Incident at 665751N 1374743W

A Trans North Turbo Air Bell 206B (C-GPWI) on a local flight from Wiley, YT (CAJ2) became overdue. Trans North used tracker to locate aircraft on ground at co-ordinates. Kamlooops, BC checked tapes for filing and discovered flight plan was incorrectly filed with an enroute time of 6 hours when it should have been 13 hours. No impact on operations

Accident at N6140 W11357

UPDATE TSB A13W0132: A Yukon Stone Outfitters owned float equipped Piper PA18-150 Super Cub (C-GQHC) was in the process of conducting an aerial drop of supplies in the vicinity of Northern Lake, YT. After completing the drop, C-GQHC attempted to climb out through a creek bed. The aircraft collided with terrain. The pilot and passenger received minor injuries. The aircraft was substantially damaged. Victoria Rescue Control Centre was notified via the 406 ELT.
JRCC Victoria [V2013-02551]: At 052235Z. 406 ELT: A78DF01BE8402F1 (614050N 1335930W - YUKON). CMCC REPORTED A 406 ELT 70 NM NE OF WHITEHORSE. TRANS NORTH AIR HELO CHARTERED AND HAS FOUND THE 2 POB IN A HUNTING CABIN NEAR FORCED LANDING SITE, NO INJURIES. POB'S FLOWN BACK TO WHITEHORSE AND TRANSFERRED TO EHS. GPWI - 4.6, BUF 457 442 - 0.0
Victoria JRCC called Whitehorse, YT (CYXY) FSS and reported a 406 ELT belonging to a 45325 Yukon Inc. Piper PA-18-150 (C-GQHC) approximately 65 NE of Whitehorse, YT. CYXY FSS contacted the owner of the aircraft and queried him on information about the aircraft. He indicated that the aircraft was enroute to Northern Lake, YT (6144N 13348W) which corresponds approximately with JRCC's satellite info. A local helicopter was dispatched by YYJ JRCC with the owner on board and they found the aircraft at approximately 0200Z. The aircraft had sustained minor damage on a hard landing. No impact to operations.

Incident at WHITEHORSE/ ERIK NIELSEN INTL (CYXY) (Conflict - potential, Communication error, Alleged Canadian Aviation Regulations (CARs) infraction)

A Trans North Turbo Air Bell Textron 206B (C-GPWI) on a VFR flight called for departure (located 1/2 NM from threshold 13R on the approach) with a Air North Charter Boeing 737 (C-FANB/ operating as ANT506) on a IFR flight on base turn for final for runway 13R. C-GPWI was issued an immediate departure reference ANT506 turning final. C-GPWI departed and then when 1/2 NM west of the approach announced a 2nd helicopter would be following them. The 2nd helicopter, a Trans North Turbo Air Bell Textron 206B (C-GTNY) on a VFR flight departed without establishing contact with control tower or receiving clearance. ANT506 was approx. 2 NM final for 13R when C-GTNY departed from Trans North.

Incident at DAWSON CITY YT (CYDA)

C-GPWI, a Bell 206 operated by Trans North Turbo Air, was on a round robin VFR flight from Dawson City and Whitehorse FIC conducted a communication search when the pilot failed to close his flight plan.

Incident at SMITHERS BC (CYYD)

At 1654Z the Pacific Western Helicopters Bell B06, VFR to Smithers, reported over Telkwa inbound approximately 8 NM SE, working low level, ETA 1710Z. No further communication was received until after the aircraft landed at 1704Z.
UPDATE from C&BA: The Operations Manager undertook to determine who the subject pilot was, brief them on the applicable procedures as detailed in the A.I.M, discuss with their Health, Safety and Environment Manager, and review the subject in their monthly safety letter for all crews. On April 1, 2008 confirmation was received of completion of all of the above actions, with the exception of the safety letter, which will take place at the end of this month.

Incident at DAWSON CITY YT (CYDA)

C-GPWI, a Bell 206, was on a VFR flight plan to Dawson and after arrival, the pilot forgot to close his flight plan. One hour after the ETA, RCC was notified and a communication search ensued. The helicopter was located parked on the company helipad at destination.

Accident at Dease Lake, 45 NM S

The Bell 206 helicopter (C-GPWI) was being used to sling equipment into a mining camp south of Dease Lake. During one takeoff, after releasing the slung load, the tether moved aft and contacted the tail rotor. The helicopter landed hard and the main rotor chopped off the tail section. There were no injuries.

Incident at INUVIK / MIKE ZUBKO NT (CYEV)

C-GPWI, a Bell 206, was inbound to Inuvik and the pilot did not report on the mandatory frequency prior to entering the MF area. Contact was made 7 to 10 miles east of YEV. The pilot advised FSS after arrival that he was experiencing intermittent problems with his GPS.

Incident at INUVIK / MIKE ZUBKO NT (CYEV)

The pilot of C-GPWI, a Bell 206, reported 15 NM south of Inuvik (YEV) on a flight from Wiley, YT (CAJ2) requesting airport advisory service. The aircraft landed at YEV without incident at 2210Z. The pilot had not filed a flight plan or flight itinerary as specified by CARS 602.145.
