Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (ATS operating irregularity)

South Tower controller departed an Air Canada Rouge Airbus A320 (ROU1992) from Toronto/Pearson, ON (CYYZ) to Ottawa/Macdonald-Cartier, ON (CYOW) off of Runway 06L and, once ROU1992 was airborne, less than the standard wake separation existed for a medium behind a heavy, with the other aircraft being a British Airways Airbus 340-300 (BAW092) from Toronto/Pearson, ON (CYYZ) to London/Heathrow, United Kingdom (EGLL).

A cargo door on the British Airways aircraft (operating as BAW092) was being closed when it hit a loader. Ops. impact -- unknown. Damage -- unknown. T.S.B. has been advised.

Incident at near Toronto/vicinity of Lake Ontario

The British Airways Boeing 747 (operating as BAW092) was on an IFR flight from London (Heathrow) International Airport, England (U.K.) (EGLL) to Toronto (LBPIA) (CYYZ). The flight crew reported that they the #1 engine had failed at 0035Z. The flight crew requested a fuel dump on the Toronto 090ยบ radial (80-40 DME) at 0045Z. At 0106Z, the flight crew requested a diversion to New York (John F. Kennedy) International Airport, NY (U.S.A.) (KJFK) and a clearance was given to JFK. No emergency was declared. When the aircraft was over Syracuse, NY (U.S.A.) at 0136Z, the flight crew requested permission to return to Toronto instead. The aircraft landed without further incident at 0209Z.
