Accident at GANDER INTL NL (CYQX)

UPDATE According to the TSB Final Report A16A0041 authorized for release on 13 September 2017: The Exploits Valley Air Services Beechcraft 1900D (registration C-FEVA, serial number UE-126), operating as Air Canada Express flight EV7804, was on a scheduled passenger flight from Goose Bay International Airport, Newfoundland and Labrador, to Gander International Airport, Newfoundland and Labrador. At 2130 Newfoundland Daylight Time, while landing on Runway 03, the aircraft touched down right of the centreline and almost immediately veered to the right. The nosewheel struck a compacted snow windrow on the runway, causing the nose landing gear to collapse. As the aircraft?s nose began to drop, the propeller blades struck the snow and runway surface. All of the left-side propeller blades and 3 of the right-side propeller blades separated at the blade root. A portion of a blade from the right-side propeller penetrated the cabin wall. The aircraft slid to a stop on the runway. All occupants on board?14 passengers and 2 crew members?were evacuated. Three passengers sustained minor injuries. The aircraft was substantially damaged. There was no post-impact fire. There were insufficient forward impact forces to automatically activate the 121.5 MHz emergency locator transmitter. The accident occurred during the hours of darkness.
UPDATE: TSB#A16A0041: C-FEVA, an EVAS Air Beech 1900D aircraft operating as flight EV7804, was en route from Goose Bay Intl, NL (CYYR) to Gander Intl, NL (CYQX). During an attempted landing on Runway 03, the nose landing gear collapsed and the aircraft slid to a stop on the runway. All occupants on board, 14 passengers and two crew members, evacuated safely. Three passengers received minor injuries. The accident occurred at about 0000 UTC.
UPDATE: JRCC Halifax SARSUM Report#[S2016-00267]: 103 Squadron advised there was possibly an aircraft that crashed in Gander as they could see an ambulance on the runway. Joint Rescue Coordination Centre (JRCC) called Gander tower and at 0001Z on the 21st of April 2016, an EVA7804, C-FEVA went off the side of the runway on landing. Case was reported as a post event, no action was required by JRCC.

Incident at ST. JOHN'S INTL NL (CYYT)

An Exploits Valley Air Services Beech 1900D (C-FEVA / EV7793) from St. John's, NL (CYYT) to Gander, NL (CYQX) was given taxi clearance for departure via Taxiway GOLF, Runway 34 Hold Short Runway 29. Aircraft read back clearance and then taxied into Foxtrot. Aircraft was queried and reissued the correct clearance.

An Exploits Valley Air Services Beech 1900D (C-FEVA/EV7782) from Gander, NL (CYQX) to St. John's, NL (CYYT) rejected take off Runway 31 due to cockpit light indication. No operational impact. National Operations Centre (NOC): The aircraft later departed under EV7803 at 1155Z.

An Exploits Valley Air Services Beech 1900D (C-FEVA/EV7808), from Sept-Îles (CYZV), QC, to Wabush (CYWK), NL, aborted takeoff for an unknown reason and left the runway without incident. EV7808 took off without incident 1604Z. No impact on operations.

Aviation Incident Report (Sept-Îles Airport): The pilot of an Exploits Valley Air Services Beech 1900D (C-FEVA), from Wabush (CYWK), NL, to Sept-Îles (CYZV), QC, did not follow the guidance lines on the apron while taxiing to the terminal from the threshold of Runway 27.

An Exploits Valley Air Services Beech 1900D (C-FEVA / EV7791) from St. John's, NL (CYYT) to Deer Lake, NL (CYDF), at 1534Z, 40NM west of Gander, NL (CYQX) requested to divert to CYQX due to cargo door indicator. No emergency declared and flight landed without further incident at 1549Z.

