Incident at RED LAKE ON (CYRL)

While on final approach, a Bearskin Airlines Fairchild SA227-DC (C-GJVO/BLS318) from Winnipeg/James Armstrong Richardson, MB (CYWG) to Red Lake, ON (CYRL) had a landing gear indication. On a fly-over, a staff vehicle advised that the gear appeared to be down. The aircraft landed safely.

Incident at THUNDER BAY ON (CYQT)

A Bearskin Airlines Fairchild SA227-DC (C-GJVO/BLS373) from Sault Ste. Marie, ON (CYAM) to Thunder Bay, ON (CYQT) initiated a missed approach due to nose wheel indication. Aircraft landed safely, no operational impact.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. C-GJVO carried out a missed approach due to a nose landing gear (NLG) having no down and locked green light indication. Eventually the green light illuminated and the aircraft landed safely. Maintenance found the NLG Right-Hand (R/H) down-lock switch intermittent. Switch replaced and aircraft returned to service.

Incident at TIMMINS / VICTOR M. POWER ON (CYTS)

A Bearskin Airlines Fairchild SA227-DC (C-GJVO/BLS371) from Timmins, ON (CYTS) to Sudbury, ON (CYSB) rejected its take-off from Runway 21 at 1352Z due to an instrument malfunction. Aircraft vacated runway to perform engine tests. Advised unable to depart and shutting down at 1357Z.

UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Aircraft C-GJVO had a rejected take-off due to the #2 engine indicating 97% torque at take-off. Maintenance rectification as follows: Swapped bridges on torque signal conditioner from bridge 2 to 1 and calibrated as per Engine Maintenance Manual (EMM). Several runs completed and aircraft checked serviceable.

Incident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG)

TSB Report #A20C0019: C-GJVO, a Fairchild SA227-DC aircraft operated by Perimeter Aviation as Bearskin Airlines flight 312, departed the Winnipeg/James Armstrong Richardson Intl. (CYWG), MB. for Red Lake (CYRL), ON with a crew of 2 and 5 passengers on board. Shortly after takeoff, the crew received a fire warning indication on the right engine. The crew decided to shut down the engine and return to CYWG. An emergency was declared and the crew then completed a successful single engine landing. An inspection of the aircraft by the operators maintenance revealed a faulty fire probe in the right engine bay area.

A Perimeter Aviation Fairchild SA227-DC (C-GJVO/BLS384) on a flight from Sault Ste. Marie, ON (CYAM) to Sudbury, ON (CYSB) aborted take off from Runway 12 due to technical issues. No delay to operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Metro 23 C-GJVO aborted take-off due to engine fuel bypass light illuminating. Crew exited runway to conduct take-off power check and all was normal. Engine fuel bypassed due to abrupt movement of power levers. Power levers advanced normally with no further faults. No maintenance action required.

The aircraft received and correctly read back REZIN-ILEKI-OVUNI. At REZIN, the aircraft turned directly to NDB YUY. No impact on operations.

BF706 was slow to depart and BLS362 was pulled up by the tower controller.

Incident at 20 SW SIOUX LOOKOUT ON (CYXL)

Follow-up information received from Flight Operations ? Airworthiness [2016-05-17]: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The CASI contacted the operator involved for more information and was provided with additional details of this occurrence. The operator advised that this occurrence was being managed through their safety management system (SMS). The operator?s Maintenance staff determined that the right bleed air mixing valve was unserviceable and replaced it. A check of the left bleed air mixing valve was worn so it was replaced as well. A small amount of excess oil was found in the cooling turbines and this was drained out. A service difficulty report (SDR) was submitted and the aircraft was returned to service. The CASI was satisfied with the information provided, the operator?s maintenance actions taken and advised that no further follow-up action by Transport Canada, Airworthiness was required at this time.
A Bearskin Fairchild SA227-DC (C-GJVO/BLS340) from Dryden, ON (CYHD) to Thunder Bay, ON (CYQT) reported an odour of smoke in the cabin and advised they were diverting to Sioux Lookout, ON (CYXL). No impact to operations.

Incident at SIOUX LOOKOUT ON (CYXL)

A Bearskin Fairchild SA227-DC (C-GJVO / BLS361) from Sioux Lookout, ON (CYXL) to Red Lake, ON (CYRL) was given IFR clearance to CYRL. After commencing takeoff roll, takeoff was aborted before Taxiway A. Aircraft exited the manoeuvring area, shut down and had their maintenance staff investigate. Aircraft subsequently departed.

Wasaya Air (WSG992) in the zone planning Runway 34, opted to circle to the west until remains could be retrieved. Weagamow corporation (C-FLIN) in the zone, landed past debris on Runway 34, safely. Vehicle (ST24) retrieved one bird. WSG992 landed Runway 34.

Incident at KITCHENER / WATERLOO ON (CYKF) (Aerodrome, runway or taxiway shutdown)

The Bearskin Lake Air Service Ltd. Fairchild SA-227DC aircraft (C-GJVO, operating as flight BLS324) was departing on an IFR flight from Kitchener/Waterloo Regional Airport (CYKF) to Ottawa (M-CIA) (CYOW). NAV CANADA staff at Waterloo Tower reported that the flight crew had advised of a birdstrike (on the aircraft's windshield) on departure off of runway 08. The runway was closed for an inspection and Airfield Maintenance staff removed the remains of a dead bird [species identification not provided by NAV CANADA]. There was no reported damage to the aircraft. Ops. impact -- one (1) aircraft was delayed from flight training for nine (9) minutes.

Incident at SIOUX LOOKOUT ON (CYXL)

The Bearskin Lake Air Service Ltd. Fairchild SA-227DC aircraft (C-GJVO, operating as flight BLS362) had just landed after an IFR flight from Red Lake Airport (CYRL) to Sioux Lookout Airport (CYXL). NAV CANADA staff at Sioux Lookout F.S.S. reported that the aircraft had landed at 1821Z. The aircraft was taxied part way up taxiway ALPHA and stopped due to stuck brakes. A Piper PA-31 landed on runway 16 and exited on to taxiway ALPHA behind the disabled aircraft to allow a Beech B-1900 aircraft to depart off of runway 16 before the Piper PA-31 aircraft could back track the runway to exit at taxiway BRAVO. Bearskin Airlines Maintenance staff proceeded down taxiway ALPHA to free up the aircraft's brakes, allowing the aircraft to exit taxiway ALPHA to the apron at 1835Z. Ops. impact -- unknown.
UPDATE Follow-up information received from Flight Operations - West [2012/07/30]: Two (2) Civil Aviation Safety Inspectors (CASIs) from maintenance and operations reviewed this occurrence. The CASIs advised that no further follow-up is required at this time.
