Incident at SMITHERS BC (CYYD) (Aerodrome, runway or taxiway shutdown)

Update Maintenance & Manufacturing: The cause of this incident was the left side wing tip fuel actuator having a loose ground. The pilot had allowed the left hand engine to quit on the runway. The pilot had selected the wing tip fuel at that point thinking he had run out of fuel in his main tank. The left actuator didn't respond and the pilot didn't observe the indication light signifying this failure. The aircraft is extremely difficult to taxi on one engine due to castering nose wheel so the other engine was shut down. The corrective action was to repair the ground wire on the actuator. The pilots have reviewed their SOP's to include a function check of the tip actuators prior to commencing any flight which will require selecting the tip tank fuel.
A Tsayta Aviation Britten Norman BN2 (C-GMZP) was inbound from the East of Smithers (CYYD) and landed at 0026Z. After landing, the aircraft's engines stalled and were unable to restart them. A Northern Thunderbird Air Beech 350 (NTA205), elected to depart runway 33 (00:36Z) aware of C-GMZP's position, which was approximately 200 ft North of the Intersection of runway 15/33 and Taxiway "A". A tow vehicle was escorted by Airport vehicle STAFF 30, and the aircraft was towed off the runway at 0043Z. There was no impact to operations other than the 10 minute delay to NTA205.

Incident at VICTORIA INTL BC (CYYJ) (Aerodrome - other)

A Pacific Wing Airlines aircraft C-GYMW a Britten Norman (BN2) landed and exited RWY 09 at Victoria Intl. (CYYJ). The aircraft then taxied from Taxiway 'W' onto Taxiway 'E' and into the Shell Aerocentre without contacting ground control for clearance. There was no operational impact.

Incident at VANCOUVER INTL BC (CYVR)

A Westjet, 737 flight WJA521 reported a bird strike short final Runway 08R. Runway closed pending inspection. No birds found. A BN2, orbitted for approximately 7 to 8 minutes during closure. Approximately 6 departures delayed 5 to 10 minutes. Runway 08L closure delayed 15 minutes pending return to normal operations. WJA521 reported no damage.

Incident at VICTORIA INTL BC (CYYJ) (Incursion - runway - aircraft)

WJA 201 a B737 landed RWY 09, exited at Sierra Taxiway. Ground Controller authorized WJA 201 to proceed to apron across RWY 31. RXX 404 a BN2 on a one mile final for RWY 13 had been cleared to land. RXX 404 pulled up by airport controller when WJA 201 observed crossing the threshold of RWY 31. RWY 13 was active and appropriate indications that it was in use were in place.

Incident at VANCOUVER INTL BC (CYVR) (Conflict - potential)

While HMY airways flight HMY612 was taxiing out on taxiway H to runway 08L for takeoff in the approximate area of between M4 and M6 taxiways a Pilatus Britten-Norman Islander BN2 owned by Nav Air Charters departed and made an immediate low level steep turn off the runway and crossed in front of HMY612 at approximately 50¿. The pilot questioned the tower control and was advised that it was a ¿very unexpected¿ manoeuvre.

GBIIP BN2 take off from Goose Bay reported problem with # 1 enigne (unable to feather). No emergency declared. Flight returned Goose Bay and landed safely 1004z.

Incident at Fort Nelson, B.C. (Aerodrome, runway or taxiway shutdown)

The following aircraft departed Runway 25 Fort Nelson airport after FSS had advised the pilots that the Runway was closed: C-GPPP BN2, April 27/98 @ 2122Z; C-GPPP BN2, April 29/98 @ 1705Z; C-FUHN, C150, April 29/98 @ 1957Z, N1322B LS2A, April 29/98 @ 1718Z. C-GGHU C206 landed Runway 25 after FSS had advised the pilot of Runway closure - April 29/98 @ 1614Z.

Incident at Charlottetown PEI

C-GFBF BN2 was cleared for E LOC BC Rwy 21 approach at Charlottetown. ATC observed the aircraft deviating from his cleared track by tracking east of the localizer and queried the pilot. Pilot advised that he was conducting the cleared approach and was not having difficulties. Radar contact was lost apprx 8 nm east of the field. During further conversation with the pilot via telephone after he had returned to Iles d'la Madeleine, pilot stated that perhaps he had not followed correct procedures. Incident at Penticton, B.C. (Incursion - runway - pedestrian)

At 0930Z a person was observed at the edge of Runway 34 taking pictures of a departing BN2 N103NE, FSS had no prior knowledge . The pedestrian did not affect aircraft operations. CFR was notified, and Pedestrian was escorted from airside.

August 14, 1996

Accident at Matheson Island, Manitoba

It was reported that the BN2 (Britten-Norman Islander) C-GMOW, departed Bolton Lake for Matheson Island. The aircraft was empty and was flying at 6500 feet. There were patches of ground fog enroute. The left engine started to run rough and lose power so the pilot applied carburetor heat. The engine continued to run rough so the pilot feathered the propeller. Then the right engine began to run rough and lose power and carb heat had no effect. The pilot landed on a roadway on Matheson Island. The wing tips were damaged when they struck trees along the side of the road. No fatalities or injuries. TSB is investigating.

Incident at BAIE-COMEAU QC (CYBC) (Incursion - runway - aircraft)

PA34 departed Havre St-Pierre and was authorized for an ILS approach for runway 10 but had to overshoot since BN2 was on the runway. The BN2 landed following a VFR flight from Rimouski (without SVFR authorization). Weather M4 BKN 18 OVC VSBY 4R-F.

A public complaint was received by Duty Specialist regarding a BN2 that departed Rwy 30 and made a low level right turn approx. 300 ft. agl over the town. FSSM also witnessed GIHF in low level right turn.