Incident at 40 S GANDER INTL NL (CYQX)

Correction to narrative: Cockpit door open indicator should read cabin door annunciator.
At 1310Z, 40NM south of Gander, NL (CYQX), an Exploits Valley Air Services Beech 1900D (C-FEVA/ EV7783) from St John's, NL (CYYT) to Gander, NL (CYQX) reported a cockpit door open indicator light. The flight proceeded direct CYQX and landed without incident at 1326Z. No emergency declared. Joint Rescue Coordination Centre (JRCC) Halifax advised.
Follow-up information received from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The CASI contacted the operator involved for more information and was provided with additional details of this occurrence. The CASI was satisfied with the information provided, and advised that no further follow-up action by Transport Canada, Airworthiness was required at this time.

Incident at 50 NM NW ST. JOHN'S INTL NL (CYYT)

Update from Maintenance and Manufacturing: On takeoff out of St. John's the landing gear circuit breaker popped. The crew elected to divert to Gander for maintenance to assess the issue. Troubleshooting revealed wires chaffed to ground through the wire harness channel. Wires were repaired . Gear swings were conducted and the system tested serviceable.
An Exploits Valley Air Services Beech 1900D (C-FEVA/ EV7631) from St John's, NL (CYYT) to Deer Lake, NL (CYDF) departed St John's, NL (CYYT) at 0843Z and at 0901Z requested to divert to Gander, NL (CYQX) due being unable to retract landing gear. Flight was recleared to CYQX. Landed 0929Z. No emergency. No assistance requested.

Incident at ST. JOHN'S INTL NL (CYYT)

At 0731Z, an Exploits Valley Air Services Beech 1900D (C-FEVA/ EV7616) departed from Gander, NL (CYQX) to St John's, NL (CYYT). At 0758Z, the flight advised they were aborting approach to Runway 16, CYYT, and returning to CYQX due to a landing gear issue. Flight manually extended landing gear and landed CYQX at 0826Z with no further incident. Although flight did not declare an emergency, emergency vehicles were standing by in CYQX.
Update from Maintenance and Manufacturing: On approach in to St. John's the crew selected landing gear to the down position and the circuit breaker (CB) popped and gear did not extend. CB was reset and the CB popped again. Crew decided to return to maintenance base. On approach into Gander, the gear was manually lowered as per checklist and the aircraft landed without incident. After trouble shooting was carried out by maintenance the fault could not be duplicated. During the gear swings the LDG hydraulic pack and motor was found to be straining and the gear was slow to operate. The power pack assembly was replaced with a serviceable unit with successful function checks carried out. The aircraft was returned to service. The aircraft flew the remainder of the day with no defects reported.

Incident at GANDER INTL NL (CYQX)

An Exploits Valley Air Services Beech 1900D (EV800) on a local flight from Gander, NL (CYQX) advised breaking off approach to deal with minor issue. No assistance required. Aircraft proceeded south for 5NM. EV800 advised issue resolved and returned to land. Pilot later reported the issue was with the gear.
Update from Maintenance & Manufacturing: Aircraft registration identified as C-FEVA. The flight crew reported that the landing gear would not extend due to a popped circuit breaker. The circuit breaker was reset by the crew. The landing gear extended normally and the aircraft landed without incident. Maintenance discovered that a service valve connector was corroded. Both the connector and plug was replaced, as well as the landing gear time delay printed circuit board (PCB). Gear swings were carried out and function tested as serviceable. Test flight was also conducted with satisfactory results.

An Exploits Valley Air Services Beech 1900D (C-FEVA / EV7643) from Gander, NL (CYQX) to St. John's, NL (CYYT) departed at 1113Z. At 1117Z, the aircraft reported a problem with the flaps and requested return to CYQX. Flight was vectored for Runway 03 and landed 1134Z. No emergency no assistance requested.

