A Trans States Airlines Embraer EMB145 (LOF4687), from Chicago/O'Hare, IL (KORD), to Montréal/Pierre-Elliott-Trudeau, QC (CYUL), and an Air Canada Boeing 787-8 (ACA300), from Vancouver, BC (CYVR), to Montréal/Pierre-Elliott-Trudeau, QC (CYUL), were diverted to Runway 06R. No other impact on operations.

FAA Report sent December 29, 2014: A Morningstar Partners Ltd Bombardier CL-600-2B16 (C-GBKB) failed to comply with an altitude crossing restriction of 1,500 feet conflicting with an ExpressJet Airlines Inc Embraer EMB145 (ASQ687T) 6 NM west southwest of Teterboro, NJ. Closest proximity was 600 feet vertical and 1.92 miles lateral.

Incident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG)

UPDATE: TSB Report#A14C0057: A14C0057: The ExpressJet Airlines Embraer EMB145 (N145587) Flt ASQ6005, was descending into Winnipeg (CYWG). Approximately 30 miles from Winnipeg the flight was cleared to descend to 7000 feet. The crew moved the thrust levers to idle and there was a popping sound. The noise was apparent during speed adjustments when the thrust levers were brought to idle. The crew confirmed the sound however engine indications appeared normal. The next time the thrust levers were brought back to idle; the #2 engine (Rolls Royce AE3007-A1P) ITT spiked and began an uncommanded shutdown. The crew declared an emergency and performed the QRH functions. The flight was cleared to 4,000 feet. While being vectored, the QRH was completed and the #2 engine was restarted. All engine indications were normal during the restart and the crew prepared for a 2 engine approach into YWG. After being cleared for the approach the thrust levers were brought back to idle. The #2 engine ITT indications were lost and again the engine began another uncommanded shutdown. The crew performed the QRH functions and requested additional vectors. The approach was cancelled. When the QRH checklist was complete the crew elected to leave the engine #2 off and perform a single-engine ILS approach. The subsequent landing was uneventful with ARFF responding as the aircraft cleared the runway. The crew received the "all clear" and taxied to the gate without further incident.
An ExpressJet Airlines Embraer EMB 145 (ASQ6005) from Chicago, IL (KORD) to Winnipeg, MB (CYWG) reported #2 engine out and declared an emergency. ASQ6005 advised engine re-lit and went out again on final. Aircraft rescue and fire fighting (ARFF) on standby. ASQ6005 landed safely at 0208Z. No impact to operations. Civil Aviation Contingency Operations (CACO) advised.

Incident at 12 NM NE MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

UPDATE: TSB Report#A13Q0213: An Atlantic Southeast Airlines Embraer EMB145, Flight ASQ4200, was on approach 12 nm north east of CYUL when the aircraft was observed on a southbound heading when it had been given instruction to fly 080. Consequently, the separation between Flight ASQ4200 and a Travel Service Boeing 737, Flight SWG9787, was reduced to 2 nm while the minimum separation required is 3 nm.
Update: Aircraft Operator changed from Sunwing Airlines to Travel Service (wet lease).
An Expressjet Airline Embraer E135 (ASQ4200) flying from Cleveland, OH (KCLE) to Montréal, QC (CYUL) received and read back instructions to follow hdg 080, but followed a south heading. Separation with a Travel Service Boeing 737 800 (SWG9787) flying from La Ceiba (MHLC) to Montréal, QC (CYUL) was reduced to 2 miles where separation of 3 NM is required.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

