Incident at CHARLOTTETOWN AIRPORT

A Government of Canada, Department of National Defence, Westland EH101 (TSKR902) on a flight from Summerside, PE (CYSU) to Sydney/J.A. Douglas McCurdy, NS (CYQY) was observed on radar entering Charlottetown, PE (CYYG) control zone (CZ) at 2900 ft without contacting CYYG mandatory frequency (MF). CYYG Radio attempted to contact the aircraft prior to it entering the MF with no response, aircraft working with Moncton area control centre (CZQM). Initial contact with TSK902 was made at 3NM N CYYG at A029 transiting through the control zone.

A westbound Government of Canada Department of National Defence Augusta West land EH101 helicopter (RSCU905) from Gander, NL (CYQX) to Sydney, NS (CYQY) departed CYQX at 2552Z. 60NM west of the airport, the aircraft reported shutting down one engine and returning to CYQX. The aircraft declared PAN. The flight landed safely at 0003Z.

Incident at GANDER INTL NL (CYQX)

At 1238Z, pilot of a Government of Canada Department of National Defence EH101 (148815 / OCST915) from Gander, NL (CYQX) to Argentia, NL, advised they were returning to Gander due to an issue they had onboard. This issue was a problem with the nose wheel steering. Aircraft was approximately 5 miles South of CYQX. Aircraft requested landing on the Search and Rescue Apron due to this problem. No emergency declared, and no assistance required. VFR flight plan closed by CYQX tower.

Incident at Vicinity of Gander (ELT)

At 1611Z, an Emergency Locator Transmitter (ELT) burst was heard by Gander Tower, Gander Low Domestic and Outcast (OCST) 905, that lasted for about 12 seconds. OCST905, an EH101 helicopter, cancelled their Instrument Flight Rules (IFR) flight plan and investigated the area where the ELT originated from and could not find any sign of trouble. The Rescue Coordination Center (RCC) was advised. It was believed to be an inadvertent ELT.

UPDATE from ANS & Airspace: No further action required.
A privately registered Piper PA46, IFR Tofino (CYAZ) to Victoria (CYYJ), was issued an IFR clearance off CYAZ to 11,000 feet. Subsequently the West sector controller instructed the Piper PA46 to maintain 9,000 feet after it was determined that the Canadian Armed Forces Cormorant Westland EH101 helicopter, operating as SNAKE904 Comox (CYQQ) to CYAZ, was IFR, and not VFR as stated by CYQQ Terminal Control Unit when the aircraft was handed off. There was no operational impact.

A Government of Canada, Department of National Defence Agusta Westland EH101 (Cormorant) operating Flight SNAK906, VFR Tofino/Long Beach (CYAZ) to Victoria (CYYJ), was observed on Auxiliary Radar Display System (NARDS) at 4819N 12332E altitude 100 ft above Mean Sea Level within CYR103. CYR103 active by NOTAM. Aircraft exited CYR103 as communication established with Victoria Harbour (CYWH) FSS.

Incident at GANDER INTL NL (CYQX)

OCST912, EH101 helicopter, inbound to Gander (CYQX), on an ILS 13 approach, reported being unable to pick up the localizer at 05:36Z. The flight switched back to the Area Control Centre (ACC) on 132.1 and was re-cleared for RNAV 21 approach. OCST912 subsequently cancelled IFR and landed on Taxiway "C" at 05:57Z. The localizer indicated being serviceable. It was later determined that the incorrect frequency was accessed by the flight crew.

OCST912, EH101, departed from Runway 21 in Gander (CYQX) for an IFR flight to Wesleyville, NL. The aircraft returned to the field with issues pertaining to the anti-icing equipment. No emergency was declared and no assistance was required. The helicopter landed without incident at 18:02Z.

Incident at ST. ANTHONY AIRPORT

OCST910, EH101, enroute from St. Anthony (CYAY) to St. John?s (CYYT), taxied to Runway 10/28 and departed without contacting FSS or broadcasting on the Mandatory Frequency (MF). Several attempts were made to contact aircraft on the mandatory and emergency frequencies without response from the aircraft. The aircraft contacted FSS after departure and exited the zone without further incident. Nil TSB.

Tuscar 941, a military EH101 Cormorant helicopter, was on a VFR search and rescue mission out of Greenwood, NS. The pilot reported difficulties and ditched the aircraft in the ocean 3 km south east of Canso, NS (4521.5N, 6100.3W). There were 7 persons on board. Coast Guard responded and rescued 4 persons. Subsequently divers located the 3 remaining persons on board inside the fuselage, deceased. Nil TSB.

SNAK902 was called, responded and stated his intentions were to proceed South West bound to Port Hardy (CYZT). Pilot stated he had tried to call.
