A Jazz de Havilland DHC-8-301 (JZA8685) from Bagotville, QC (CYBG) to Montreal/Pierre Elliott Trudeau, QC (CYUL) on approach for Runway 24L indicated that it was crossing through wake turbulence coming from an Air Canada Rouge Boeing 767-33A (C-GHPE/ROU1533) from Halifax/Stanfield, NS (CYHZ) to Montreal/Pierre Elliott Trudeau, QC (CYUL) on final for Runway 24R. The separation was still maintained. No impact on operations.
Update TSB #A19Q0134: C-GABP, a De Havilland DHC-8-300 aircraft operated by Jazz Aviation LP, was conducting flight JZA8685 from Bagotville, Qc (CYBG) to Montréal/Pierre Elliott Trudeau Intl, QC (CYUL). As the aircraft was on approach to Runway 24R at CYUL behind a Boeing 767-300, operated by Air Canada Rouge flight ROU1533 it encountered wake turbulence, followed immediately by an uncommanded 30° roll to the left and an uncommanded roll to the right. The crew notified ATC, and continued the approach without further incident.

Incident at THUNDER BAY ON (CYQT)

The pilot of a Thunder Airlines Limited Mitsubishi MU-2B-60 (C-GZNS/THU870) from Kenora, ON (CYQK) to Thunder Bay, ON (CYQT) called at 0551Z and said they noticed some damage on the aircraft after landing. C-GZNS landed at 0517Z. The pilot advised they had hit a windrow on the right side of Runway 07 and damaged both left and right landing gear fairings. Two other aircraft, a Jazz de Havilland DHC-8-311 (C-GABP/JZA8298) from Winnipeg/Richardson Intl, MB (CYWG) to Thunder Bay, ON (CYQT) and a WestJet Encore Ltd. de Havilland DHC-8-402 (C-GENM/WEN3219) from Toronto/Pearson Intl, ON (CYYZ) to Thunder Bay, ON (CYQT), landed before the received phone call, and a Jazz de Havilland DHC-8-402 (C-GKUK/JZA8547) from Toronto/Pearson Intl, ON (CYYZ) to Thunder Bay, ON (CYQT) advised of possible debris on the runway and elected to land without a runway inspection. A WestJet Encore Ltd. de Havilland DHC-8-402 (C-GJEN/WEN3204) from Winnipeg/Richardson Intl, MB (CYWG) to Thunder Bay, ON (CYQT) had its landing delayed due to a runway inspection. No debris was found. No other operational impact.
Update TSB Report #A19C0012: C-GZNS, a Mitsubishi MU-2B-60 aircraft operated by Thunder Airlines, was conducting flight THU860 from Kenora (CYQK), ON to Thunder Bay (CYQT), ON with 2 crew members and 2 passengers on board. During the landing roll on Runway 07 at CYQT, the aircraft collided with a snow drift, and directional control of the aircraft was lost. The aircraft veered to the right, then to the left, however remained on the runway. There were no injuries, and the aircraft sustained minor damage to the landing gear fairings and doors.

During its approach for Montreal, QC (CYUL) the separation between a Porter Airlines Inc. de Havilland DHC-8-402 (C-GKQC/POE471) from Toronto/Billy Bishop, ON (CYTZ) to Montreal, QC (CYUL) and a Jazz de Havilland DHC-8-311 (C-GABP/JZA735) from Îles-de-la-Madeleine, QC (CYGR) to Montreal, QC (CYUL) was reduced to 1.8 NM and 800 ft. No other impact on operations.
Update - TSB report #A18Q0121:C-GABP, a Jazz Aviation LP Bombardier DHC-8-300, was on flight JZA735 from Îles-de-la-Madeleine, QC (CYGR) to Montreal/Pierre Elliott Trudeau Intl, QC (CYUL). C-GKQC, a Porter Airlines Bombardier DHC-8-400, was on flight POE471 from Toronto/Billy Bishop Toronto City, ON (CYTZ) to CYUL. After an operational error from the air traffic control (ATC) unit, the separation between the 2 aircraft was not maintained during the approach at CYUL. Lateral separation between the two aircraft was reduced to 1.8 NM, and vertical separation was reduced to 800 ft, while the standard separation is 3 NM or 1 000 ft.

