Incident at KUUJJUARAPIK QC (CYGW) (Aerodrome, runway or taxiway shutdown, ATM - other)

At Kuujjuarapik, QC (CYGW) the original NOTAM for Runway 03/21 indicated that the runway was closed from 140000Z to 141900Z. At approximately 1420Z, a replacement NOTAM (NOTAMR) was received indicating that Runway 03/21 was closed until 141700Z. At 1523Z, a cancellation NOTAM (NOTAMC) was received indicating that Runway 03/21 was open (Quebec City (YQB) flight information centre (FIC) was advised by e-mail by the Kativik Regional Government (KRG) and Kuujjuaq; ground personnel not advised). An Air Inuit Ltd. de Havilland DHC-8-314 (C-FOAI/AIE802), from Montréal/Pierre Elliott Trudeau, QC (CYUL) to Kuujjuarapik, QC (CYGW) landed on Runway 03 at 1647Z when the runway was considered officially open. At 1650Z, a vehicle (ST32) requested permission to drive on the runway to finish the work. The vehicle was not aware that Runway 03/21 was reopened as of 1523Z. Runway 03/21 closed from 1650Z to 1705Z by NOTAM via voice communication.

Incident at CALGARY INTL AB (CYYC)

A WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3270) from Calgary, AB (CYYC) to Regina, SK (CYQR) departed, then reported a pressurization issue and requested to return to CYYC, with no assistance required. The tower contacted the Security Operations (IOC) unit with a code orange. The aircraft landed safely at 0141Z. No impact to operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. On climb-out through approximately 10,000, the crew received a fuselage door warning light. The aircraft levelled at 13,000 to carry out the checklist. The right-hand (RH) emergency exit (E-exit) door was indicating "open" on the doors page. At 13,000, the cabin altitude (ALT) was reading 4000 ground level. Operations (OPS) check of the forward (FWD) E-exit door was accomplished in accordance with (IAW) aircraft maintenance manual (AMM) 52-24-00, with no fault found. Proximity Sensing Electronics Unit (PSEU) values checked IAW AMM 32-61-00, with no faults found for the E-exit door. Pressurization run carried out (C/O) IAW AMM 21-31-00. No faults found. Aircraft is serviceable at this time.

The CYGT community aerodrome radio station (CARS) missed two weather observations. The CARS of CYXN had no (NIL) weather (WX) observations published from 1300Z to 1400Z. The stations were called, with no response heard (NRH). The stations were closed via NOTAM at 1419Z. Subsequently, CYXN was opened and a cancelling NOTAM (NOTAMC) was issued at 1607Z. The CARS of CYLK did not publish weather observations from 1500Z until 1600Z. The station was called, with NRH. The station was closed via NOTAM (i.e. NOTAMed closed) at 1616Z.

The community aerodrome radio station (CARS) of Igloolik, NU (CYGT) missed its first two observations. At 1215Z, the station was closed via NOTAM from 1000Z to 2100Z. CYGT then called to open. NOTAM cancelled. At 1452Z, the CARS of Naujaat, NU (CYUT) was closed from 1145Z to 2215Z. CYUT then called to open at 1330Z. NOTAM cancelled. The CARS of Aklavik/Freddie Carmichael, NT (CYKD) missed its first two observations. At 1415Z, the station was closed via NOTAM from 1300Z to 2300Z. CYKD opened the station at 1552Z. The CARS of Lutselke, NT (CYLK) missed its first two observations. At 1520Z, the station was closed via NOTAM from 1400Z to 0000Z. The CARS of Wrigley, NT (CYWY) missed its first two observations, and was closed via NOTAM from 1200Z to 2200Z at 1520Z. CYUT hours of operations were extended, and a NOTAM was issued from 0124Z to 0520Z. The CYUT call out was subsequently ended, and the NOTAM was cancelled.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