The ExpressJet Airlines Canadair CRJ-900 aircraft (operating as flight ASQ5249) was concluding an IFR flight from Hartsfield-Jackson International Airport (KATL) to Toronto LBPIA (CYYZ). The ExpressJet Airlines Embraer EMB-145 aircraft (operating as flight ASQ4605) was initiating an IFR flight from Toronto LBPIA (CYYZ) to Cleveland Hopkins International Airport (KCLE). NAV CANADA staff at Toronto Tower reported that ASQ5249 landed RWY 24L exited D4 and was instructed to hold short RWY 24R, which was read back correctly. The aircraft was observed to cross the hold short line and was instructed to stop. The aircraft stopped at the edge of RWY 24R as ASQ4605 was on the roll and airborne between D2 and D4.
UPDATE: TSB A13O0071: The ExpressJet CRJ700 (N754EV), operating as flight ASQ5249, was inbound to CYYZ Toronto from KATL Atlanta. After landing on Runway 24L ASQ5249 was instructed by ATC to exit on taxiway D4 and hold short of runway 24R. The hold short instruction was read back by the flight crew. Shortly thereafter an ExpressJet EMB145, operating as flight ASQ4605, was cleared to take-off on Runway 24R. While ASQ4605 was on the take-off roll ATC observed ASQ5249 crossing the hold short line and instructed them to stop. ASQ5249 stopped just before the edge of Runway 24R. ASQ4605 became airborne while passing taxiway D2.

Embraer EMB-145SEP aircraft (operating as flight EGF4112) was on a scheduled IFR flight from Toronto (LBPIA) (CYYZ) to Chicago (O'Hare) International Airport, IL (U.S.A.) (KORD). The Air Canada Airbus A-320 aircraft (operating as flight ACA556) was nearing the conclusion of a scheduled IFR flight from Las Vegas (McCarran) International Airport, NV (U.S.A.) (KLAS) to Toronto (LBPIA) (CYYZ). NAV CANADA staff at Toronto ACC reported that a loss of both vertical and lateral separation occurred when the two aircraft were 53NM west of Toronto (LBPIA). Ops. impact -- unknown.
UPDATE Supplemental information received from T.S.B. Daily Notification [#A11O0060]: American Eagle flight EGF4112, an EMB145, was departing Toronto (LBPIA) (CYYZ) destined for Chicago (O'Hare) International Airport, IL (U.S.A.) (KORD). Air Canada flight ACA556, an A-320, was inbound to Toronto (LBPIA) (CYYZ) from Las Vegas (McCarran Field) International Airport, NV (U.S.A.) (KLAS). A loss of separation occurred 53NM west of Toronto (LBPIA) when lateral spacing was reduced to four (4.0) miles in an area where five (5) miles is required.

UPDATE from TSB Daily Notification Log 02-12-2009 Occurrence Summary A09O0252: The American Eagle EMB145, N606AE, operating as EGF4187, was inbound to Toronto from Chicago. On descent through 12,000 feet the aircraft began to accumulate ice and shortly thereafter the crew received a "A-ICE STAB FAIL" EICAS message. Some minor control difficultly was experienced; however this was rectified by an increase in airspeed. The appropriate checklists were completed, an emergency was declared, and the aircraft landed without further incident.
UPDATE EGF4187 on a scheduled Instrument Flight Rules (IFR) flight from Chicago O'Hare International Airport, Chicago, IL, U.S.A. (KORD) to Pearson International Airport, Toronto, ON, (CYYZ) declared an emergency. The aircrew reported control problems due to ice on the stabilizer. The aircraft was immediately vectored by Pearson Control Tower and landed RWY 24R.

At approximately 4,000 ft, BTA3076, Embraer EMB145, enroute from Newark (KEWR) to Halifax (CYHZ), reported several cockpit strikes from a green laser. The source of the beam seemed to be near the Halifax Harbour. TSB Case Closed.
UPDATE TSB: BTA3076, a Jet Link Embraer E145 en route from Newark to Halifax was approaching Halifax when, at 4000 feet ASL, the flight crew reported several cockpit strikes from a green-colored laser. They also advised that the source of the beam appeared to be near Halifax harbour. The aircraft landed safely without further incident. The crew of another airline reported that they had a similar occurrence on the previous evening (See AOR 112429).

As BTA4141 (EMB145) was taxiing for take-off at Dorval Airport, the crew was forced to return to the gate by a washroom fire message. Emergency services were advised. No impact on operations. Alors que BTA4141 (EMB145) circulait pour le décollage à l'aéroport de Dorval, l'équipage a dû retourner à la barrière à cause d'une indication de feu dans la toilette.