Douglas McCurdy, NS (CYQY) at 17,000, QY altimeter 29.75. WEN3484 was issued a descent to flight level 18,000 (FL180), which is not a usable flight level for the altimeter setting.
UPDATE to narrative: An Aviation Safety Program Analyst looked into this occurrence and the narrative was replaced with "A WestJet Encore de Havilland DHC-8-402 (C-GEWR/WEN3484) from Halifax/Stanfield International, NS (CYHZ) to Sydney/J.A. Incident at Kelowna

Update: AOR 211218-V1: A Jazz de Havilland DHC-8-311 (C-GABP/ JZA8416) from Vancouver, BC (CYVR) to Kelowna, BC (CYLW) was five miles final for Runway 16 when the pilot informed the tower that the landing gear would not deploy. The aircraft executed a missed approach and then diverted to CYVR. No operational impact reported.
UPDATE: TSB Report#A17P0024: C-GABP, a de Havilland DHC-8-311 aircraft operated by Jazz Aviation Lp, was conducting flight JZA8416 from Vancouver Intl, BC (CYVR) to Kelowna, BC (CYLW). Upon selection of the landing gear down during the final approach at CYLW, the nose landing gear failed to extend. A missed approach was conducted and the decision was made to return to CYVR to conduct an alternate extension. The aircraft landed at CYVR without further incident. The operator?s maintenance discovered that the sequence solenoid valve was unserviceable. The sequence valve was replaced and the landing gear was tested serviceable.
A Jazz de Havilland DHC-8-311 (C-GABP/ JZA8416) from Vancouver, BC (CYVR) to Kelowna, BC (CYLW) could not extend their landing gear on approaching CYLW and returned to CYVR landing safely on Runway 08L at 0102Z.

As a Jazz de Havilland DHC-8-311 (C-GABP/ JZA8239) from Terrace, BC (CYXT) to Vancouver, BC (CYVR) was lining up for departure on Runway 15, Quantum West advised they spotted an oil streak on JZA8239's right engine. Flight Service Station (FSS) advised JZA8239, who aborted take-off and taxied back to ramp for visual inspection. Nothing was found and JZA8239 proceeded to take off without incident. No impact to operations.

UPDATE TSB #A16P0106: C-GABP, a de Havilland DHC-8-311, operated by Jazz as JZA262 departed Nanaimo (CYCD) for Vancouver Intl. (CYVR). During the climb out, the crew responded to a TCAS "climb" RA. The conflicting traffic was unknown and there were no targets displayed on ATC radar.

Traffic was a Victoria Flying Club Cessna C172S (C-GTDK) on a local flight from Victoria, BC (CYYJ) and a Pacific Sky Aviation Cessna 172P (C-GZMJ) on a local flight from Victoria, BC (CYYJ) one at 2,500 feet the other at 2,600 feet approximately 1.5 NM north of JZA8066.

Incident at NANAIMO AIRPORT (Incursion - runway - vehicle)

A Jazz de Havilland DHC-8-311 (C-GABP/JZA8264) from Vancouver, BC (CYVR) to Nanaimo, BC (CYCD) made initial report inbound to CYCD at a 7 mile final for Runway 16 and conducted missed approach due to vehicles on Runway. Event happened during closed hours of CYCD flight service station (FSS).
Follow-up information received from Aerodromes & Air Navigation: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The CASI contacted the operator involved for more information and was provided with additional details of this occurrence. Nanaimo plow truck was clearing snow from runway 16/34. The pilot broadcast inbound at 7 miles out on final and was advised that there were vehicles on the runway plowing snow. Radio in plow truck was malfunctioning and as a result, was unable to clear the runway in time. Pilot executed a missed approach and attempted another landing once the runway was confirmed cleared of all vehicles. Aircraft landed without incident. The CASI was advised that the Airport was managing this occurrence through their safety management system (SMS). The CASI was satisfied with the information provided, and advised that no further follow-up action by Transport Canada was required at this time.

Air traffic control (ATC) instructed a Jazz de Havilland DHC-8-311 (C-GABP / JZA8189) from Kamloops, BC (CYKA) to Vancouver, BC (CYVR) to pull up and go around because a Skylink Express Beech 1900C (SLQ768) from Vancouver, BC (CYVR) to Kamloops, BC (CYKA) was observed across the hold line for Runway 26R on Taxiway N7. SLQ768 had been instructed to hold short of Runway 26L and appeared more than 200 feet from the runway edge but across the hold line for the instrument landing system (ILS) protected area. Minimal impact on operations.

JZA8247 had received a wake turbulence cautionary prior to the event.

