Incident at 94NM NE Houston (KIAH)

TSB #A18F0276: C-FMZW, an Embraer ERJ190-100 aircraft operated by Air Canada, was conducting flight ACA595 from Toronto/Lester B. Pearson Intl (CYYZ), ON to Houston/George Bush Intercontinental (KIAH), TX with 4 crew members and 64 passengers on board. During the cruise portion of the flight, the flight crew experienced an auto-pilot disconnect, while receiving an ADS 1 FAIL and AVNX MAU 1A FAIL EICAS messages. The flight crew reverted to manual control mode, referenced the QRH, and declared a PAN PAN during the initial descent. The flight continued to KIAH without further incident, and taxied to the gate. The operator’s maintenance replaced the Module Assembly Control and carried out a few checks. The aircraft was returned to service.

Incident at ST. JOHN'S INTL NL (CYYT)

An Air Canada Embraer ERJ 190-100 IGW (C-FNAJ/ACA651) from Halifax/Stanfield Int'l, NS (CYHZ) to St. John's Int'l, NL (CYYT) reported an engine failure on the left side after landing on Runway 29 at 1921Z. The aircraft was able to exit on Runway 16. They stopped on the runway and had the emergency response vehicles inspect the left engine. There was no obvious signs. The aircraft taxied to the gate.
Update TSB Report # A18A0054: C-FNAJ, an Embraer ERJ190 aircraft operated by Air Canada, was conducting flight ACA651 from Halifax/Stanfield Intl, NS (CYHZ) to St John's Intl, NL (CYYT) with 97 passengers and 4 crew members on board. After the landing on Runway 29, the number 1 engine flamed out as the aircraft was back tracking on the runway. ARFF was called by ATC, however the flight crew did not require assistance. The flight crew continued taxing to the gate on engine number 2. The operator?s maintenance could not determine any faults and the engine, when run, operated normally. In the absence of any faults, the aircraft was released back to service.

Incident at Chicago O'Hare International Airport (KORD) (Aerodrome - runway or taxiway surface condition)

TSB REPORT#A17F0123: C-FLWK, an Embraer ERJ190-100 aircraft operated by Air Canada, was conducting flight ACA505 from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Chicago/O?Hare Intl, IL (KORD). During the intermediate approach to Runway 09L at KORD, the flight crew was notified that the runway was now closed due to a required runway inspection following a bird strike from the preceding aircraft. The flight crew executed a missed approach and proceeded to their alternate airport, Detroit/Metropolitan Wayne County, MI (KDTW). On initial approach to KDTW, the flight crew declared a minimum fuel emergency and was given a 10 nautical miles long final approach. The aircraft landed without further event with the required minimum reserve fuel.

An Air Canada Embraer E190 (ACA360) from Toronto, ON (CYYZ) to Boston, MA (KBOS) departed CYYZ, reported PAN PAN with Cleveland air route traffic control centre (CLE ARTCC) due to a hydraulic issue. The aircraft advised returning to Toronto, ON. The aircraft landed runway 23 at 1957Z and taxied to the gate with no issues. Emergency vehicles on standby. No operational impact.
UPDATE: TSB Report#A16O0173: C-FHKP, an Embraer ERJ190-100 aircraft operated by Air Canada, was conducting flight ACA360 from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Boston/General Edward Lawrence Logan Intl, MA (KBOS). During cruise flight, the flight crew received an indication of a failure for the green hydraulic system. The flight returned to CYYZ and declared a PAN PAN. The aircraft landed uneventfully with ARFF standing by. The operator?s maintenance replaced the Alternating Current Motor Pump (ACMP) 3B in accordance with AMM 29-12-01 and the aircraft was returned to service.

