A Jazz de Havilland DHC-8-300 (JZA8750) from Val-dOr, QC (CYVO) to Montreal/Pierre Elliott Trudeau, QC (CYUL) at 60 NM SE FL200 reported that it had lost contact (CTC) with Montreal on frequency 125.9. Montreal was notified. The technical operations coordinator (TOC) was notified.

Incident at 35NM E PORT HARDY BC (CYZT)

An American, corporately registered Bombardier BD-700-1A11 (N3333U) on a flight from Vancouver Intl, BC (CYVR) to Tokyo/Haneda, Japan (RJTT) declared PAN PAN due to a cracked windshield. The aircraft was given descent to a safe altitude and was cleared to return to CYVR. After holding to burn fuel, N3333U landed safely at CYVR. No operational impact.
UPDATE TSB Report #A20P0050: N3333U, a Bombardier BD-700-1A11 (Global 5000) owned by Wilmington Trust Company Trustee, was conducting a flight from Vancouver International (CYVR), BC, to Tokyo International (RJTT), Japan, with 4 crew members and 4 passengers onboard. During cruise flight at approximately 35 NM east of Port Hardy (CYZT) and FL398, the right hand cockpit windshield cracked. The crew declared a PAN PAN and performed an emergency descent to 12 800 ft as per ATC instructions. The crew followed the QRH and verified the cabin pressure remained stable when the aircraft passed through FL200. The crew cancelled the PAN PAN on return to CYVR and performed a 90 minute fuel burn. The aircraft landed safely with no reported injuries and no damage to the aircraft. A third party maintenance vendor has been contacted to replace the windshield.

Incident at In the vicinity of: GRAVELBOURG SK (CJM4) (Bird presence, Aerodrome - other)

Aviation Incident Report #16596: Information for Winnipeg Centre: a suggestion was made that a NOTAM be issued for Gravelbourg, SK (CJM4) and Moose Jaw, SK (CYMJ) regarding increased migratory waterfowl (i.e. geese). Flights of approximately 50 geese have been observed on April 23 and 24, flying Northbound near Gravelbourg. Due to thermal activity, these flights are currently much higher than would be expected normally. Flights of these geese have been seen at approximately FL200 to FL250 above sea level (ASL). This would be an unexpected hazard to civil and nearby military aviation. 15 Wing Royal Canadian Air Force (RCAF) should be advised, as they use this area extensively to transit jet trailers to their southern training zone (CYA-310M). This is a semi-circular area around N49°30.16 W106°0.48.

Incident at 30NM NW IQALUIT NU (CYFB)

After it had mentioned to air traffic services (ATS) that it was conducting its flight on a company itinerary, a Keewatin Air Beech B200 (C-FSKO/KEW204) from Iqaluit, NU (CYFB) to Cape Dorset, NU (CYTE) entered controlled airspace without clearance. The aircraft reached FL200. No impact on operations.

Incident at 5908N / 07243W (Communication navigation surveillance/air traffic system)

An Air Inuit de Havilland DHC-8102 (C-FDAO/AIE842) from Kuujjuaq, QC (CYVP) to Puvirnituq, QC (CYPX) received and read back clearance to descend to FL200. Air traffic services (ATS) entered FL220 by mistake. When the controller was switched, and when AIE842 was asked again, AIE842 confirmed it was at FL200, not at FL220 as indicated by ATS. AIE842 was cleared again for FL200. No impact on operations.

An Leeward Islands Air Transport Aerospatiale ATR 42 (V2LIA/LIA991) from Quebec/Jean Lesage, QC (CYQB) to Fort Lauderdale, FL (KFXE) received and read back FL200 but was seen climbing at FL204. When questioned, the aircraft reported that it was climbing to FL220, but the flight data entry (FDE) indicated FL200. LIA991 was cleared for FL220, and there was no impact on operations.

