Incident at SEPT-ÎLES QC (CYZV)

At departure from CYZV, a Provincial Airlines de Havilland DHC-8-315 (C-FPAE/PVL920) from Sept-Îles, QC (CYZV) to Wabush, NL (CYWK) notified air traffic services (ATS) that it had received a traffic alert and collision avoidance system resolution advisory (TCAS RA). The aircraft did not deviate from its standard instrument departure (SID). No traffic. No impact on operations.

A Provincial Airlines de Havilland DHC-8-315 (C-FPAE/PVL1910) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Quebec/Jean Lesage Intl, QC (CYQB) reported that it had received a traffic alert and collision avoidance system resolution advisory (TCAS RA) concerning a target 200 ft above it while it was at approximately 10 000 ft. The only other radar target close to PVL1910 was 1.1 NM south, at 2 900 ft.

The aircraft was diverted to Lourdes-de-Blanc-Sablon, QC (CYBX). There was a nurse on board. An ambulance was needed on the ground. The flight service station (FSS) at Mont-Joli, QC (CYYY) called 911 services. The aircraft landed at 1745Z.

Incident at DEER LAKE NL (CYDF)

A Provincial Airlines de Havilland DHC-8-315 (C-FPAE/PVL1922) from Churchill Falls, NL (CZUM) to Deer Lake, NL (CYDF) reported a gear indication (nose gear) issue at 1430Z, did a low and over and the flight service station (FSS) confirmed that nose gear was not down. Air traffic control (ATC) was advised and the aircraft went into a hold. The aircraft was re-cleared to Stephenville, NL (CYJT) at 1540Z.

Incident at STEPHENVILLE NL (CYJT)

At 1430Z, a Provincial Airlines de Havilland DHC-8-315 (C-FPAE/PVL1922) from Churchill Falls, NL (CZUM) to Deer Lake, NL (CYDF) executed a missed approach at CYDF due to a nose gear issue. The flight was held for approximately one hour trying to resolve the issue. At 1532Z, the flight declared an emergency and advised that it was diverting to Stephenville, NL (CYJT) due to winds at CYDF. At 1600Z, the aircraft landed at CYJT with the nose gear collapsing and the aircraft coming to a rest about half way down the runway.
Update TSB Report #A18A0088: C-FPAE, a Bombardier DHC-8-300 aircraft operated by Provincial Airlines, was conducting flight PVL1922 from Churchill Falls (CZUM), NL to Deer Lake (CYDF), NL with 4 crew members and 47 passengers on board. At approximately 1430Z during the approach at CYDF, the flight crew selected the landing gear down; however, the nose gear did not extend completely. The flight crew executed a missed approach and entered a hold for approximately 1 hour, in order to resolve the problem. At 1532Z, the flight crew declared an emergency and requested a diversion to Stephenville (CYJT), NL due to deteriorating runway conditions at CYDF. The aircraft landed at CYJT at 1600Z, with the nose landing gear in a partially extended position. The aircraft came to a stop approximately half way down the runway. All on board egressed the aircraft with no reported injuries.

The Montreal, QC (CZUL) area control centre (ACC) advised the Wabush, NL (CYWK) flight service station (FSS) that a Provincial Airlines de Havilland DHC-8-315 (C-FPAE/PVL922) from Wabush, NL (CYWK) to Churchill Falls, NL (CZUM) informed them that they found bird remains on the aircraft on their arrival at CZUM. One small snowbird was found, and the information was relayed to CZUL area control centre (ACC) at 2055Z.

A Provincial Airlines de Havilland DHC-8-315 (C-FPAE/PVL921) on a flight from St. John's, NL (CYYT) to Gander, NL (CYQX) landed Runway 31 at 1519Z and reported a possible bird strike while in the flare. One snow bunting was found just off the edge of the runway in several pieces. The flight departed Runway 21 at 1536Z without further incident.

A Provincial Airlines de Havilland DHC-8-315 (C-FPAE/PVL901), from St. John's, NL (CYYT) to Deer Lake, NL (CYDF) cancelled IFR at 1058Z before entering the control zone and advised they were staying to the southeast over Grand Lake at A012 to check on an issue. No assistance was required. At 1103Z, the aircraft advised that the issue had been resolved and subsequently landed Runway 07 without incident at 1110Z.