Incident at 20 NE HALIFAX / STANFIELD INTL NS (CYHZ)

An Exploits Valley Air Services Beech 1900D (C-FEVA/ EV7584) from Halifax, NS (CYHZ) to Charlottetown, PE (CYYG) returned to Halifax due to a flap issue. No emergency declared. Aircraft landed at 2105Z.
Update from Maintenance & Manufacturing: After take-off the crew called for flaps up and climb power. The flaps were selected to zero, but the indicator remained at the 17 degree setting. A decision was made to return to Halifax, NS (CYHZ) and the aircraft landed without further incident. Once on the ground, the crew called maintenance to discuss issue. The crew were advised to cycle the aircraft power on/off which they did and once the power was turned back on the indicator moved to the zero position. The flaps were cycled several times with no issues noted. The aircraft continued without further incident. The aircraft has been flying from Dec 10th to the 19th with no flap anomalies noted.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ)

On departure, the controller issued a climb to 11000 feet to an Exploits Valley Air Services Beech 1900D (C-FEVA/ EV7586) from Halifax, NS (CYHZ) to Charlottetown, PE (CYYG). The pilot read back 12000 feet; the controller did not detect the discrepancy. No other traffic in the vicinity.

Incident at DEER LAKE NL (CYDF)

Update Maintenance & Manufacturing: Connector P200 of landing (LDG) gear control extension/retraction selector handle found to be loose. Connector P200 re-secured. Gear swings carried out with system function testing normally. Aircraft was returned to service.
Shortly after departure, an Exploits Valley Air Services Beech 1900D (C-FEVA/ EV7633) from Deer Lake, NL (CYDF) to Gander, NL (CYQX) reported gear issues with the area control centre (ACC) and returning to CYDF. Gear was locked in the down position. Security was advised. Aircraft changed plan and elected to continue to CYQX. No operational impact.

An Exploits Valley Air Services Beech 1900D (C-FEVA/ EV7647) from Gander, NL (CYQX) to Goose Bay, NL (CYYR) rejected takeoff for Runway 01 at 1215Z due to a cockpit environmental indication. Taxied back to the apron.
Update Maintenance & Manufacturing: Left environment indicator (ENVIRO) failed. The defect was evaluated. Ground runs were carried out , ENVIRO over pressure & temperature checks were performed. Bleed fail & ENVIRO off annunciators were tested. Pneumatic & vacuum system checked. No faults detected. Minimum equipment list (MEL) was cleared.

The aircraft landed without further incident at 1832Z. 1 aircraft was given speed reduction to final approach speed to facilitate the arrival of EV7643.

Incident at DEER LAKE NL (CYDF)

UPDATE TSB: A13A0021: The Exploits Valley Air Services Beech 1900D aircraft (C-FEVA), operating as flight EV7631, was on approach for landing at Deer Lake airport. When the landing gear was selected down one of the right main gear bulbs was not illuminated. The aircraft was flown over the runway to visually confirm the landing gear was down. An airport staff member confirmed it was down but could not verify if it was locked. EV7631 landed without further incident with ARFF on site. Company maintenance replaced the burned out bulb.
EV7631, Beech 1900D, enroute from St. John?s (CYYT) to Deer Lake (CYDF), while on approach into Deer Lake, reported an issue with the landing gear. The flight cancelled IFR at approximately 10:56Z and conducted the approach to Runway 07 without intent to land. The pilot flew past the Flight Service Station (FSS) and asked the operator to view whether the aircraft's rear landing gear appeared to be down. The FSS operator advised the pilot that the rear landing gear appeared to be down but it was difficult to tell with certainty. EV7631 circled back to line up for Runway 07 and landed at 11:06Z without incident. Aircraft Rescue and Fire-Fighting (ARFF) services were ready on scene.

An ELT signal was detected starting at 16:19Z by Gander Low Domestic Air Traffic Control outside the test period. It was later confirmed (at 17:34Z) that the ELT signal was coming from C-FEVA, Beech 1900D in Gander (CYQX).

Incident at GANDER INTL NL (CYQX) (Aerodrome, runway or taxiway shutdown)

EV174, a Beech 1900 inbound from St. John?s (CYYT) was cleared for an approach to Runway (RWY) 03. When instructed to Hold Short of RWY 03, the operator of Loader (LDR) 156 advised he was stuck approximately 50 feet from the edge of the runway. With coordination from Low Domestic, EV174 was issued a visual approach to RWY 13, landing without incident. Departure of EV7620 and C-FFUD were delayed approximately five minutes.
