A Moncton Flight College Diamond DA 20-C1 (C-FMZZ) from Moncton, NB (CYQM) to Moncton, NB (CYQM) was instructed to break off its approach prior to the threshold for Runway 29. The aircraft did not and overflew the runway, causing a very short delay for a Jazz de Havilland DHC-8-300 (JZA8903) from Moncton, NB (CYQM) to Montreal/Trudeau, QC (CYUL) waiting to depart from Runway 29. No other impact on operations.

A Moncton Flight College Diamond DA 20-C1 (C-FMZZ) from Moncton, NB (CYQM) to Moncton, NB (CYQM) was cleared for a touch and go on Runway 24 with a Sunwing Airlines Boeing 737-800 (SWG445) from Puerto Plata, Dominican Republic (MDPP) to Moncton, NB (CYQM) 5 to 6NM final. Once C-FMZZ was observed rolling and airborne, SWG445 was cleared to land. C-FMZZ then dropped back onto the runway abruptly and requested a full stop. SWG445 was told to pull up due to C-FMZZ on the runway at approximately 1NM final.

A Moncton Flight College Diamond DA 20-C1 (C-FSXA) from Moncton/Roméo LeBlanc International, NB (CYQM) to Moncton/Roméo LeBlanc International, NB (CYQM) was on final for Runway 29 and was instructed to break off the approach to the left to rejoin left downwind. C-FSXA then turned greater than 180 degrees directly into final (i.e. the opposite direction). A Moncton Flight College Diamond DA 20-C1 (C-FMZZ) from Moncton/Roméo LeBlanc International, NB (CYQM) to Moncton/Roméo LeBlanc International, NB (CYQM) that was on final behind C-FSXA was given a turn north. C-FSXA passed 0.6NM behind C-FMZZ at the same altitude.

C-GAHQ was advised that their lights were turned off and was instructed to turn them on. No operational impact.

Incident at FREDERICTON INTERNATIONAL AIRPORT

A Canlink Aviation Diamond DA 20-C1 (C-FNGN) on a local flight from Fredericton, NB (CYFC) was instructed to line up Runway 27 at alpha for departure. The aircraft previously coordinated with ground that they wanted an intersection departure from alpha. After being told to line up, C-FNGN requested the backtrack to conduct a take off with the option on departure. Tower was unable to accommodate the request due to another aircraft 2 mile final. C-FNGN was still behind the hold short line and the aircraft was instructed to hold short and read back the instruction. Using binoculars, the tower controller could see C-FNGN's nose wheel was on the hold short line. A Canlink Aviation Diamond DA 20-C1 (C-FMZZ) on a local flight from Fredericton, NB (CYFC) on final was pulled up while still 1 1/2 mile final and -CFNGN was given the backtrack on Runway 27 for departure. No impact on operations.

A CanLink Aviation Diamond DA 20-C1 (C-FMZZ) on a local flight from Moncton, NB (CYQM) inbound from the southeast entered the CYQM Control Zone at 2302Z without an air traffic control (ATC) clearance and without first having established communication with the Tower. When the aircraft established communication with Moncton Tower in the Control Zone they were sequenced into the circuit and proceeded to land Runway 24 at 2311Z without incident.

A CanLink Aviation Diamond DA 20-C1 (C-FMZZ) on a local flight from Fredericton, NB (CYFC) was on final Runway 27 and cleared to land. A privately registered Cessna 172C from CYFC to Stanley, NS (CCW4) called in Alpha holding short. Tower controller acknowledged the Cessna 172C with "roger". The aircraft was observed crossing the hold short line. C-FMZZ was pulled for a left hand circuit. Since the Cessna 172C had already intruded on the runway protected area they were told to line up for Runway 27. The Cessna 172C departed at 1744Z with no further incident.

A CanLink Aviation Diamond DA 20-C1 (C-FMZZ) on a local flight from Fredericton, NB (CYFC) departed at 1242Z. On the climb out crew indicated high oil pressure and requested to return to land. Aircraft indicated no assistance was required. Aircraft rescue and fire fighting (ARFF) was on standby. Aircraft landed safely 1248Z. No impact on operations.

Three attempts were made to get the pilot to read back the restriction.

A Canlink Aviation Diamond DA 20-C1 (C-FMZZ) from Sydney, NS (CYQY) to Moncton, NB (CYQM) reported directed laser at 0413Z at position 45 56 49N/ 060 37 40W. The aircraft was at altitude 6500 asl, heading 260 degrees, the laser was from the ground and left of track. Laser was green in colour. Exposure was 10-15 seconds three times. The effect was temporary blindness, the aircraft banked to avoid the laser. Royal Canadian Mounted Police (RCMP) contacted.