Incident at 6 NM E NANAIMO AIRPORT (CYCD)

UPDATE: TSB Report#A14P0160: The flight crew of the Dehavilland DHC-8, C-GABP, operated by Jazz as Flight JZA8259, performed a missed approach at Nanaimo (CYCD) due to weather. In the missed approach, the crew reported climbing to 3000 feet and was given vectors for Vancouver (CYVR). JZA8259 was observed climbing through 3100 feet as it passed abeam an American registered, N426KM, Cessna C208, operated by KA Associates LLC as Flight KEN 815, descending through 4700 feet for 4000 feet. Safety was not assured as the controller queried JZA8259 before they descended back to 3000 feet from 3400 feet. At the point where the airplanes passed abeam of each other there was 1600 feet and 1.5 NM separation.

When queried by the controller, JZA8259 descended back to 3000 feet from 3400 feet. No operational impact.

The aircraft taxied off the runway, and after assessing the situation continued to taxi and safely departed runway 26 without further incident or an impact on operations.

UPDATE: TSB REPORT#A13P0218: A Jazz DHC-8-300 C-GABP flight JZA8350 departed Vancouver for Penticton. On climb out there was a right hand elevator horn heat caution message. The flight was in icing conditions and elected to return to Vancouver. An emergency was not declared. The fault could not be duplicated on the ground. A connector was found to be contaminated with moisture. The connector was cleaned and the aircraft returned to service.
A Jazz Aviation de Havilland DHC 8 300 (JZA8350) from Vancouver, BC (CYVR) to Penticton, BC (CYYF) departed CYVR at 1551Z. The pilot requested return to CYVR for electrical / deice problem. No Emergency . Landed runway safely at 1624Z.

Incident at SMITHERS BC (CYYD) (ATM - operations)

The FSS stayed open several minutes after published hours dues to an arriving Hawkair Aviation de Havilland DHC 8-102 (C-FDNG/ HA312) from Vancouver, BC (CYVR) to Smithers, BC (CYYD) and a departing Jazz Aviation de Havilland DHC 8-301 (C-GABP/ JZA8565) from Smithers, BC (CYYD) to Vancouver, BC (CYVR).

Incident at FORT MCMURRAY AB (CYMM)

SW172 was holding short runway 25 on taxiway F. A Jazz Aviation De Havilland DHC 8 (C-GABP/ operating as JZA8380) on a IFR flight, had been cleared to land runway 25. SW172 was instructed to continue holding short and move over to taxiway E to allow AR128 unimpeded taxi on taxiway G and F. SW172 proceeded on to the runway from taxiway F and exited on to taxiway E. JZA8380 was approximately 2 1/2 Miles final. SW172 was off the runway and RPA before JZA8380 crossed the threshold. No impact to operations.

UPDATE TSB A12P0211: A Jazz DHC-8-300 C-GABP Flight JZA8278 departing Vancouver for Prince Rupert was issued instructions to 'Taxi via Golf, Hotel and Hold Short Lima'. The pilot read back the taxi clearance correctly but failed to hold short of Lima and taxied across runway 12 (which was not active) .The controller observed the aircraft was not holding short and mentioned it to the pilot.

Incident at VANCOUVER INTL BC (CYVR)

CACO reports that at 1915Z (1115PST) an Air Canada Jazz deHavilland DHC8 operating flight JZA192, IFR Vancouver (CYVR) to Kamloops (CYKA), requested to return to Vancouver due to an engine failure, the crew did not declare an emergency. The flight was 50NM North East of the Vancouver airport at the time of the incident and they landed safely at 1934Z (1134PST) with airport rescue and fire fighting (ARFF) standing by. TC Communications and TSB advised.
UPDATE from CACO: Shortly after departure from Vancouver, at 19:15Z / 14:15 EST and 50 NM northeast of CYVR, the pilot of Jazz flight 192, a de Havilland Dash-8 destined for Kamloops (CYKA) reported an engine failure and requested a return to Vancouver. No emergency was declared but airport rescue and fire fighting (ARFF) were called out for landing. The aircraft landed without incident at CYVR at 19:34Z / 14:34 EST. Chief CACO, Pacific Region System Safety Duty Officer, TC HQ Communications and TCSC notified.
UPDATE from TSB: A11P0168: The Jazz De Havilland Dash 8-300 (C-GABP) was about 50 nm northeast of Vancouver, en route to Kamloops, BC when the flight crew noticed decreasing torque on the number 2 engine. The aircraft made a single engine return to Vancouver where an uneventful landing was made. Company maintenance replaced the number 2 engine's MFC.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