Incident at 33NM NNW THUNDER BAY ON (CYQT)

TSB#A15C0172: The Air Canada Airbus A320, C-FPWD, was operating as flight ACA159 from Toronto/LBPIA, ON (CYYZ) to Edmonton Intl, AB (CYEG) at FL320. Abeam Thunder Bay, ON (CYQT), a descent to FL260 was initiated. As ACA159 was descending through FL270 for FL260, a TCAS RA to adjust vertical speed was generated due to an Air Canada Embraer ERJ190 operating on a reciprocal track at FL250. The crew of ACA159 disconnected the autopilot, complied with the RA guidance by reducing the vertical speed and levelled the aircraft at the cleared altitude of FL260. Radar data shows that the two aircraft were level at their cleared altitudes and crossed 33 nautical miles NNW of CYQT with 1 000 feet vertical separation. No loss of separation occurred as the vertical separation requirement for the airspace is 1000 feet.

At 2204Z, an Air Canada Embraer E190 (ACA1117) from Toronto, ON (CYYZ) to Regina, SK (CYQR) advised departure that they had a slat problem and would require a return to CYYZ. At 2211Z, ACA1117 requested CFR (non Emergency). ACA1117 landed runway 05 at 2251Z without incident stopping on the runway waiting for inspection. Normal operations resumed at 2258Z. No operational impact.
UPDATE: TSB Report#A14O0001: The Air Canada Embraer ERJ190-100 (C-FHNL) was operating as flight ACA1117 between Toronto LBPIA (CYYZ) and Regina Int'l (CYQR). During departure the flight crew experienced difficulty retracting the flaps. The aircraft returned to Toronto and landed without further incident with ARFF standing by. Maintenance replaced the left hand slat skew sensor and harness. An operational test was carried out and the aircraft was found serviceable.

An Air Canada Embraer ERJ190 (operating as ACA 215) was on final 8nm in trail of WJA 453, was advised of bird-strike (CADOR 2012P1445) and elected to continue to land runway 08L. ACA 215 reported bird-strike at 2138Z . Runway closed after ACA 215, opened at 2145Z. Several birds found after ACA 215. All other 08L arrivals diverted to runway 08R. No significant delays.

At 0520z Vancouver Airport Authority (YVRAA) operations reported a security threat regarding an Air Canada Embraer ERJ190, flight ACA298, IFR Vancouver (CYVR) to Winnipeg (CYWG). ACA298 was instructed to hold on Apron VI pending further information. At 0531z YVRAA operations indicated that the threat was non-specific and authorised ACA298 to depart. Incident at Kelowna (Conflict - potential, ATM - other)

UPDATE from NavCanada: An Air Canada Embraer ERJ190, operating flight ACA197, issued an approach for CYLW, reported responding to a "TCAS RA" [traffic collision avoidance system resolution advisory] and subsequently climbed. A privately registered Piper 34 200T - VFR was the aircraft ACA197 responded to. Both aircraft were in Class E airspace. Both aircraft were known to each other and traffic had been passed by the controller; however, neither had visual contact.
An Air Canada Embraer ERJ190, operating flight ACA197, IFR Toronto (CYYZ) to Kelowna (CYLW), issued an appoach for CYLW, reported responding to a "TCAS TA" and subsequently climbed. A privately registered Piper 34 200T, VFR Kelowna to Edmonton/Cooking Lake, AB (CEZ3) was the aircraft ACA197 responded to. Both aircraft were in Class E airspace. Both aircraft were known to each other and traffic had been passed by the controller.

An Air Canada Embraer ERJ190, IFR Vancouver (CYVR) to San Francisco (KSFO), operating flight ACA564, departed YVR for SFO and experienced an electrical problem. The pilots decided to return for a landing at YVR Runway 08L (non-emergency). ACA564 landed normally and taxied off the runway. Runway returned to use after inspection.

The flight crew reported to air traffic control that they had been struck by a green laser that appeared to originate seven miles north of the airport.

Incident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG) (Conflict - potential, ATS operating irregularity)

WJA124, a Boeing 737-800, operated by WestJet was en route from Calgary to Toronto and cleared to FL390 with opposite direction traffic ACA 1153, an ERJ190, operated by Air Canada which was en route from Toronto to Calgary. The Winnipeg ACC controller realized the situation and issued 30 degree turns to both aircraft. Separation was maintained.