Incident at CYUL 240 / 25

A SkyWest Airlines Bombardier CL-600-2B19 (SKW3130) from Chicago, IL (KORD) to Montreal, QC (CYUL) reported that it had lost its airspeed indicator and declared an emergency. Emergency measures were taken. SKW3130 landed on Runway 06R at 1510Z. No impact on operations.
TSB Update #A19Q046: A SkyWest Airlines Bombardier CRJ-200 (N928EV/SKW3130), with 3 crew members and 44 passengers on board was on an instrument flight rules (IFR) flight from Chicago O’Hare, IL (KORD) to Montreal/Pierre Elliott Trudeau, QC (CYUL). While inbound for CYUL, at FL200, speed indications became unreliable; the pilot-in-commands anemometer displayed an "amber IAS flag" (indicated airspeed); the co-pilots indicated 40 kts and his altimeter was fluctuating. The flight crew declared an emergency. The aircraft landed without incident on Runway 06R, with Aircraft Rescue and Fire Fighting (ARFF) services present and ready to intervene. No damage or injuries to report.

A Porter Airlines de Havilland DHC-8-402 (POE471) from Toronto/Billy Bishop, ON (CYTZ) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was in a holding pattern at ARVIE and FL200, and at 1912Z, it was cleared for descent. POE471 was seen at FL203 before starting to descend. A WestJet Encore de Havilland DHC-8-402 (WEN3526) from Toronto/Lester B. Pearson, ON (CYYZ) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was also in the holding pattern at ARVIE and FL200. For a few seconds, separation was reduced to 700 ft and 3,3 NM, where normally, separation must be 1 000 ft or 5 NM.
Update: TSB Report #A19Q0027: C-GLQZ, a Porter Airlines Bombardier DHC-8-400, was on flight POE471 from Toronto/Billy Bishop Toronto City, ON (CYTZ) to Montreal/Pierre Elliott Trudeau Intl, QC (CYUL). During the descent at CYUL, air traffic control (ATC) assigned POE471 to a holding pattern at ARVIE, at FL200. C-GENM, a WestJet Encore Bombardier DHC-8-400, was on flight WEN3526 from Toronto/Lester B. Pearson Intl, ON (CYYZ) to CYUL, and was also in the holding pattern at ARVIE, FL210. When POE471 was cleared to leave the holding pattern, it was seen at FL203, which caused a loss of separation with WEN3526. Separation was reduced to 700 ft vertically and 3.3 NM laterally, while the standard was 1 000 ft or 5 NM.

Incident at BOBKI intersection

An Air Canada Airbus A330-300 (ACA419) from Montreal/Trudeau, QC (CYUL) to Toronto/Pearson, ON (CYYZ), which was on a heading of 280 and climbing to FL200, was instructed to fly direct MELTI. Fifty seconds passed before the beginning of the turn. The result was a loss of separation with a Jazz Bombardier CL-600-2B19 (series 200) (JZA8851) from Moncton/Roméo LeBlanc, NB (CYQM) to Ottawa/Macdonald–Cartier, ON (CYOW), which was abeam and descending to FL210. The paths were therefore divergent. No impact on operations.
Update - TSB Report #A19Q0013: C-GFAJ, an Air Canada Airbus A330-300, was on instrument flight rules (IFR) flight ACA419 from Montreal/Pierre Elliot Trudeau Intl., QC (CYUL) to Toronto/Lester B. Pearson Intl., ON (CYYZ). While it was climbing to FL200, the flight crew was instructed to proceed direct MELTI. 50 seconds passed before the beginning of the turn. Consequently, its separation with C-GGJA, a Jazz Aviaton LP Bombardier CRJ-200 on flight JZA851 to Ottawa/MacDonald-Cartier Intl., ON (CYOM), was reduced to 4.7 miles laterally, while the standard is 5 miles. The paths of the aircraft were divergent.

A British Aerospace BAE Jetstream 3100, operated by Pascan (PSC882), flying from Bagotville (CYBG), QC, to Montreal/Saint-Hubert (CYHU), QC, was handed off from Bagotville climbing to FL200. Upon initial contact with Montreal, PSC882 reported that it was going to climb from 10,500 feet to 15,000 feet. PSC882 was then re-cleared to FL200. No impact on operations.

