A Jazz Bombardier DHC-8-402 (C-FSRZ/JZA8310) on a flight from Comox, BC (CYQQ) to Vancouver Intl, BC (CYVR) was inbound for Runway 26R and cleared to 3,000, descended to 2,700. When the pilot was instructed again to maintain 3,000 the aircraft climbed back up to 3,300. The approach clearance for the aircraft was cancelled and the aircraft was vectored back for another approach. No operational impact.

Incident at Kelowna

A Jazz Bombardier DHC-8-402 (C-FSRZ/JZA8413) from Kelowna, BC (CYLW) to Vancouver Intl., BC (CYVR) aborted its takeoff due to maintenance issues.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The flight crew rejected the takeoff twice after the #2 engine torque dropped off and then went back to normal. The engine display and the Full Authority Digital Engine/Electronics Control Unit (FADEC) circuit breakers were cycled by the flight crew and re-set successfully. Engine runs were carried out to test both right FADEC channels and were serviceable.

Incident at 25NM TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

During their initial climb out, a Jazz Aviation de Havilland DHC-8-400 (JZA8926) on a flight from Toronto/Lester B. Pearson, ON (CYYZ) to Quebec/Jean Lesage, QC (CYQB) reported high oil temperature. They explained they would have to shut down an engine and would be returning to Toronto. The aircraft eventually declared and emergency. Priority was given. Aircraft Rescue and Fire Fighting (ARFF) was on standby. The aircraft landed Runway 23 at 0206Z, and taxied to the gate under its own power and without incident.
UPDATE: TSB Report#A17O0223: C-FSRZ, a Bombardier DHC-8-400 aircraft operated by Jazz Aviation Lp, was conducting flight JZA8926 from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Quebec/Jean Lesage Intl, QC (CYQB) with 4 crew members and 72 passengers on board. During the initial climb after the departure from CYYZ, the flight crew noticed that the number 2 engine oil temperature had risen to 117°C. The flight crew actioned QRH procedures and contacted the operator?s maintenance. As the temperature remained high, a decision was made to return to CYYZ. The propeller was feathered and the flight crew declared an emergency. The flight crew advised the passengers and the cabin crew of the situation, and the aircraft landed uneventfully with ARFF standing by.

A Porter Airlines de Havilland DHC-8-402 (C-GLQO/POE936) from Boston, MA (KBOS) to Toronto Billy Bishop, ON (CYTZ) elected to go around and wait for a runway inspection. Runway inspection revealed no dead birds, POE936 landed without incident.

Incident at 150NM WSW MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

TSB Update #A16Q0161: A Sky Regional de Havilland DHC-8-400 (C-FSRZ/ SKV7526) was on an IFR flight from Toronto/Billy Bishop Toronto City (CYTZ), ON, to Montréal/Pierre Elliott Trudeau (CYUL), QC. While the aircraft was about 150 NM west-southwest of CYUL, a left propeller overspeed occurred. The crew shut off the left engine, feathered the propeller and declared an emergency to air traffic control (ATC). The aircraft landed without incident on Runway 24 with aircraft rescue and firefighting (ARFF) services on alert. No one was hurt and the aircraft was not damaged.
At 0244Z, a Sky Regional Airlines de Havilland DHC-8-402 (C-FSRZ/SKV7526), from Toronto, (CYTZ), ON, to Montréal (CYUL), QC, declared MAYDAY and had to shut off its no. 1 engine. The crew decided to continue to destination (CYUL). Aircraft rescue and firefighting (ARFF) services were deployed. The aircraft landed without incident on Runway 24R at 0323Z. Joint Rescue Coordination Centre (JRCC) Trenton, Aviation Operations Centre, manager, area control centre operations (MACCO), and general manager, flight information region (GMFIR) were notified.

The tower informed the aircraft on final of the situation and asked two aircraft?a Sky Regional Airlines Embraer ERJ 170-200 SU (C-FEIQ/SKV7683), from St. John's (CYYT), NL, to CYUL, and an Air Transat Airbus A330-342 (C-GTSO/TSC397), from Paris (LFPG), France, to CYUL?for their intentions. The two aircraft decided not to land and both pulled up. Nothing was found on runway inspection.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

UPDATE TSB #A15Q0069: A Sky Regional Dash-8-400 (DH8D) (SKV7526) was on an IFR flight from Montréal/Pierre Elliott Trudeau (CYUL), QC, to Moncton/Greater Moncton International (CYQM), NB, with 74 passengers and four crew members. During the initial climb after takeoff, there was an abnormal indication that the landing gear had not retracted. The crew conducted the standard procedures and succeeded in extending the landing gear using an alternate procedure. A state of emergency was declared as a precautionary measure and aircraft rescue and fire fighting (ARFF) was alerted. The aircraft returned to land without incident on Runway 24R at CYUL. No one was injured and the aircraft was not damaged.
Update (Commercial and Business Aviation): The pilot flying the aircraft holds a foreign licence. Following the incident, it was decided that the student cannot conduct new solo flights before he gains greater knowledge of communications and the procedures to be followed in controlled airspace.

Air Traffic Control (ATC) authorized a Sky Regional Airlines de Havilland DHC-8-402 (C-FSRZ/ SKV7518) from Toronto/ Billy Bishop, ON (CYTZ) to Montreal, QC (CYUL) to 17 000ft in the holding pattern and informed both aircraft of traffic. SKV7518 descended to 16 700ft. POE417 was 4.2NM from SKV7518. ATC queried SKV7518 and the pilot confirmed the altimeter setting error and corrected immediately. SKV7518 climbed back to 17 000ft. No impact on operations.

Aircraft was given descent from FL230 to 8000 feet 55NM W/HABBS and crossed HABBS at 16000 feet whereas the standard terminal arrival (STAR) stipulates to cross HABBS at 9000 feet or below for Runway 06s. Impact, terminal control unit (TCU) instructed aircraft to do a 360 degree turn to lose altitude. No other impact.

A Sky Regional Airlines de Havilland DHC 8 402 (C-FSRZ/ SKV7516) from CYTZ to CYUL was instructed to contact the tower controller holding short of taxiway Charlie. SKV7516 was observed taxiing Charlie and Delta. No impact on operations.

The Sky Regional Airlines Inc. de Havilland DHC-8-402 aircraft (C-FSRZ, operating as Air Canada Express flight SKV7507) was concluding a scheduled IFR flight from Montréal (PETIA) (CYUL) to Toronto (BB/TCA) (CYTZ). The Porter Airlines Inc. de Havilland DHC-8-402 aircraft (C-GLQL, operating as flight POE253) was ready to depart on a scheduled IFR flight from Toronto (BB/TCA) (CYTZ) to Ottawa (M-CIA) (CYOW). NAV CANADA staff at Toronto (City Centre) Tower reported that the SKV7507 flight crew was instructed to fly their aircraft in an overshoot of its approach to runway 26 due to POE253 being on runway. Ops. impact -- unknown.

Incident at TORONTO / BILLY BISHOP TORONTO CITY AIRPORT ON (CYTZ)

The Sky Regional Airlines Inc. de Havilland DHC-8-402 aircraft (C-FSRZ, operating as Air Canada Express flight SKV7524) was preparing to depart on an IFR flight from Toronto (BB/TCA) (CYTZ) to Montréal (PETIA) (CYUL). SKV7524 was being pushed back from Gate 1 by a tow cart. While being towed, the tow cart became wedged beside the aircraft which resulted in a small puncture of the fuselage. No injuries reported.
