A Jazz de Havilland DHC-8-402 (C-FSRW/JZA8064) from Victoria, BC (CYYJ) to Vancouver, BC (CYVR), at 3 000 feet, reported a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) with a southbound Pacific Professional Flight Centre Ltd. Cessna 172N from Vancouver/Boundary Bay, BC (CZBB) to Vancouver/Boundary Bay, BC (CZBB) that was at 3 500 feet. Traffic was passed and both aircraft had each other in sight.

Incident at VANCOUVER INTL BC (CYVR)

A Jazz de Havilland DHC-8-402 (C-FSRW/JZA8059) from Vancouver, BC (CYVR) to Victoria, BC (CYYJ) departed CYVR and discovered a gear issue. The aircraft returned and landed safely. No operational impact.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Second occurrence for this event. After Landing Gear retraction the R/H Gear Door advisory, Landing Gear selector handle, and Red Gear unsafe lights remained on. ATC advised that the R/H Main Landing Gear was down. The Alternate Extension System was used and the aircraft returned to CYVR. Maintenance followed the Fault Isolation Manual, and could find no fault after repeated gear swings. Maintenance replaced the R/H Main Landing Solenoid Sequence Valve, Mechanical Sequence Valve, and performed functional and leak checks before returning the aircraft to service. SDR NO. 20190410012 has been submitted.”

A Jazz de Havilland DHC-8-402 (C-FSRW/JZA8073) from Vancouver, BC (CYVR) to Victoria, BC (CYYJ) returned to CYVR due to a gear issue. The aircraft landed safely on Runway 08L at 2346Z.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. After Landing Gear retraction the R/H Gear Door advisory light remained illuminated. The Cabin Crew confirmed that the door remained open. The flight returned to CYVR. Maintenance performed Gear Swings and found that the L/H and R/H Main Gear aft doors failed to close. The Built-In-Test for the (PSEU) Proximity Sensor Electronic Unit did not identify any faults. The PSEU was replaced and a satisfactory Landing Gear Operational Test completed. The aircraft was returned to service. SDR NO. 20190410012 has been submitted.

As a Jazz de Havilland DHC-8-402 (C-FSRW/JZA8245) from Terrace, BC (CYXT) to Vancouver International, BC (CYVR) was lining up for departure, fireworks began going off just off the threshold of Runway 33. JZA8245 proceeded to take off anyways, without incident. The fireworks continued for approximately 15 minutes. The Royal Canadian Mounted Police (RCMP) were notified.

A Government of Canada, Department of Transport Cessna 550 (C-FLZA/TGO859), from Winnipeg (CYWG), MB, to Montréal (CYUL), QC, received and read back the authorization to intercept Runway 24L. TGO859 intercepted the wrong runway and was instructed to pull up from Runway 24R in order to maintain separation with a Sky Regional Airlines de Havilland DHC-8-402 (C-FSRW/SKV7512), from Toronto/Billy Bishop (CYTZ), ON, to Montréal (CYUL), QC, which was on final for Runway 24R. No loss of separation.

Incident at BURWA intersection

A Jazz de Havilland DHC-8-402 (JZA8288) on a flight from Timmins/ Victor M. Power, ON (CYTS) to Toronto/ Lester B. Pearson, ON (CYYZ) declared a PAN PAN due to landing gear not indicating locked. JZA8288 also stated they were tight on fuel and requested Runway 05. Runway 33L was coordinated and accepted for the arrival of JZA8288 due to the north wind. JZA8288 landed without incident Runway 33L at 2357Z and was met by emergency vehicles. Several flights were held while JZA288 prepared for landing.
UPDATE: TSB Report#A17O0268: C-FSRW, a Bombardier DHC-8-402 aircraft operated by Jazz Aviation LP was conducting flight JZA8288, from Timmins/Victor M. Power, ON (CYTS) to Toronto/Lester B. Pearson Intl, ON (CYYZ). After takeoff, the landing gear would not retract. The flight crew followed the emergency checklist and performed an alternate gear extension; however, the alternate gear extension lights indicated the left gear was not locked despite showing three green. The flight crew elected to continue enroute to CYYZ. On approach to CYYZ the crew declared an emergency, and performed an uneventful landing with ARFF standing by. The operator?s maintenance performed an operational test of the primary system extension and retraction as per AMM-32-31-00-710-801. A fault was detected with LGDLK #2 sensor. The sensor was replaced IAW AMM 32-61-06, and a rigging and inductance check was performed after replacement. No fault was found and aircraft was returned to service.