Incident at ONDOB intersection

An Air Bravo Corp. Pilatus PC-12 (AB237) from Sudbury, ON (CYSB) to Ottawa/Macdonald-Cartier, ON (CYOW) crossed ONDOB while following the MEECH arrival at CYOW at FL207, though there was a crossing restriction at FL200 or less. AB237 was also seen conducting a right turn and deviating from the MEECH standard terminal arrival (STAR) after ONDOB, without clearance.

QXE2303 understood climbing to FL220. QXE2303 was cleared to and had read back FL200. No operational impact.

A Pacific Coastal Airlines Saab SF340A (C-GPCN/PCO971) from Vancouver, BC (CYVR) to Masset, BC (CZMT) reported a pressurization issue at FL200. Aircraft descended to 10 000 feet and elected to return to CYVR. Landed safely on Runway 26L at 1705Z. No operational impact.

After departure, a North Star Air Pilatus PC-12/45 (C-GVKC/BF708) on a flight from Sioux Lookout, ON (CYXL) to Bearskin Lake, ON (CNE3) was cleared to FL190 and told to expect higher due to crossing traffic, a Thunder Airlines Beech A100 (C-FJJH/THU104) on a flight from Thunder Bay, ON (CYQT) to Deer Lake, ON (CYVZ) at FL200. Traffic was passed to both aircraft and BF708 was observed climbing through FL195 in close proximity (1.4 miles) to THU104, who reported traffic in sight and clear of traffic.
UPDATE TSB Report #A19C0154: C-GVKC, a Pilatus PC-12 operated by North Star Air Ltd, was operating as flight BF708 from Sioux Lookout (CYXL), ON to Bearskin Lake (CNE3), ON, with 2 crew and 8 passengers onboard. During the climb through 5 700 feet ASL, BF708 was cleared by ATC to climb and maintain FL190. At FL190 the aircraft did not level off and continued the climb to FL196. ATC asked BF708 to verify the aircrafts current altitude at which time BF708 descended to FL190. A loss of separation occurred with aircraft C-FJJH, a Beech A100 operated by Thunder Airlines as flight THU104, that was enroute from Thunder Bay (CYQT), ON to Deer Lake (CYVZ), ON at FL200. Vertical separation between the two aircraft reduced to 400 feet vertically where 1000 feet is required. There were no TCAS resolution advisories from either aircraft.

Incident at HABBS intersection

While inbound for Montreal/Pierre Elliott Trudeau, QC (CYUL) and on standard terminal arrival (STAR) HABBS4, an American, corporately registered Cessna 525B (N777NJ) from Milwaukee, WI (KMKE) to Montreal/Pierre Elliott Trudeau, QC (CYUL) did not adhere to the altitude restriction at HABBS. N777NJ crossed HABBS at FL200, while crossing altitude is 16 000 ft or less. Also, at HABBS, N777NJ conducted a left turn that was not in the plan. No impact.

A North Star Air Pilatus PC-12/45 (C-FVPC/BF705) on a flight from Sioux Lookout, ON (CYXL) to Sioux Lookout, ON (CYXL) requested descent and return to CYXL. BF705 was given descent clearance to FL200 direct CYXL. BF705 was observed on radar descending through FL200. When queried by the controller, the pilot advised on an emergency descent. The aircraft descended to 10,000ft above sea level (ASL) advised no further assistance required and landed CYXL at 1529Z. No impact to operations.
Update TSB Report #A19C0129: C-FVPC, a Pilatus PC-12 aircraft operated by North Star Air Ltd, was conducting a passenger flight from Sioux lookout (CYXL), ON to Sachigo Lake (CZPB) ON. While enroute approximately 100 nm south of CZPB, the aircraft temperature auto selection failed to produce warm air, the flight crew selected the manual temperature switch to hot. Shortly after the manual temperature selection, the flight crew experienced physiological symptoms of rapid pressure change and noted a cabin pressure indicator spike to approximately 4000 feet per minute climb with illumination of CABIN PRESSURE CAWS. The flight crew actioned the appropriate drill, commenced an emergency descent, notified ATC and returned the aircraft to CYXL. The operator’s maintenance identified the temperature control valve was unserviceable and the temperature valve was replaced. Applicable ground runs were completed and aircraft was returned to service.

