An Aéroclub Gatineau Ottawa Inc. Cessna 150M (C-GEEH) from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND) executed a missed approach for Runway 27 due to an Evolution Flight School Inc. Diamond DA 20-C1 (C-GEWM) from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND) being slow to vacate the runway. C-GEEH rejoined the circuit and landed without further incident.

An Acer Cold War Museum Aero L-29 Delfin (C-GGRI) from Ottawa/Gatineau, QC (CYND) to Montreal/Mirabel, QC (CYMX) and an Acer Cold War Museum Aero L-29 Delfin (C-GGRY) from Ottawa/Gatineau, QC (CYND) to Montreal/Mirabel, QC (CYMX) were in position on the threshold of Runway 27, when the second aircraft reported a mechanical issue. Pilot got out of plane and walked over to fix the aircraft for 4 minutes. Both aircraft took off at 1718Z. Impact: An École De Pilotage Évolution Inc. Diamond DA 20-C1 (C-GEWM) from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND), inbound for a full stop, was delayed in their approach by about 3 minutes by going eastbound (EB) for a long final instead of right base (RB).

Cessna 172N (C-GYTI) from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND) and a Sander Geophysics Limited Cessna 208B (C-GSGV) from Ottawa/Macdonald-Cartier, ON (CYOW) to Ottawa/Gatineau, QC (CYND), in the circuit, elected to conduct missed approaches.

A pilot of an École De Pilotage Évolution Inc. Diamond DA 20-C1 (C-GEWM) from Ottawa/Gatineau, QC (CYND) to Pembroke, ON (CYTA) on a VFR flight plan (FP) with the London flight information centre (FIC) failed to file an arrival report prior to the overdue time. A communications search yielded no results and the Joint Rescue Coordination Centre (JRCC) Trenton was advised. Pilot called shortly thereafter to close the FP.

Accident at OTTAWA / GATINEAU QC (CYND) (Aerodrome, runway or taxiway shutdown)

A 6647049 Canada Inc. Corsair FG 1D (C-GVWC) from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND) had a Runway excursion after touch and go (T/G) on Runway 09. Emergency services were advised (ADZ). The area control centre (ACC) was advised (ADZ). A vehicle (ST22) was advised (ADZ). The Runway was closed by NOTAM. Three aircraft (A/C), an Evolution Flight School Inc. Diamond DA 20-C1 (C-GEWM) from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND), a Rockcliffe Flying Club Cessna 172M (C-GSGG) from Ottawa/Rockcliffe, ON (CYRO) to Ottawa/Rockcliffe, ON (CYRO) and a Kkb Aeronefs Ltee Piper PA-28R-200 (C-FKKB) from Montreal/St-Hubert, QC (CYHU) to Montreal/St-Hubert, QC (CYHU) were re-routed to other airports. Two aircraft (A/C) were delayed approximately 10 minutes.
TSB Update #A19Q0100: a privately registered Goodyear Corsair FG 1D (C-GVWC) was on a local visual flight rules (VFR) flight from Ottawa/Gatineau, QC (CYND) with only the pilot on board. While the aircraft was conducting a touch-and-go on Runway 09 at CYND, there was a directional loss of control to the left. The pilot tried in vain to control the aircraft by using the rudder and fully applying the right brake. The aircraft made a runway excursion and ended up in a ditch that runs along the runway. The pilot was seriously injured and there was major damage to the aircraft.

The new frequency came into effect on July 19, 2018 at 7:00 a.m. (local time). The change of frequency was published in the Canada Flight Supplement (CFS) from July 19, 2018.

TSB update #A18Q0119: C-GEWM, an École de pilotage Évolution Inc. Diamond DA 20-C1, was on a local VFR flight from Ottawa/Gatineau, QC (CYND). While the aircraft was located approximately 5 NM northeast of CYND at an altitude of 4 000 ft ASL, the pilot reported a risk of collision with a drone and had to perform an evasive manoeuvre.
At 4 000 ft, an École de pilotage Évolution Inc. Diamond DA 20-C1 (C-GEWN) from Gatineau, QC (CYND) to Gatineau, QC (CYND) performed an evasive manoeuvre due to a drone at a separation distance of 100 ft. No other impact on operations.