JZA7676, un De Havilland Dash 8 immatriculé C-GABP, exploité par Air Canada Jazz, effectuait un vol selon les règles de vol aux instruments (IFR) depuis Montréal/Trudeau (CYUL) à destination de Charlottetown (CYYG). Vers 0200Z, l'équipage a avisé le contrôleur du centre de contrôle régional (CCR) de Montréal (CYUL), que l'aéronef avait un problème technique et a demandé à revenir à Montréal/Trudeau. Aucune urgence n'a été déclarée et aucune assistance n'a été demandée. L'aéronef a atterri sur la piste 24 gauche à 0223Z, sans encombre. Aucun impact sur les opérations ATC.*** ** *** Air Canada Jazz De Havilland Dash 8 JZA7676, registered C-GABP, was IFR from Montréal/Trudeau (CYUL) to Charlottetown (CYYG). Around 0200Z, the crew informed the Montreal (CYUL) area control centre (ACC) that the aircraft had a technical problem and asked to return to Montréal/Trudeau. No emergency was declared and no assistance was requested. The aircraft landed on rwy 24 left at 0223Z, without incident. No impact on ATC operations.

Incident at Kelowna (Conflict - IFR & VFR, Communication error, Alleged Canadian Aviation Regulations (CARs) infraction)

Air Canada Jazz flight JZA399 Dehavilland DHC 8 301, IFR Origin unknown to Kelowna was on the ILS approach for Runway16 for Kelowna descending through 6500' . An unidentified target appeared at 7000' and 3 NM directly ahead of JZA399. Traffic was passed to JAZ399 who confirmed visual contact. As JZA399 continued its approach the other aircraft also started descending and entered the Class D control zone extension without contacting ATC. JZA399 advised it was deviating East of the centerline to avoid the other aircraft. JZA399 changed to Kelowna Tower and landed without further incident. VFR aircraft, a privately registered Cessna 182G, VFR Port Hardy to Kelowna, later called Kelowna Tower and also landed at Kelowna.
UPDATE / Add Info from TSB: A10P0356 - The Air Canada Regional (Jazz) DHC-8-300 aircraft (C-GABP), operating as JZA8399, was on the ILS approach to Runway 16 at Kelowna, after a flight from Vancouver. While descending through 6,500 feet the crew observed, on TCAS and visually, an aircraft 3 nm ahead, same direction and 400 feet above. As they were overtaking the other aircraft and it was descending, the crew manoeuvred to the east of the localizer in order to avoid it. Once past the aircraft, JZA8399 re-intercepted the localizer and landed at Kelowna without further event. The other aircraft was a VFR privately-registered Cessna 182G arriving at Kelowna after a flight from Port Hardy, which had entered the Class D control zone extension without contacting ATC.
UPDATE from General Aviation: The pilot verified the events. He stated that he was unfamiliar with the Class D control zone extension at Kelowna Airport and it did not appear in his GPS. He has since thoroughly reviewed the Kelowna procedures and indicated that in the future he will not rely on the GPS as much. Oral counselling was provided. This report has been reviewed by General Aviation and no further action is required.

Incident at VICTORIA INTL BC (CYYJ)

UPDATE/ Add info from TSB: A09P0200: The Air Canada Regional (Jazz) DHC-8-300 aircraft, C-GABP, operating as JZA50, was taking off from runway 09 at Victoria on a flight to Vancouver. As take-off power was applied the take-off warning horn sounded and the captain rejected the take-off at about 20 knots. The crew determined that the warning was caused by a sticking flap actuator. The flaps were reset and the aircraft took off on runway 09 without further difficulty.
Air Canada Jazz Flight JZA50, a Dehavilland DH8C IFR Victoria to Vancouver, aborted take-off roll for Runway 09 at 1232Z and reported exiting on Taxiway S. IFR clearance was cancelled and re-validated at 1235Z when JZA50 re-entered Runway 09 and departed without further incident.

Incident at Centre de contrôle de Montréal

An emergency locator transmitter (ELT) signal was heard for approximately 10 minutes in the vicinity of the Montréal area control centre (ACC). The source of the signal was not found. A Canadian armed forces Hercules was sent to search for the source, and traced it to C-GABP, an Air Canada Jazz Dash-8.
