Incident at NANAIMO AIRPORT

The pilot of an Alkan Air Beech 300 (C-FAKW) from Nanaimo, BC (CYCD) to Nanaimo, BC (CYCD) called to advise they inadvertently activated their emergency locator transmitter (ELT) for approximately 60 seconds, ten minutes after the hour, at CYCD. Area control centre (ACC) Shift Manager advised.

AKN183 deviated slightly south off the localizer and landed safely Runway 26L at 0254Z.

An Alkan Air Beech 300 (C-FAKW/AKN183) from Port Hardy, BC (CYZT) to Victoria, BC (CYYJ) was assigned a vector of 210 to follow a Pacific Coastal Airlines Beech 1900C (C-GIPC/PCO1525) from Vancouver, BC (CYVR) to Victoria, BC (CYYJ) when in sight. AKN183 misidentified an Airbus 320 as the Beech 1900C and turned to follow the wrong aircraft.

Twenty one miles out of Whitehorse/Erik Nielsen (CYXY) on the 076R a WestJet Boeing 737 600 (WJA142) from Whitehorse/Erik Nielsen (CYXY) to Vancouver BC (CYVR) advised air traffic control (ATC) they were turning 15 degrees for VFR traffic, an Alkan Air Beech 300 (C-FAKW) at 12000 feet. WJA142 did not receive a traffic alert and collision avoidance system (TCAS) resolution advisory (RA). Traffic was passed by YXY tower. No impact to the operation.

An Air Canada Embraer E190 (ACA290) departed Whitehorse, YT (CYXY) VFR enroute to Vancouver, BC (CYVR) via DUXAR. ACA290 reported through 14000 feet on course 7 distance measuring equipment (DME) in class B airspace without clearance. An Alkan Air Beech 300 (C-FAKW) inbound IFR to Whitehorse, YT (CYXY) from Watson Lake, YT (CYQH) passed approximately 21 nm to the north east descending to 12000 feet.

Medevac, an Alkan Air Beech 300 (C-FAKW) from Whitehorse, YT (CYXY) to Vancouver, BC (CYVR) departed CYXY runway 14R at 0614Z. Shortly after departure, the flight reported a mechanical issue and returned to CYXY. The aircraft crew was asked if any emergency assistance was required and they indicated that they required no such assistance. C-FAKW medevac landed CYXY runway 32L at 0648Z without incident. No operational impact.

Incident at WHITEHORSE/ ERIK NIELSEN INTL (CYXY)

Pilot of an Alkan Air Beech 300 (C-FAKW) on a IFR flight from Watson Lake (CYQH) to Whitehorse (CYXY) reported an unsafe gear indication. ARFF was called to standby. Aircraft landed safely and proceeded to hangar. No operational impact.
UPDATE TSB A13W0032: The Alkan Air Ltd. Beech 300, C-FAKW, was conducting a flight from Watson Lake, YT to Whitehorse, YT. When the landing gear was selected down on approach into Whitehorse, the flight crew did not observe a green light for the nose landing gear or an in transit light in the landing gear handle. The landing gear was cycled and there were no changes to the indication lights. The nose landing gear was visible in the mirror and the landing gear warning horn did not activate on power reduction. An uneventful landing was performed with ARFF standing by. After shutdown, when the master switch was cycled the nose gear landing light illuminated green. Maintenance operationally checked the landing gear system with no faults found. The aircraft was returned to service.

Incident at WHITEHORSE/ ERIK NIELSEN INTL (CYXY)

An Alkan Air Beech 300 medevac (C-FAKW) on a IFR flight from Edmonton City Centre (CYXD) to Whitehorse (CYXY) on approach CYXY runway 31L experienced an unsafe gear indication. The pilot elected to overshoot and troubleshoot the problem while orbiting the airport. ARFF standby assistance was offered and declined. After aprox. 5 min the pilot reported that the unsafe indication had been cleared and the aircraft landed CYXY runway 13R without incident. No other aircraft or vehicles were involved and operational impact was negligible.
UPDATE Maintenance & Manufacturing: Pilots reported that indication functioned normally after cycling the gear. Alkan Air maintenance did gear swings and were unable to duplicate the fault. All gear were inspected, lubricated, and rigging was checked. Gear indication wiring and switches were inspected. Gear swung again without fault. The Director of Maintenance suspects ice to be the problem.

Incident at WHITEHORSE/ ERIK NIELSEN INTL (CYXY)

The crew of C-FAKW, an Alkan Air King Air 300 with 4 people on board, executed a missed approach at Whitehorse due to an unsafe nose gear indication. The airport emergency equipment was called out. After holding for 30 minutes, they subsequently advised that the gear indication was good and they landed without further incident at 0647z.
UPDATE Maintenance and Manufacturing reported that while on approach it was noted that the nose gear down and locked light was not illuminated. After cycling the gear several times, the indication functioned normally. Maintenance swung the gear repeatedly, but were unable to duplicate the fault. The nose gear down and locked micro-switch was changed as a precaution and the aircraft was returned to service.

Incident at DAWSON CITY YT (CYDA) (Incursion - runway - animal)

UPDATE Aerodrome Safety reported that the dog owners are owners of an aircraft based at the airport and they allow the dogs to be loose and unrestrained at the airport which is in violation of CARS 302.10 (i). Airport staff have received complaints before about these dogs running loose. If there are repeat cases of the same dogs running loose on airside, then the airport will escalate the response to the incidents, culminating in the request for charges to be laid.
C-FAKW, an Alkan Air King Air, was on a 3 minute final for Runway 20 at Dawson City when 2 dogs were observed on the threshold. The crew was immediately made aware. C-FAKW landed and the dogs ran back across the threshold of Runway 20 to a vehicle parked outside the hangar. No communication with the dog owner was possible.

Incident at EDMONTON CITY CENTRE / BLATCHFORD AB (CYXD)

C-FAKW, a Beech King Air operated by Alkan Air, landed at Edmonton City Centre airport and the flight crew failed to report down and clear to Red Deer FSS. Nav Canada initiated a ground search and the aircraft was located after a ramp check. One IFR departure was delayed during the search.

Incident at WHITEHORSE/ ERIK NIELSEN INTL (CYXY) (Incursion - runway - animal)

UPDATE Aerodrome Safety reported that the dogs were rounded up and transferred to the city's by law officer. An inspection of the airport's boundary and security fence revealed a furrow or trench underneath the fencing, where it is believed the dogs accessed airport property. The hole has been plugged and life at the Whitehorse Airport minus the dogs is back to normal. This event caused the delay of one departing aircraft there were no other reported interruptions of air traffic at Whitehorse.
C-FAKW, Beech King Air 300, was delayed departing Whitehorse for 10 minutes while airport maintenance crews chased dogs off the runways.

The crew of C-FAKW, a Beech King Air, reported hitting a bird on departure from Runway 13R at Whitehorse. No remains were found by airport staff.
UPDATE Aerodrome Safety reported that this event did not cause any reported aircraft damage nor any interruption of air traffic at Whitehorse. Although the remains of the bird were not recovered it was in all likely hood a Gull.

Accident at Brampton, Ontario

Aircraft enroute Lindsay to YKF. Spoke with Toronto ACC, then contacted YKF Tower. Pilot requested and received Special VFR clearance into the YKF control zone. Controller observed, on radar, aircraft descend rapidly and disappear off the screen. No contact could be made with C-FAKW. Aircraft crashed north of Brampton. The pilot, the only occupant, was fatally injured. Ops. Impact -- unknown.