C-FSXD broke the restriction and was observed on the east side of the river. C-FMZZ reported the traffic passing 50-100 above them. No impact on operations.

Incident at GREATER MONCTON ROMEO LEBLANC INTERNATIONAL AIRPORT (Aerodrome, runway or taxiway shutdown)

A Canlink Aviation Diamond DA 20-C1 (C-FMZZ) on a local flight from Moncton, NB (CYQM) experienced a flat tire on runway 11/29 while taxing for departure on runway 24. Moncton Flight College (MFC) sent mechanic to change flat on runway. Aircraft taxied off at 1438Z. Runway 11/29 closed for 23 minutes. No impact on operations as runway 24 was active.
Update Maintenance & Manufacturing: The wheel assembly was disassembled and inspected. A small split was found in the tube about 3/16th of an inch long. There were no marks on the tube to show that it was chaffing and there were no marks on the inside of the tire that might have caused a tear in the tube. The tire and tube were installed new in October of 2012. The aircraft had flown 370 hours since then. The tube assembly is part of an older batch of tubes manufactured in 2011. A product improvement has been completed with the tubes in 2012 improving the quality of manufacture and also decreasing failures in service.

Incident at GREATER MONCTON ROMEO LEBLANC INTERNATIONAL AIRPORT

UPDATE Airworthiness: Maintenance removed the rudder to access the electric trim actuator. The problem was duplicated on the ground. After sitting in the hangar for a while water was found dripping out of the bottom of the trim motor/gearbox. After the aircraft had a chance to warm up the trim was found to be working normally. It is believed that water had worked it?s way into the motor gearbox area, froze and prevented the motor from working. Maintenance replaced the trim actuator and released the aircraft.
C-FMZZ, Diamond DA 20 C1, departed from Runway 29 in Moncton (CYQM) for a training flight. At 14:40Z 20 miles northeast of Moncton the pilot reported a trim switch failure and declared an emergency. The aircraft returned for landing at 14:48Z. Emergency response vehicles were in position.

Incident at CYR 724

C-FMZZ, a DV20 enroute to Fredericton (CYFC) from Saint John (CYSJ) was observed by the Moncton (CYQM) Area Control Center (ACC) controller entering CYR 724 (Gagetown restricted airspace). Attempts to contact the aircraft on CYSJ frequencies were unsuccessful. The Flight Service Specialist (FSS) called the CYCX Range Control , no active live firing was underway. JZA7062, a CRJ 100 departing CYSJ called C-FMZZ on radio frequency 126.7 advising the pilot that he was in a restricted area. The aircraft was observed departing on the eastern boundary of CYR 724 at 0211Z. AOR 144691-V1 is a duplicate of this event.

Incident at FREDERICTON INTERNATIONAL AIRPORT (Incursion - runway - animal)

JZA958, IFR, was inbound for Runway 09 and was instructed to overshoot at approximately 23:30Z due to a flock of birds landing about halfway down the runway with what appeared to be a coyote crossing the runway after them. C-FSXA, Diamond DA 20 C1, was also in the circuit for Runway 09 and circled in the right downwind until the runway inspection was complete. Vehicle Red 6 was used disperse the birds and completed the inspection at 23:40Z. JZA958 was delayed about eight to ten minutes before landing at 23:43 and C-FMZZ, Diamond DA 20 C1, was delayed on the apron for approximately ten minutes.

C-FMZZ, Diamond DA 20 C1, holding short of Runway 24 on Runway 29 in Moncton (CYQM), reported a flat tire. The tire was repaired and the aircraft taxied off the runway at 18:41Z.
UPDATE Maintenance & Manufacturing: The tube was inspected and small puncture hole opposite to the valve stem was found on the tube. A new wheel assembly was installed and the aircraft was released.

C-FSXG, Beech C90A, landed on Runway 09 in Fredericton (CYFC) and was instructed to exit on Runway 33. C-FMZZ, Diamond DA 20 C1, on Taxiway "A" was issued and read back an instruction to hold short of Runway 09. C-FMZZ was observed crossing the hold short line and was then instructed to stop. There was no impact on operations.

Two aircraft were conducting local VFR circuits in Fredericton (CYFC). C-FMZZ, Diamond DA 20 C1, was cleared to back-track to position on Runway 09. C-FNJW, Diamond DA 20 C1, was cleared to perform a touch-and-go manoeuvre on Runway 09 before C-FMZZ was given departure clearance.

UPDATE Maintenance & Manufacturing: A new oil pressure gauge was installed and the system ground checked serviceable.
C-FMZZ, Diamond DA 20 C1, enroute from Fredericton (CYFC) to Moncton (CYQM), reported a low oil pressure indication. The aircraft returned to Fredericton. No emergency was declared. The aircraft arrived safely at 04:42Z.