At 1551z ACA1113 ERJ190 reported to tower that it had a bird strike on final runway 13. No impact on operations.
UPDATE Airport staff were unable to locate any bird remains. Bird activities continue to be monitored in accordance with the Regina Airport Wildlife Management Plan. There was no impact on airport operations.

ACA652, Embraer ERJ190, enroute from Halifax (CYHZ) to St. John?s (CYYT), advised that they were returning to Halifax due to "technical Issues" and that no assistance was required. The Airport Duty Officer was advised and Aircraft Rescue and Fire-Fighting (ARFF) were notified and remained on standby in the fire hall. The flight landed on Runway 05 without incident at 15:46Z.

The Air Canada Embraer ERJ190-100-IGW aircraft (operating as ACA1161) was on a scheduled IFR flight from Halifax International Airport (CYHZ) to Toronto (LBPIA) (CYYZ). At 2042Z, the flight crew advised Montréal ACC that they required a slow descent due to a minor pressurization problem. No emergency was declared or special assistance was required. The aircraft landed at Toronto at 2128Z without further incident. Ops. impact - none.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

Initial information received from TSB Daily Notification Log [#A10O0108]: The Air Canada ERJ190-100-IGW (registration C-FNAI, operating as ACA1121) was on a scheduled IFR flight from Toronto (LBPIA) (CYYZ) to Saskatoon International Airport (CYXE). The aircraft departed Toronto and was climbing through flight level (FL) 200, when the crew became unable to control the cabin temperature. The aircraft returned to Toronto and landed overweight. Maintenance staff changed the right-hand pack bypass valve as per AMM 21-51-10, carried out an operational check of cabin zone temperature control as per AMM task 21-62-00-710-801-a/500, and changed right hand pack outlet temp sensor as per AMM 21-51-18.

Incident at DAUPHIN (LT. COL W.G. (BILLY) BARKER, VC APRT) MB (CYDN) (Wake turbulence/vortices)

As reported by TSB, ACA1127 reported being affected by wake turbulence from a B744 that passed over top of them. No prior awareness of any difficulty was previously made to the ACC.
UPDATE ERJ190-100 operated by Air Canada ACA1127 was cruising enroute at FL340. An opposite-direction B747 passed overhead at FL350 on a reciprocal track. About one minute after the flights passed, ACA1127 experienced a roll excursion to the left and a nose-down pitch excursion. During the attitude excursion, ACA1127 descended to FL332 and deviated about one NM left of track. AC1127 climbed back to FL340 and continued to destination without further incident.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ) (Overshoot/missed approach)

CFC4024, C17, was issued clearance to taxi to position in Halifax (CYHZ) with ACA2108, Embraer ERJ190, enroute from Montreal (CYUL) to Halifax, on final approach. CFC4024 was then cleared for takeoff but did not reply. After several unsuccessful attempts to re-establish contact, the tower then initiated a overshoot of ACA2108.

ACA144, Embraer ERJ190, enroute from Montreal (CYUL) to Halifax (CYHZ), conducted a missed approach while on final approach for Runway 32, due to CFG02, Boeing 767-300, enroute from Frankfurt (EDDF) to Halifax (CYHZ), rolling past Taxiway Hotel after landing and slow to turn around and vacate the runway. ACA144 completed a VFR circuit and landed routinely on Runway 32.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ)

UPDATE A10A0051: An Air Canada Embraer E190 (C-FHNP), departed Halifax for Toronto. During the climb the flight crew advised of a pressurization malfunction. The crew did not declare an emergency; however, they requested and received a clearance to return to Halifax. They carried out a rapid descent to 9000 ft. The aircraft landed without further incident. Prior to the incident flight, the aircraft had been released to service with the number two bleed air system disabled in accordance with the MEL. During the climb, the number one system malfunctioned causing the depressurization. After repairs the aircraft was returned to service.
ACA611, Embraer ERJ190, enroute from Halifax (CYHZ) to Toronto (CYYZ), in a climb to 22,000 feet reported a pressurization problem and requested clearance to descend and return to Halifax. The aircraft conducted a rapid descent to 9,000 ft. No assistance was required. The aircraft landed safely. TSB Case Closed.