UPDATE: TSB Report#A18O0006: C-GBZR, a Boeing 767-38E aircraft operated by Air Canada rouge, was conducting flight ROU1823 from Toronto/Lester B. Pearson Intl., ON (CYYZ) to Honolulu/Daniel K. Inouye Intl., Hawaii (PHNL) with 8 crew members and 274 passengers on board. During climb, the flight crew selected the landing gear up, and received a GEAR DISAGREE EICAS message. The crew noticed that the left main landing gear was indicating green, with a door open indication. The aircraft climbed to 9000 feet ASL, and orbited north of CYYZ where the crew declared a PAN PAN before returning to CYYZ. The operator?s maintenance performed several cycles of the landing gear, and determined that the left main landing gear actuator was unserviceable. The actuator was replaced and subsequent cycles determined that no further fault existed.
An Air Canada Rouge Boeing 767-38E (C-GBZR/ROU1823) on a flight from Toronto/Lester B. Pearson, ON (CYYZ) to Honolulu, HI (PHNL) declared a PAN PAN after departing Runway 23 due to a possible gear door issue. The aircraft requested Aircraft Rescue and Fire Fighting (ARFF) and landed Runway 23 at 0240Z without incident.

A Calgary Flying Club Piper PA-34-200T (C-GXAR) from Calgary/Springbank, AB (CYBW) to Calgary/Springbank, AB (CYBW) departed on an IFR training flight. Two minutes after departure, C-GXAR informed departure that they needed to return due to an open door. C-GXAR cancelled IFR and joined the circuit. No impact on operations.
UPDATE from Flight Operations: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Door became unlatched inflight. Aircraft returned and landed to address the unlatched door issue.

Incident at MONT-JOLI QC (CYYY) (Aerodrome, runway or taxiway shutdown)

A privately registered Cozy III, on a local flight from Mont-Joli (CYYY), QC, left Runway 06 via Taxiway C (closed). The flight service station (FSS) confirmed that the Cozy III was clear for the arrival of a privately registered Cessna 172N, from Chicoutimi (CYRC), QC, to Mont-Joli (CYYY), QC. The Cessna 172N landed. The FSS asked the two aircraft to backtrack on Runway 06 in order to leave at Taxiway G (open). An Air Cadet League of Canada Cessna 305A (C-FTGC/ SDOG60), on a local flight from Mont-Joli (CYYY), QC, was advised and confirmed that it would lengthen its flight to enable the two aircraft to backtrack and leave the runway. The Cessna 172N did not respond to the request to backtrack on the runway and headed to Taxiway C (closed) and then to Taxiway D (closed). Glider landing activities were taking place on Taxiway D (closed). The people responsible for glider activities on Taxiway D notified the FSS that they had indicated to the Cessna 172N to reintegrate Runway 06 via Taxiway E (closed) since an Air Cadet League of Canada Schweizer SGS2-33A (C-FZIQ), on a local flight from Mont-Joli (CYYY), QC, was inbound for landing on Taxiway D (closed). The Cessna 172N and the Cozy III eventually left Runway 06 on Taxiway G (open). It was not possible to contact the Cessna 172N during ground taxi.

An Air Canada Embraer E190 (ACA513) on a flight from Toronto/Lester B. Pearson, ON (CYYZ) to Chicago/O'Hare, IL (KORD) declared Pan Pan and advised of an open door indication. The flight requested to return to Toronto.
UPDATE: TSB Report#A17O0117: The Air Canada Embraer E-190 aircraft (AC513, C-FNAW) was on the climb out from Lester B. Pearson Intl Toronto (CYYZ) when the crew received an unsafe main cabin door indication, followed by the crew not being able to pressurize the aircraft. A PAN PAN was declared with ATC and the aircraft returned to CYYZ. The landing was uneventful with ARFF on stand by. Maintenance replaced the Proximity Sensor Electronic Module 1 (PSEM) IAW AMM Task 32-61-01 PB 401 and system was checked serviceable. The aircraft was returned to service.

Accident at 2NM E PONOKA INDUSTRIAL (LABRIE FIELD) AB (CEH3)

TSB Report#A17W0106: an Amateur Built Just Aircraft Highlander, had departed from Ponoka Industrial (Labrie Field), AB (CEH3) for a private farm strip about 3 nautical miles East of CEH3. Shortly after leveling off after takeoff, the pilot heard a bang from the engine (Rotax 914 EFI/Turbo) and a strong smell of oil. The pilot headed for an open field and, during the descent, saw flames coming from the engine cowling area. The engine was shut down and the descent speed was increased in an attempt to extinguish the flames; however, that was unsuccessful. The pilot completed a forced landing and, after evacuation, attempted to extinguish the fire without success. The fire consumed the entire aircraft except for the vertical and horizontal stabilizers. Regional fire fighting services were on scene within 10 minutes. The pilot, who was the sole occupant, was uninjured.
