230 and the aircraft continued its descent below FL230. At that moment, C-FTIL, a First Air ATR-42-500 (FAB871) on an IFR flight from CYFB to Igloolik, NU (CYGT) with 21 passengers and 3 crew members on board, was climbing to FL220. Air traffic control noticed FAB883s descent below FL230 and intervened through the CYFB flight service station (FSS), asking FAB883 to stop the descent to FL200 and FAB871 to stop the climb to FL190. Due to the time required to issue clearances through the FSS, FAB883 descended to FL198 before pulling up to FL200 while the two aircraft were approximately 40 NM north of CYFB. Separation between the two aircraft was reduced to 800 ft vertically and 6 NM horizontally while the required separation is 1 000 ft vertically and 10 NM horizontally. The two aircraft were in radio contact on frequency 126.7 MHz and were displayed on their traffic alert and collision avoidance systems (TCAS). There were no injuries or damage.

A Jazz de Havilland DHC-8-402 (C-GGOY/JZA8441) on a flight from Regina Intl, SK (CYQR) to Calgary Intl, AB (CYYC) was cleared to and read back FL200. The aircraft was observed levelling off at FL220 and queried. The aircraft was then re-cleared to FL220. No impact to operations. No other aircraft in the vicinity.
UPDATE AOR 256898-V3: Update to narrative: Upon further review, this was determined to be an air traffic service (ATS) operating irregularity (OI) due to the length of time required for the controller to recognize the discrepancy.

A Jazz de Havilland DHC-8-402 (C-GXJZ/JZA8589) on a flight from Saskatoon/John G. Diefenbaker Intl, SK (CYXE) to Calgary Intl, AB (CYYC) was observed on radar climbing through FL210 without clearance. When queried by the Winnipeg area control centre (ACC) controller, the pilot said they were cleared to FL220. The pilot was cleared to FL200 and the clearance was readback correctly. JZA8589 was subsequently cleared to FL220. No impact to operations.

An Air Spray (1967) Ltd. Aero Commander 690A (C-GIPM) from Casper–Natrona County Intl, WY (KCPR) to Calgary Intl, AB (CYYC) declared an emergency due to engine trouble and was diverted to Lethbridge, AB (CYQL). The aircraft landed at 2225Z without incident.
Update TSB Report #A19W0028: C-GIPM, an Aero Commander 690A aircraft operated by Air Spray (1967), was conducting an IFR flight from Casper/Natrona County Intl (KCPR), WY to Calgary Intl (CYYC), AB with one pilot on board. During cruise flight at FL220 south of Lethbridge (CYQL), AB, the pilot noticed a loss of oil pressure for the number 1 engine (Honeywell TPE-331-5-251). As the oil pressure continued to drop, the pilot elected to shut down and feather the engine. An emergency was declared, and the pilot diverted to CYQL where a single-engine approach and landing was performed without further incident.

This error was not discovered by the air traffic service (ATS) unit.

Update: An Air Creebec Inc. de Havilland DHC-8-102 (C-FCWP/CRQ236) from Chibougamau/Chapais, QC (CYMT) to Montreal/Pierre Elliott Trudeau International, QC (CYUL) levelled off at FL200, even though it had been cleared for FL220 and had correctly read it back. When questioned by ATC, CRQ236 believed to have received FL200. CRQ236 was cleared again for FL200. No impact on operations.

Even though it was cleared for FL200 and correctly read it back, a Jazz de Havilland DHC-8-102 (JZA8731) from Sept-Iles, QC (CYZV) to Quebec/Jean Lesage, QC (CYQB) was seen at FL212 still climbing. The air traffic controller (ATC) brought this to the pilot?s attention and gave a new clearance for FL220. No impact on operations.

A Porter Airlines de Havilland DHC-8-402 (C-GKQB/POE482) on a flight from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Toronto/Billy Bishop, ON (CYTZ) was cleared to FL220, and readback was correct. The aircraft was observed at FL229 and climbing. POE482 was re-cleared to FL220. No impact on operations.

Incident at 4822N / 07754W

Shortly after its takeoff from Runway 36 at Val-d'Or, QC (CYVO) a Sunwing Boeing 737-8K5 (C-FAWC/SWG650) from Val-d'Or, QC (CYVO) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was instructed to advise when it reached the very high frequency omnidirectional range (VOR) 180 radial from YVO. The aircraft confirmed that it was on the radial. Although it had received and read back the instruction to climb to FL280 and maintain the 180 radial until further notice, SWG650 was seen on the VOR 360 radial from YVO. Impact: A loss of separation with a Hydro-Quebec de Havilland DHC-8-311 (C-GJNL/HYD150), flying from Chibougamau/Chapais, QC (CYMT) to Rouyn-Noranda, QC (CYUY), which was descending for CYUY and leaving FL220.

Incident at VANCOUVER INTL BC (CYVR)

At 1316Z, an Air Canada Boeing 777-233LR (C-FIUA/ACA034) from Sydney Airport, Australia (YSSY) to Vancouver Int'l, BC (CYVR) declared an emergency with Oakland Centre due to bleed-air failure. The pilot advised they had a pressurization issue and descended from FL360 to FL220. They further advised of a medical emergency on board. The aircraft continued to CYVR and landed without further incident at 1527Z. No operational impact.

Incident at 3NM RUTLAND (KELOWNA) BC (EX) NDB

A WestJet Encore de Havilland DHC-8-402 (C-GKWE/WEN3349) from Kelowna, BC (CYLW) to Vancouver International, BC (CYVR) was cleared to FL200. The pilot read back FL240. When the aircraft was observed climbing through FL200, WEN3349 was re-cleared to FL220. No operational impact.