A 1929927 Ontario Inc. Piper PA-46-310P (C-GHWQ) from Fort Hope, ON (CYFH) to Sudbury, ON (CYSB) was observed on radar climbing into controlled airspace without clearance. When the pilot contacted the Winnipeg Area Control Centre (ACC) controller for clearance, they reported through FL200. When queried by the controller, the pilot reported that they had the wrong frequency and was picking up icing, therefore needed to climb. No impact to operations.

At 2029Z, JSL21 was observed just climbing through FL200 and was instructed to level off at FL210. Nearest approach to a Thunder Airlines Limited Mitsubishi MU-2 (THU860) from Toronto/Billy Bishop, ON (CYTZ) to Thunder Bay, ON (CYQT) was 1000 feet and 3.3NM.

Incident at INL 180 / 15 (ATS operating irregularity)

A North Central Aviation Pilatus PC-12/47 (NCJ96) from Minneapolis, MN (KANE) to Kenora, ON (CYQK) was handed off by the MSP controller at FL200 with descent to 8,000 at the pilots discretion. The Trout Lake Controller did not recognize that the MSP controller stated the aircraft was descending to 8,000 and did not enter it in Canadian Automated Air Traffic System (CAATS). The Trout Lake Controller handed off the position to the second Trout Lake Controller. The pilot commenced descent and was immediately queried by the second Trout Lake Controller. The aircraft was recleared to 8,000. No other aircraft involved.

TSB Report #A19F0016: The Sunwest Aviation Ltd. Learjet 45, C-GVVZ, was operating as flight CNK645 from Scottsdale (KSDL), AZ to Calgary Intl (CYYC), AB. While climbing to cruising altitude, at approximately FL200, the flight crew received an AP Aileron Trim CAS message. The pilot flying disengaged the autopilot and noted that the ailerons felt frozen. The ailerons were able to be broken free with minimal effort and the autopilot was re-engaged. As per SOP’s the flight crew performed a control check every 5000 feet in the climb thereafter. During the remainder of the flight, the issue did not re-appear and CNK645 landed uneventfully. No emergency was declared.

Incident at 40NM N IQALUIT NU (CYFB) (Communication navigation surveillance/air traffic system)

While inbound for Iqaluit, NU (CYFB), a First Air Aerospatiale ATR 42-500 (FAB883) from Artic Bay, NU (CYAB) to Iqaluit, NU (CYFB) was cleared for FL230 but read it back wrong and it was not corrected. Impact: loss of separation with an Aerospatiale ATR 42-500 (FAB871) from Iqaluit, NU (CYFB) to Igloolik, NU (CYGT) climbing to FL220 in the opposite direction.
Update: TSB Report A19Q0191: C-FTIQ, a First Air ATR-42-500 (FAB883), was on an IFR flight from Arctic Bay, NU (CYAB) to Iqaluit, NU (CYFB) with 8 passengers and 3 crew members on board. There was a misunderstanding with respect to descent clearance at flight level (FL) 230 and the aircraft continued its descent below FL230. At that moment, C-FTIL, a First Air ATR-42-500 (FAB871) on an IFR flight from CYFB to Igloolik, NU (CYGT) with 21 passengers and 3 crew members on board, was climbing to FL220. Air traffic control noticed FAB883s descent below FL230 and intervened through the CYFB flight service station (FSS), asking FAB883 to stop the descent to FL200 and FAB871 to stop the climb to FL190. Due to the time required to issue clearances through the FSS, FAB883 descended to FL198 before pulling up to FL200 while the two aircraft were approximately 40 NM north of CYFB. Separation between the two aircraft was reduced to 800 ft vertically and 6 NM horizontally while the required separation is 1 000 ft vertically and 10 NM horizontally. The two aircraft were in radio contact on frequency 126.7 MHz and were displayed on their traffic alert and collision avoidance systems (TCAS). There were no injuries or damage.

At 0235Z VN02 was observed on the radar climbing through FL210, controller queried the pilot and VN02 descended to FL200. No impact on operations.

