Incident at STEINBACH MB (CJB3)

A Harvs Air Service Ltd. Beech D95A (C-FVJC) from Steinbach (South), MB (CKK7) to Steinbach (South), MB (CKK7) was issued an IFR departure clearance, with clearance being cancelled if not airborne by the time of 1610Z. C-FVJC checked in with the Terminal controller and reported a departure time of 1616Z. No impact to operations.

Accident at STEINBACH MB (CJB3)

TSB #A18C0083: C-FVJC, a Harv's Air Service, Beech D95A was on a local training flight at the Steinbach, MB airport (CJB3) with a student and instructor pilot on board. While on approach and the landing gear selected in the down position, the landing gear extended only half way. A manual gear extension was performed, but the gear did not extend fully. The aircraft then landed on grass with the landing gear partially extended and the aircraft eventually settled onto its belly. There were no injuries, the aircraft sustained substantial damage. An inspection by the operator's maintenance of the landing system revealed a failure of the landing gear actuator gearbox.

Incident at WINNIPEG / ST. ANDREWS MB (CYAV)

A Harv's Air Service Cessna 152 (C-GONS) on a local flight from St Andrews, MB (CYAV) executed a touch and go on Runway 04. A Harv's Air Service Beech D95A (C-FVJC) on a local flight from Steinbach, MB (CKK7) was subsequently issued landing clearance for Runway 31 and passed closely behind C-GONS at the same altitude. Traffic was passed to both aircraft. No impact on operations.

Incident at STEINBACH (SOUTH) MB (CKK7)

Harv's Air Service reported to Winnipeg FIC that one of their aircraft, C-FVJC, a Beech Travel Air, was airborne and the ELT was activated. The company was in cell phone contact with the pilot, who indicated that the ELT was interfering with his radio. The pilot squawked 7600 and subsequently landed uneventfully at Steinbach. The ELT was then turned off at 0645z.
UPDATE Maintenance and Manufacturing reported that the ELT activated in flight as the pilot was flying in cruise. The pilot reported that the air was smooth at the time, and he had made a radio call shortly before the ELT activated. This ELT was subsequently removed and will be sent away to be tested; a different ELT was installed and ground checked serviceable, with no in-flight reports of any more inadvertent activations.

Accident at STEINBACH (SOUTH) MB (CKK7)

The instructor and student pilot in C-FVJC, a Beech Travel Air, were conducting circuits at Steinback South when the landing gear was raised with the aircraft still on the ground. The aircraft collapsed onto the runway and skidded to a stop. The two occupants were not injured, but there is substantial damage to the belly, flaps and propellers.
UPDATE TSB reported that the Harv's Air Ltd Beech D95A Travel Air, C-FVJC, was on a local training flight at Steinbach South, MB. During a touch-and-go landing, the landing gear retracted and the aircraft skidded to a halt on the runway with substantial damage. There were no injuries.

Incident at STEINBACH (SOUTH) MB (CKK7) (ATS operating irregularity)

C-FVJC, a Beech Travel Air, arrived at Steinbach after a VFR flight from Hearst, ON and the pilot contacted Winnipeg FIC via radio to close his flight plan. No flight plan was on file at YWG FIC. The pilot stated that he filed a flight plan with the London, ON FIC. The flight was conducted without flight following. TSB will be discussing this incident with Nav Canada.

C-GTWW was unable to adhere to a crossing restriction and separation may have been lost with C-FVJC (BE95) and Perimeter Airways 620 (BE80). Ops. Impact -- unknown.
