Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL) (Aerodrome, runway or taxiway shutdown)

A Lufthansa Airbus A330-300 (DLH475), from Munich (EDDM), Germany, to Montréal (CYUL), QC, aborted takeoff from Runway 24R because of a vibration felt on board. The crash alarm was activated. After a few checks, DLH475 taxied to the terminal. Runway 06L/24R was closed from 0050Z to 0117Z. Impact: All traffic was diverted to Runway 28, a Trans States Airlines Embraer EMB-145XR (LOF4647), from Chicago (KORD), IL, to CYUL, was instructed to pull up from Runway 24R and was delayed by 12 minutes on arrival and a Royal Air Maroc Boeing 787-8 (RAM208), from Casablanca (GMMN), Morocco, to CYUL, was delayed by 16 minutes on arrival. The National Operations Centre (NOC) and Aviation Operations Centre were notified.

An Air Canada security vehicle did not stop at the Route 630 traffic lights on Taxiway ALPHA. The light was fixed red for a Lufthansa Airbus A330-300 (DLH475), from Montréal (CYUL), QC, to Munich (EDDM), Germany, which was taxiing on Runway 10/28 towards Taxiway ALPHA.

A Lufthansa Airbus A330-300 (DLH475) from Montreal, QC (CYUL) to Munich, Germany (EDDM) reported a medical emergency, requested and was issued clearance to CYUL. No assistance required, Boston Area Control Centre (BOS ACC) was advised. The aircraft exited Moncton Flight Information Region (QM FIR) at 0357Z. DLH475 landed at CYUL at 0451Z. No impact on the operation.

Update: According to the Aerodrome Safety Branch, measures were taken against the employee who made the mistake.
While taxiing towards the Runway 06R holding bay, a Lufthansa Airbus A330-300 (DLH475), from Montréal (CYUL), QC, to Munich (EDDM), reported that it had to apply the brakes to avoid a vehicle that had cut it off. The vehicle was on the apron. The airport authority was notified.

Incident at Radiophare (NDB) St-Félix de Vallois (UFX) (ATS operating irregularity)

Deutsche Lufthansa Airbus A-340 DLH475 was IFR from Montreal/Trudeau (CYUL) to Munich (EDDM). There was an operating irregularity when DLH475 flew UFX YQB MIILS while ATC had UFX ANCER as the result of a handling error. No impact on ATC operations.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

DLH475, an Airbus A-340 operated by Deutsche Lufthansa, was on an IFR flight from Montréal/Trudeau (CYUL) to Munich (EDDM). Due to the weather, the crew requested to turn right while leaving 2 000 ft upon takeoff from Runway 06R. As a result, the applicable noise abatement procedures were not followed.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL) (TCAS alert)

At Montréal/Trudeau (CYUL), all departures from Runway 24L received a resolution advisory from the traffic alert and collision avoidance system (TCAS) during takeoff, with relation to the threshold of Runway 06R, between 0020Z and 0105Z. There was no known or observed traffic in the area.
Update #2: According to TSB information, the source of the TCAS notices was C-GISY, a Global Express, undergoing maintenance in a Bombardier hangar, located on Stuart-Graham Blvd., between the 2 thresholds of runways 06. The altimeter of C-GISY indicates 1900 feet, which is in line with the TCAS notices. The personnel were advised and corrective actions were taken.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Communication navigation surveillance/air traffic system)

The Deutsche Lufthansa Aktiengesellshaft (Lufthansa German Airlines) Airbus A-340-300 series aircraft (operating as flight DLH471) was departing on a scheduled IFR flight from Toronto (LBPIA) (CYYZ) to Frankfurt am Main International Airport (Germany) (EDDF). NAV CANADA staff at Toronto Tower reported that the aircraft departed off of runway 05 on the LESTER SEVEN DEPARTURE at 2253Z. Tower staff observed the aircraft in a right turn after take-off and immediately instructed DLH475 to turn left. DLH475 indicated that they were intercepting the Toronto VOR (YYZ) 079º radial. Ops impact -- none.

Update #1: The TSB occurrence number and class of investigation were added. According to TSB report #A10Q0150: The CRJ2 (RJ800) operated by Air Canada Jazz as flight number JZA7744, was on an IFR flight from the Montréal/Trudeau (CYUL) airport to Newark (KEWR). A loss of separation occurred when JZA7744 flew 4.5 NM behind the Airbus 340 (A340-300) operated by Lufthansa as flight number DLH475, which was on a flight from the Montréal/Trudeau (CYUL) airport to Munich (EDDM). The required separation was 5.0 NM horizontally.
An operating irregularity occurred due to a loss of separation when JZA7744, a Regional Jet operated by Air Canada Jazz, on a flight from Montréal/Trudeau (CYUL) was 4.5 NM behind DLH475, an Airbus 340 operated by Deutsche Lufthansa. The required distance was 5 NM.
Update #2: According to information received from the ANS and Airspace Division, an operational safety investigation will be conducted by NAV CANADA.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

JZA964, a de Havilland Dash 8 operated by Air Canada Jazz, was preparing to conduct an instrument flight rules (IFR) flight from Montréal/Trudeau (CYUL) to Mont-Joli (CYYY). The tower controller instructed the crew to abort takeoff from Runway 28 because the aircraft had started its take-off run without authorization. The controller wanted to maintain wake turbulence separation behind DLH475, an Airbus 330-300, which had just taken off from Runway 24R.

Upon departure from Runway 06 at Montréal/Trudeau (CYUL), bound for Munich (EDDM), the crew of DLH475, an Airbus 340-400 operated by Deutsche Lufthansa, did not comply with the DORVAL 6 standard instrument departure (SID) that it had been assigned. The crew conducted the KIPAX2 SID. A bomb alert was received by Aéroports de Montréal employees for Lufthansa Airbus 340-300 DLH475. The search was cancelled at 0039Z because the alert was not specific to the aircraft. The passengers were able to embark DLH475, and at 0114Z the aircraft took off for Munich (EDDM).

Incident at 14 Rotherglen Road South, Ajax

On January 29, 1998, the home owner at 14 Rotherglen Road in Ajax, Ontario reported finding some aircraft parts in his yard. Discussion with the home owner suggested that the parts landed on his propery sometime during the night of Wednesday, January 28 or the morning of Thursday, January 29. A Transportation Safety Board official obtained the parts from the home owner on January 30 and transported them to the facilities in Richmond Hill. The parts appear to be part of a large panel possibly, from a Boeing aircraft. The panel parts are similar in design the the panels found between the wings of a Boeing 727 just forward of the main landing gear. However, positive identification of the parts is not possible at this time. OPI: PAH Supplemental information from T.S.B. Initial Notification (#A98O0095): Ten minutes after take-off from Toronto, at an altitude of 12,000 feet, a sudden bang was heard in the cabin of flight 475. This was accompanied with the loud noise of rushing wind (aircraft at 280 knots) and vibration in the floor at doors 3L and 3R. The noise and vibration lessened when the #3 air conditioning pack was turned off. The aircraft continued to Frankfurt without further incident. Upon arrival into Frankfurt, a ground inspection revealed that the access doors to the #1 and #3 air conditioning packs were missing. Also, the panel behind the #3 door was damaged. Besides the noted damage, two of the locks on the #2 air conditioning access door were secured improperly. This incident coincides with the discovery of some aircraft panels at 14 Rotherglen Road South, Ajax on January 29, 1998 and 92 Lincoln Street, Ajax on February 1, 1998.