Incident at 57NM SYDNEY / J. A. DOUGLAS McCURDY NS (CYQY) (ATS operating irregularity)

UPDATE: TSB Report#A17A0052: C-FSRW, a Bombardier DHC-8-402 aircraft operated by Jazz Aviation Lp, was conducting flight JZA8993 from St. John?s Intl, NL (CYYT) to Halifax/Stanfield Intl, NS (CYHZ). C-FRQP, an Embraer ERJ 170-200 aircraft operated by Sky Regional Airlines, was conducting flight SKV7626 from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Sydney/J.A. Douglas McCurdy, NS (CYQY). Both aircraft were being vectored by ATC; JZA8993 was in cruise flight at FL220, while SKV7626 was in the descent for CYQY. Approximately 55 nautical miles West of CYQY at FL220, a loss of separation occurred with the aircraft being 4.8 nautical miles apart. The separation reduced to approximately 4 nautical miles when SKV7626 descended below FL210. The required separation is 5 nautical miles laterally, or 1000 feet vertically.

Incident at GREATER MONCTON ROMEO LEBLANC INTERNATIONAL AIRPORT

After lining up, a Sky Regional Airlines de Havilland DHC-8-402 (C-FSRW/ SKV7515) from Moncton, NB (CYQM) to Montreal, QC (CYUL) on Runway 24, the controller passed the pilot traffic on the right downwind, a Canlink Aviation Diamond DA 20-C1 (C-FCUS) on a local flight from Moncton, NB (CYQM). The pilot was then cleared to take-off Runway 24. The controller then passed traffic to C-FCUS which the pilot did have in sight. After leaving 4000ft, SKV7515 advised the controller that the pilot got a Traffic Advisory from the previously mentioned traffic. No operational impact reported.

While leaving Runway 06R, a Skyservice Business Aviation Bombardier BD-100-1A10 (C-GJDU), from Québec (CYQB), QC, to Montréal (CYUL), QC, reported a dead bird on the runway. An Air Canada Airbus A321-211 (C-FLKX/ACA423), from CYUL to Toronto (CYYZ), ON, was in position and was not able to take off. A Sky Regional Airlines de Havilland DHC-8-402 (C-FSRW/SKV7527), from Moncton (CYQM), NB, to CYUL, on final, was instructed to pull up. SKV7527 landed on Runway 06L at 0003Z and ACA423 took off at 0006Z. Runway 06R was inspected and the bird was collected. No other impact on operations.

Impact: An Air Transat Airbus A330-342 (C-GCTS/TSC976), from CYUL to Punta Cana (MDPC), Dominican Republic, and a Sunwing Boeing 737-8Q8 (C-FDBD/SWG728), from CYUL to Liberia (MRLB), Costa Rica, were delayed about 10 min while an inspection was conducted and the bird remains were collected.