Incident at QUÉBEC / JEAN LESAGE INTL QC (CYQB)

After takeoff from Runway 29, a PAL Airlines de Havilland DHC-8-101 (PVL1901), from Québec (CYQB), QC, to Montréal (CYUL), QC, declared an emergency and asked to return to land. Landing without incident on Runway 29 at 1920Z. The cause of the emergency was not specified. No impact.
UPDATE: TSB Report#A15Q0004: The Provincial Airlines De Havilland Dash-8-315 registered as C-FPAE, with 4 crew and 35 passengers on board was taking off from Quebec City, QC (CYQB) for a flight to Montreal, QC (CYUL) under flight number PVL1901. During the initial climb, the crew noticed smoke starting to appear in the cockpit and cabin. The engine bleed valves were turned off and the smoke stopped. The crew declared an emergency and returned to land safely in CYQB. There were no injuries and the passengers deplaned normally at the terminal.

Incident at QUÉBEC / JEAN LESAGE INTL QC (CYQB)

Jean Lesage International Airport, QC (CYQB), Aviation Incident Report OIRS #14257: The tower reported that a PAL Airlines de Havilland DHC-8-315 (C-FPAE) was on final and that it had declared an emergency in flight. A code yellow was immediately announced. The tower also reported that it was about to land so we went immediately to Runway 06 via DELTA and ALPHA with the necessary authorizations. The evacuation had already started when we neared the aircraft. The aircraft was on Runway 06 short of Runway 11/29. We received confirmation that everyone had evacuated and was doing okay (16 passengers and three crew members). It was confirmed that there was white smoke coming from the cockpit. The passengers were transported to air bridge 24 and only the pilot and co-pilot remained at our request. When we entered the aircraft, we noticed that there was smoke everywhere. No hot spot or fire was found on board. We carried out natural ventilation, and then forced ventilation using negative pressure ventilation (NPV) to evacuate smoke. The runway inspection was completed while we were in control of the intervention, shortly before the aircraft was towed. We received help from Servisair personnel to bring the aircraft to air bridge 35 in accordance with the wishes of the duty manager (DM). A Discovery mechanic showed up at air bridge 35 for aircraft inspection and asked us to stay a few minutes for the aircraft restart. We left a few minutes later as it was impossible for the mechanic to find the real problem.
TSB Report#A14Q0142: The Dash 8-300, registration C-FPAE, was operating IFR as flight PVL920 from Quebec/Jean Lesage Int'l airport, QC (CYQB) to Sept-Iles, QC (CYZV). During the take off climb, smoke was observed in the cockpit. The crew declared an emergency and requested landing at CYQB. Aerodrome Rescue and Fire Fighting (ARFF) vehicles met the aircraft Runway 06 and the passengers were deplaned. An air cycle machine was found faulty and caused bleed smoke to enter the cockpit.

A PAL Airlines de Havilland DH8C Dash 8 (300) (PVL1902), from Montréal, QC (CYUL), to Québec, QC (CYQB), en route for CYQB, declared an emergency due to an indication of smoke in the washroom and asked to return to CYUL. Arrangements were made. PVL1902 landed without incident on Runway 06L at 1042Z. Impact: A few arrivals were diverted to Runway 06R. The National Operations Centre (NOC) and Civil Aviation Contingency Operations (CACO) were notified.
UPDATE: TSB Report# A14Q0141: The Provincial Airlines Dash 8-300, registration C-FPAE, was operating IFR as flight PVL1902 between CYUL Montreal/Pierre Elliott Trudeau Int'l airport and CYQB Quebec/Jean Lesage Int'l airport. Shortly after take-off and while on climb to cruise altitude, the lavatory smoke detector alarm sounded. No smoke was observed. The crew requested a return to CYUL. The aircraft landed runway 06L without further incident. The lavatory smoke detector alarm was found to be faulty; it was replaced and the aircraft returned to service.

