Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

Air traffic services (ATS) instructed a Jazz Bombardier CL-600-2B19 (C-GQJA/JZA8503), from Fredericton, NB (CYFC) to Montreal/Trudeau, QC (CYUL) to conduct a go-around while the aircraft was approximately 2 1/2 NM on final for Runway 24L. The preceding arrival, a SkyWest Bombardier CL-600-2B19 (N902EV/SKW3130) from Chicago/OHare, IL (KORD) to Montreal/Trudeau, QC (CYUL) had missed exit A4, and a Jazz de Havilland DHC-8-402 (C-GGAH/JZA7959) from Montreal/Trudeau, QC (CYUL) to Toronto/Billy Bishop, ON (CYTZ) was lined up for departure. JZA8503 landed on Runway 24L at 1534Z.

TSB Report #A19Q0024: C-GGAH, a Jazz Aviation LP Bombardier DHC-8-400, was on flight JZA8971 from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Ottawa/MacDonald-Cartier Intl, ON (CYOW) with 4 crew members and 41 passengers on board. During the pushback from the gate at CYUL, light smoke and the smell of oil were discovered in the cabin. When the bleeds to the engines were turned off, the smoke and odour went away. The aircraft returned to the gate and passengers were transferred to another aircraft.

During its initial contact, a Jazz de Havilland DHC-8-402 (C-GGAH/JZA8712) from Montreal/Trudeau, QC (CYUL) to Quebec/Jean Lesage, QC (CYQB) indicated that it was descending to 8 000 ft. JZA8712 was seen at 7 600 ft, and air traffic services (ATS) questioned JZA8712, which then went back up to 8 000 ft, while citing a technical problem. Impact on operations: a loss of separation with a Porter Airlines de Havilland DHC-8-402 (C-GLQP/POE514) from Quebec/Jean Lesage, QC (CYQB) to Toronto/Billy Bishop, ON (CYTZ).

JZA8469 landed on Runway 23 at 1854Z without further incident. No operational impact.

At 1842Z, a Jazz de Havilland DHC-8-402 (C-GGAH/JZA8883) on a flight from Deer Lake, NL (CYDF) to Halifax/Stanfield Intl, NS (CYHZ) advised of a rejected takeoff from Runway 25. No assistance required. JZA8883 taxied clear of the runway and returned to terminal at 1843Z. No further operational impact and no reason given for rejected takeoff.

The aircraft requested that Aircraft Rescue and Fire Fighting (ARFF) meet them at the apron. No problems were reported and the aircraft departed at 2030Z. There was no operational impact.

Incident at 5NM MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL) (Aerodrome, runway or taxiway shutdown)

UPDATE: TSB Report#A17Q0199: C-GGAH, a Bombardier Dash 8-402 aircraft operated by Jazz Aviation LP was conducting flight JZA8722 from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Quebec/Jean Lesage, QC (CYQB), with 4 crew members and 73 passengers on board. During the climb out of CYUL, the crew heard a loud bang and noticed that the cabin did not pressurize. There were no door indications or any other abnormalities. The crew declared an emergency, and returned to CYUL, where the aircraft landed without further incident.
At 2205Z, a de Havilland DHC-8-402 operated by Jazz Aviation (JZA8722), flying from Montréal/Pierre-Elliott-Trudeau (CYUL), QC, to Québec/Jean Lesage (CYQB), QC, declared a ?PAN PAN PAN? and requested to return to CYUL. The aircraft reported a pressurization problem and that a loud noise was heard shortly after take-off. The aircraft requested that the emergency services be deployed. At 2233Z, the flight reported that it needed about 15 minutes to prepare for an emergency landing. The aircraft landed on runway 24R at 2304Z and departed the runway normally. Operational impact: change of runway to prevent aircraft from following JZA8722.

Incident at VANCOUVER INTL BC (CYVR)

UPDATE: TSB Report#A17P0027: C-GGAH, a de Havilland DHC-8-400 aircraft operated by Jazz Aviation Lp, was conducting flight JZA068 from Victoria Intl, BC (CYYJ) to Vancouver Intl, BC (CYVR). After landing, the flight crew was instructed to hold short of Taxiway L, however failed to comply. The aircraft conflicted with N609CZ, an Embraer 170-200LR aircraft operated by Compass Airlines as flight CPZ5742, which was taxiing for a departure on Taxiway J in the opposite direction. JZA068 was turned onto the south apron to free the passage for CPZ5742.

Incident at CALGARY INTL AB (CYYC)

UPDATE: TSB Report#A17W0032: C-GGAH, a de Havilland DHC-8-402 aircraft operated by Jazz Aviation Lp, was conducting flight JZA8225 from Calgary Intl, AB (CYYC) to Edmonton Intl, AB (CYEG). During the take-off roll on Runway 17R at CYYC, the flight crew rejected at 80 knots when several bright flashes from the number 2 engine (Pratt & Whitney Canada PW150A) occurred, along with high turbine temperatures. The engine spooled down, the crew secured the engine and taxied off the runway. The operator?s maintenance borescope inspection of the number 2 engine low pressure (LP) and high pressure (HP) turbines revealed 4 HP shroud segments that appeared to have liberated, resulting in severe and extensive damage to the HP and LP turbine blades, as well as LP vanes.
A Jazz Aviation de Havilland DHC-8-400 (JZA8225) from Calgary, AB (CYYC) to Edmonton, AB (CYEG) rejected the takeoff roll on Runway 17R due to issues with engine #2. The aircraft taxied back to the gate. No operational impact.

A Jazz de Havilland DHC 8 402 (C-GGAH/ JZA8382) from Edmonton, AB (CYEG) to Fort McMurray, AB (CYMM) exited the runway and was told to hold on taxiway C until their company had a parking spot for them on the main apron. A few minutes later, JZA8382 asked if they could taxi to the apron and hold in the de-ice bay. During the scan of the manoeuvring area prior to issuing taxi authorization, JZA8382 was observed already taxiing in.

Incident at YELLOWKNIFE NT (CYZF)

A strong Emergency Locator Transmitter (ELT) signal began at 0953Z. No known aircraft operating in the vicinity of Yellowknife, NT (CYZF). Signal was continuous for 20-40 seconds followed by a short break then continuous for 20-40 seconds, repeating the pattern throughout the duration of the ELT. Signal ended at 1036Z. Joint rescue co-ordination centre (JRCC) Trenton advised ELT was from a JAZZ Aviation de Havilland DHC-400 (C-GGAH / JZA8222) aircraft parked on the airport property. Other than difficulty hearing vehicle transmissions, not a major impact on operations.

While 30 NM east of CYUL Montreal/Pierre Elliott Trudeau Int'l, the crew declared an emergency, asked for priority and diverted to land CYUL due to an ill passenger. The landing was without incident. The aircraft was met by emergency vehicles.

The aircraft was on a VFR flight to Port Huron Airport, MI (U.S.A.). The pilot declared an emergency on 126.7 (which was acknowledged by London F.S.S.). After determining that the aircraft was over Romeo, MI (U.S.A.), London F.S.S. staff advised the pilot to switch to 121.5 and then co-ordinated communications with Lansing F.S.S. Several ATS units and aircraft heard a ""Mayday"" call as the pilot of C-GGAH reported engine problems and was attempting a landing at Romeo Airport, MI (U.S.A.) -- approximately 33 miles north of the Windsor VOR (bearing 355 degrees). Lansing F.S.S. staff advised Windsor Tower staff at 1443Z that the aircraft was down safely at Romeo Airport. The Airport Manager advised that a lot of smoke was coming from the engine.