A Jazz de Havilland DHC-8-402 (C-GGOY/JZA8441) on a flight from Regina Intl, SK (CYQR) to Calgary Intl, AB (CYYC) was cleared to and read back FL200. The aircraft was observed levelling off at FL220 and queried. The aircraft was then re-cleared to FL220. No impact to operations. No other aircraft in the vicinity.

A Jazz de Havilland DHC-8-402 (C-GXJZ/JZA8589) on a flight from Saskatoon/John G. Diefenbaker Intl, SK (CYXE) to Calgary Intl, AB (CYYC) was observed on radar climbing through FL210 without clearance. When queried by the Winnipeg area control centre (ACC) controller, the pilot said they were cleared to FL220. The pilot was cleared to FL200 and the clearance was readback correctly. JZA8589 was subsequently cleared to FL220. No impact to operations.

A Keewatin Air Beech B200 (C-FXMY/KEW209) from Rankin Inlet, NU (CYRT) reported returning to CYRT due to an engine issue. No emergency declared and no assistance required. Flight landed at 0742Z without incident. No impact to operations.
Update TSB #A19C0031: C-FXMY, a Beech B200 King Air aircraft operated by Keewatin Air LP, was conducting medevac flight KEW209 from Rankin Inlet (CYRT), NU to Winnipeg/James Armstrong Richardson Intl (CYWG), MB with 5 persons on board. During the climb to cruising altitude through FL200, the right engine (PT6A-42) made a loud bang, followed by flames emitting from the exhaust. The flight crew conducted a precautionary engine shutdown, declared an emergency, and requested a return to CYRT. The aircraft landed without further incident, and taxied to the apron. The operator’s maintenance found a failed engine compressor blade.
UPDATE from company Maintenance: The pilot reported RH engine loss of torque (rolled back to 300 FT lbs) - shut down RH engine in flight. Upon investigationit was discovered that the 1st first stage compressor blade missing. #2 engine replaced with overhauled unit IAW P&W MM 72-00 and AMM 71-00 and 76-00. Leak check carried out, checked ok. Ground runs and data check carried out IAW AMM 77, checked ok. S/N engine on: PCE93931 S/N engine off: PCE94312. Aircraft released subject to satisfactory test flight. Independent inspection carried. Note: All Keewatins PT6A-42-41 engine 1st stage compressors inspected for damage. No faults found. All documented with maintenance tracking program. TSB, TC and P&W notified of incident. Engine has been sent to Standard Aero for tear down report on 1st stage compressor. P&W Canada to be involved in the process.

A Perimeter Aviation Swearingen SA226-TC (PAG955) from Thompson, MB (CYTH) to Lac Brochet, MB (CZWH) was cleared to maintain FL200. Aircraft was subsequently observed levelling at FL210 and reported level at FL210. Aircraft was re-cleared to FL200. No operational impact.

A Porter Airlines de Havilland DHC-8-402 (C-GKQA/POE256) from Saint John, NB (CYSJ) to Montreal/Pierre Elliott Trudeau, QC (CYUL) requested and was cleared to FL200 but coordinated at FL240.

A Moncton Flight College Beech C90A (C-GDHS) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) reported a laser illumination in the cockpit at FL200. The Royal Canadian Mounted Police (RCMP) was advised. No emergency or impact to operation.

Incident at In the vicinity of: MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

A Government of Quebec, Service aérien gouvernemental Canadair CL600-2B16 (series 601-3A) (C-GQBQ/QUE20) on a MEDEVAC flight from Rouyn-Noranda, QC (CYUY) to Montreal/Trudeau, QC (CYUL), while in descent at FL210, rejected its descent to FL220 after receiving a Traffic Alert and Collision Avoidance System Resolution Advisory (TCAS RA) involving a Jazz de Havilland DHC-8-102 (C-FGRP/JZA8721) Montreal/Trudeau, QC (CYUL) to Val-dOr, QC (CYVO), which was level at FL200. The information was transferred successfully, and JZA8721 had the other aircraft in sight. No impact on operations.