Incident at WABUSH NL (CYWK) (FOD (foreign object debris), Aerodrome, runway or taxiway shutdown)

A PAL Airlines de Havilland DHC 8-315 (C-FPAE/ PVL1910) from Sept-Îles, QC (CYZV) to Wabush, NL (CYWK) reported a bird strike on arrival. There was visible damage to the aircraft and runway inspection found several carcasses and aircraft debris. The runway was reopened at 2358Z. No other impact on operations.

Incident at DEER LAKE NL (CYDF)

UPDATE: TSB REPORT#A13A0083: The Provincial Airlines DHC-8 (registration C-FPAE) aeroplane was cleared an Instrument Flight Rules (IFR) approach at Deer Lake (CYDF). At the same time an EVAS BE1900 was departing for St John's (CYYT). The EVAS flight was cleared a Visual Flight Rules (VFR) departure. On descent in cloud the DHC-8 received a TCAS "Traffic Traffic" advisory" and deviated to the right for the outbound traffic and continued descent to ensure adequate separation. IFR was cancelled once below cloud and the flight landed safely.
A PAL Airlines Ltd. de Havilland DHC-8-315 (C-FPAE / PVL901) was inbound to Deer Lake (CYDF) from St. John's, NL (CYYT) when it recieved and followed a resolution advisory. Traffic was an outbound Exploits Valley Air Services Ltd. Beech 1900, from CYDF to CYYT, that departed and was operating under visual flight rules. There was no further impact and PVL901 cancelled IFR at 1019Z.

Incident at ST. JOHN'S INTL NL (CYYT)

A P.A.L Airlines Ltd. de Havilland DHC-8-315, on a flight from Goose Bay, NL (CYYR) to St. John's, NL (CYYT) cancelled IFR and requested an overshoot due to an abnormal gear indication. Did some checks and returned to land safely at 1341Z.
UPDATE: TSB REPORT#A13A0068: The Provincial Airlines Dash 8-300 (registration C-FPAE) was on final approach to St Johns (CYYT) airport and selected gear down. The gear extension took approximately 5 minutes. The crew elected to overshoot the runway and returned making an uneventful landing. Maintenance was able to reproduce this discrepancy during function checks in the hangar. It was determined that a poor wire connection at the landing gear inhibit switch was the root cause. These connections were resecured and a test flight conducted with no recurrence. The aircraft was returned to service.

An Exploits Valley Air Services Beech 1900D (C-GLHO / EV7633) from Deer Lake, NL (CYDF) to Goose Bay, NL (CYYR) at FL200 started a descent just prior to the pilot declaring mayday due engine failure. There was opposite direction traffic, a PAL Airlines de Havilland DHC-8-315 (C-FPAE / PVL922) from Goose Bay, NL (CYYR) to Deer Lake, NL (CYDF) 10 NM at FL190. Both aircraft were given turn instructions and passed with 3.1 NM lateral spacing at FL190. The pilot requested and was cleared direct CYDF and landed without further incident at time 2215Z.
UPDATE: TSB REPORT#A13A0058: The Exploits Valley Air Services Beechcraft 1900D aircraft (C-GLHO), operating as flight EV7633 was enroute at FL 200 from Deer Lake to Goose Bay. About 80nm North of Deer Lake the crew noticed a rapid increase in fuel flow on the number 1 engine (PT6A-67D) followed quickly by an excessively high inter-turbine temperature and the smell of burning oil. The crew carried out the engine intentional shutdown in flight procedures, and declared an emergency to descend to a lower altitude and return to Deer Lake. There was opposite direction traffic 10 nm at FL190. Both aircraft were given turn instructions and passed with 3.1 nm lateral spacing at FL190. Flight EV7633 landed without further incident at Deer Lake. Company maintenance found metallic debris in the engine oil and the main oil filter as well as on the chip detector. The compressor was difficult to turn by hand and it is suspected the compressor bearing failed. The engine has been removed.
UPDATE: JRCC Halifax SARSUM [S2013-00053]: EVAS7633 Emergency (491002N 0572598W - Deer Lake). Gander ACC reported a Beech 1900 declaring an emergency due to Engine Failure. Returned to Deer lake without further incident. Case Closed