C-GZXJ exited the runway but did not depart again.

C-GZXJ on 1/2NM final Runway 09 was asked to initiate a missed approach due to TGO409 still on the runway. C-GEWM also elected to initiate a missed approach.

Incident at OTTAWA / GATINEAU QC (CYND) (Aerodrome, runway or taxiway shutdown)

An École de Pilotage Évolution Diamond DA 20-C1 (C-GEWM), on a local flight from Ottawa/Gatineau (CYND), QC, blew a tire on Runway 27 (eastern part) upon landing. An Académie de Pilotage Internationale Cessna 172N (C-GZTE), on a local flight from Ottawa/Gatineau (CYND), QC, which was on base when the event occurred, pulled up and returned to the circuit. C-GZTE and a privately registered, amateur-built Lambair Super Lambair, on a local flight from Ottawa/Gatineau (CYND), QC (inbound from the north), were notified of the situation and landed without incident on the western part of Runway 27. Runway 27 (the eastern part, east of Taxiway A) was closed by NOTAM for about 30 min. The aircraft was towed at 2225Z.

A Cargair Ltee Cessna 152 (C-GZSS) from Montreal/St-Hubert, QC (CYHU) to Ottawa/Gatineau, QC (CYND) was inbound to join mid left downwind Runway 09. At 1NM S CYND, C-GZSS did a 360 degree turn without advising. This turn put C-GZSS in conflict with an École de Pilotage Évolution Inc. Diamond DA 20-C1 (C-GEWM) on a local flight from Ottawa/Gatineau, QC (CYND). C-GZSS was suggested to do another 360 turn south of the runway. C-GZSS acknowledged but continued overhead and the flight service station (FSS) was forced to have C-GEWM do a 360 from the downwind to avoid C-GZSS who was proceeding directly into mid-left downwind in front of C-GEWM. FSS had to request that an Académie de Pilotage Internationale Inc. Cessna 172N (C-GZTE) on a local flight from Ottawa/Gatineau, QC (CYND) to leave the circuit to avoid C-GEWM's orbit.

Incident at LONDON ON (CYXU)

The Diamond Flight Centre London Inc. Diamond Aircraft Industries DA-20-C1 Katana aircraft (C-GEWM) had just landed after a local VFR flight from Kitchener/Waterloo Regional Airport (CYKF). NAV CANADA staff at London Tower advised that the pilot reported that the aircraft had a flat tire after landing and exiting onto taxiway ALPHA. Ops. impact -- none.
UPDATE Follow-up information received from Safety Oversight - Operations [2012/02/22]: A Civil Aviation Safety Inspector (CASI) contacted the operator about this occurrence. The CASI advised that an acceptable corrective action plan was received from the company and no further action is required.

Incident at LONDON ON (CYXU) (Conflict - potential, Conflict - loss of separation, Communication navigation surveillance/air traffic system)

UPDATE Follow-up information received from Safety Oversight - Operations [2011/06/30]: A Civil Aviation Safety Inspector (CASI) reviewed this occurrence and advised that the operator provided an acceptable corrective action plan (CAP) and no further follow-up was required from their office.
The 1423063 Ontario Inc. - Crosswind Aviation Cessna 172N aircraft (C-FYFT) was on a local VFR flight from London International Airport (CYXU). The Diamond Flight Centre London Inc. Diamond Aircraft Industries DA-20-C1 aircraft (C-GEWM) was on a local VFR flight from London International Airport (CYXU). NAV CANADA staff at London Tower reported that the Cessna pilot was given an altitude restriction of 2,500 feet. The Katana pilot was restricted to not above an altitude of 2,000 feet. The Cessna aircraft's radar tag showed an altitude of 2,300 feet. When queried, the pilot stated that the aircraft's altitude was 2,500 feet. The radar tag for the Cessna aircraft continued descent to 2,200 feet when the targets passed. Traffic information was passed to the pilots of both aircraft. Ops. impact -- no other.