UPDATE TSB #A15Q0017: A Sky Regional DH8 (C-FSRW/SKV7512), from Billy Bishop/Toronto City Airport to Montréal/Pierre Elliott Trudeau Airport, was authorized to descend from 21 000 ft to 14 000 ft. At 20 300 ft, a loss of separation occurred between the SKV7512 and a Cessna 560XL (N151XL) which was conducting a holding pattern at TALNO. Separation was reduced to 4.3 NM (horizontal) and 300 ft (vertical) while the required separation was 5 NM and 1 000 ft.
Separation between an American, corporately registered Cessna 560XL (N151XL), from Gastonia (KAKH), NC, to Montréal (CYUL), QC, and a Sky Regional Airlines de Havilland DHC-8-402 (C-FSRW/SKV7512), from Toronto (CYTZ), ON to Montréal (CYUL), QC, was reduced to 4.13 NM and 300 ft while the recommended separation was 5 NM or 1000 ft. UPDATE: According to information from the Maintenance Division, the outboard right hand (R/H) MLG bearing was found damaged.
UPDATE: TSB Report#A14Q0046: The DHC-8-400, registration C-FSRW, operated by Sky Regional under flight number SKV7527 was on a IFR flight from Moncton, NB (CYQM) to Montreal Pierre Elliot Trudeau Int'l, QC (CYUL). Upon take-off, a passenger noticed outer right main wheel stop suddenly and smoke. The crew decided to continue to Montreal to burn fuel. During the approach, the crew advised Montreal ACC that they had a main wheel issue and requested the emergency equipment on standby for landing. The aircraft landed safely. The right main wheel bearing was replaced. The brake, wheel, and axle assembly were also replaced. Bombardier is investigating this incident.
A Sky Regional Airlines de Havilland DHC-8-400 (SKV7527), from Moncton, NB (CYQM) to Montréal, QC (CYUL), asked that emergency vehicles be present upon landing because it had a right landing gear problem. No emergency was declared. SKV7527 landed without incident at 2238Z. Runway 24R was inspected and reopened 5 min later. No impact on operations.

A tow vehicle (SVC270) that was towing C-FSRW, a Sky Regional de Havilland DHC-8-402, reported that it had lost its protective propeller cover while repositioning on the main apron at general aviation. Aéroports de Montréal was notified. An Air Canada flight (ACA481) was informed of the FOD while taxiing on the ground and stopped in order to prevent its engines from sucking up the object.

A Porter Airlines de Havilland DHC-8-402 (C-GKQF / POE532) from Sudbury, ON (CYSB) was cleared for a visual approach to Toronto City airport, ON (CYTZ) but was not advised of traffic (a Sky Regional Airlines Inc de Havilland DHC-8-402, C-FSRW / SKV7062), also on visual approach to CYTZ arriving from Toronto, On (CYYZ).

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL) (Aerodrome, runway or taxiway shutdown)

Sky Regional company operations called the control tower to report that C-FSRW, a de Havilland Dash 8 operated as flight number SKV7503 that was on an IFR flight from Montréal/Trudeau (CYUL) to Toronto (CYTZ), may have lost a piece upon departure from Runway 28. Impact: 3 aircraft were diverted to Runway 24L for departure. There were no delays and nothing was found on the runway after inspection.

Incident at TORONTO / BILLY BISHOP TORONTO CITY AIRPORT ON (CYTZ) (Communication navigation surveillance/air traffic system)

The Sky Regional Airlines Inc. de Havilland DHC-8-402 aircraft (C-FSRW, operating as Air Canada Express flight SKV7513) was concluding a scheduled IFR flight from Montréal (PETIA) (CYUL) to Toronto (BB/TCA) (CYTZ). NAV CANADA staff at Toronto (City Centre) Tower reported that the aircraft landed on runway 26 and exited. The flight crew was requested to contact the Ground Controller (as the aircraft crossed runway 15 without authorization prior to contacting the Ground Controller). Ops. impact -- none.
Update (Commercial and Business Aviation): A civil aviation safety inspector (CASI) looked into this incident. The CASI communicated with the operator to obtain more information, and he thought that the operator was managing this incident using its safety management system (SMS). The CASI was satisfied with the information provided and indicated that no other follow-up by Transport Canada was required for the time being.

Incident at Au sud de l'intersection KANUR

C-FSRW, a de Havilland Dash 8 operated by Sky Regional Airlines as flight number SKV7519, was on an IFR flight from Montréal/Trudeau (CYUL) to Toronto/City Centre (CYTZ). At 2012Z, the crew reported high oil pressure and requested to return to Montréal/Trudeau. No emergency was declared or priority requested. The aircraft landed without incident on Runway 24R at 2041Z.

The Sky Regional Airlines de Havilland DHC8-402 aircraft (operating as Air Canada Express SKV7520, C-FSRW) has just departed on a scheduled IFR flight from Toronto (BB/TCA) (CYTZ) to Montréal (PETIA) (CYUL). NAV CANADA staff at City Centre Tower reported that the aircraft departed at 2037Z and the flight crew reported a possible bird strike at 2045Z. Runway inspection found one dead sparrow. No reported damage to the aircraft. Ops. impact - none
