Incident at 30NM SEPT-ILES QC VOR/DME (YZV) (ATS operating irregularity)

A United Airlines Boeing 777-200 (UAL56) from Paris, France (LFPG) to Newark, NJ (KEWR) and an Air Canada Boeing 787-9 (ACA879) from Zurich, Switzerland (LSZH) to Toronto/Lester B. Pearson, ON (CYYZ) had less than required separation after air traffic control (ATC) issued a climb clearance to UAL56.
TSB #A19Q0054: N788UA, a United Airlines Boeing 777-200, was on instrument flight rules (IFR) flight UAL56 from Paris/Charles de Gaulle, France (LFPG) to Newark/Liberty Intl, NJ (KEWR). C-FRTU, an Air Canada Boeing 787-9, was on instrument flight rules (IFR) flight ACA879 from Zürich-Kloten, Switzerland (LSZH) to Toronto/Lester B. Pearson Intl, ON (CYYZ). UAL56 was level at FL360, while ACA879 was also level at FL380. The two aircraft followed headings that were similar, but divergent. Approximately 30 NM southwest of Sept-Îles, QC (CYZV), UAL56 was cleared to climb to FL380. There was a loss of separation when UAL56 established itself at FL380 and the horizontal distance between the 2 aircraft was reduced to 8.8 NM. The required separation was 10 NM horizontally, or 1 000 ft vertically.

Two aircraft on final approach, an Air Transat Airbus A310-304 (C-GTSW/TSC60) on a flight from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Toronto/Lester B. Pearson Intl, ON (CYYZ) and an Air Canada Boeing 787-9 (C-FGHZ/ACA879) on a flight from Zurich Intl, Switzerland (LSZH) to Toronto/Lester B. Pearson Intl, ON (CYYZ) were sent around and AAL2676 subsequently departed without further incident. Transportation Safety Board (TSB) advised.

Pearson Intl, ON (CYYZ) due to spacing on final with an American Airlines Airbus A319 (AAL2676) on a flight from Miami Intl, FL (KMIA) to Toronto/Lester B. Pearson Intl, ON (CYYZ) landing ahead. ACA879 returned to land safely. No impact to operations.

Pearson Intl, ON (CYYZ) on final Runway 23. No impact to operations, the flight landed safely.

The sector notified the shift manager that an Air Canada Airbus A330-300 (ACA879) from Zurich, Switzerland (LSZH) to Toronto/Pearson Intl, ON (CYYZ) had reported a passenger-related medical emergency on-board. The tower and the Greater Toronto Airports Authority (GTAA) Integrated Operations Control Centre (IOCC) were notified. ACA879 landed on Runway 24R at 1754Z and taxied to the gate without further incident.

Incident at In the vicinity of: Basel (LFSB)

TSB #A19F0111: C-FVLU, a Boeing 787-9 aircraft operated by Air Canada, was conducting flight ACA879 from Zurich-Kloten (LSZH), Switzerland to Toronto/Lester B. Pearson Intl (CYYZ), ON with 11 crew members and 175 passengers on board. Shortly after the departure from LSZH, the flight crew received a C1 Hydraulic Pressure message, followed a Generator Control L1 EICAS message, which caused the associated generator to trip offline. The flight crew declared a PAN PAN and, after executing the checklist, the system successfully returned online. The flight crew cancelled the emergency, and continued to destination without further incident.

An Air Canada Airbus A330-343 (C-GFAF/ACA879) from Zurich, Switzerland (LSZH) to Toronto/Pearson, ON (CYYZ) initiated an overshoot to CYYZ Runway 05 due to an unstable approach and still being on terminal frequency. A Jazz Aviation de Havilland DHC-8-402 (JZA8936) was given takeoff clearance before a 2 minute wake turbulence delay. ACA879 landed on Runway 05 at 1738Z without incident.

An Air Canada Boeing 777-300 (ACA879) from Zurich, Switzerland (LSZH) to Toronto/Lester B. Pearson International, ON (CYYZ) was authorized for and correctly read back a clearance to descend to F340. A few moments later, an Air China Ltd. Boeing 787-9 (B1466/CCA879) from Beijing Capital International, China (ZBAA) to Montreal/Pierre Elliott Trudeau International, QC (CYUL) was observed descending through FL350 for FL340. When questioned, CCA879 thought the authorization was intended for them. No operational impact.

Incident at 96NM E QUÉBEC / JEAN LESAGE INTL QC (CYQB)

UPDATE: TSB Report#A17F0235: C-GFAH, an Airbus 330-300 aircraft operated by Air Canada, was conducting flight ACA879 from Zürich-Kloten, Switzerland (LSZH) to Toronto/Lester B. Pearson Intl, ON (CYYZ) with 11 crew members and 240 passengers on board. During cruise flight, one of the cabin crew members became ill and was unable to continue duties. Positions were reassigned among the other cabin crew members for the remainder of the flight.
An Air Canada Airbus A330-300 (ACA870) on a flight from Zurich, Switzerland (LSZH) to Toronto/Lester B. Pearson, ON (CYYZ) declared a medical emergency onboard. Emergency services arranged by Air Canada. The aircraft landed Runway 15L without incident at 1619Z. No operational impact.

The Air Canada Airbus A-330-300 aircraft (operating as flight ACA879) had just landed after a scheduled IFR flight from (LSZH) to Toronto (LBPIA) (CYYZ). The Air Canada Airbus A-320 aircraft (operating as flight ACA161) was ready to depart on a scheduled IFR flight from Toronto (LBPIA) (CYYZ) to Vancouver International Airport (CYVR). NAV CANADA staff at Toronto Tower advised that the ACA034 flight crew was instructed to overshoot their approach to runway 05. The previous arrival (ACA879) had rolled out longer than expected (to taxiway BRAVO) with ACA161 waiting in position on runway 05 for departure. Ops. impact -- none.

The crew of ACA879, a Boeing 767 operated by Air Canada, reported having heard two distress calls (MAYDAY) at 30-second intervals south of Bagotville (YGB). The origin of the calls is still unknown. The Trenton rescue co-ordination centre (TR RCC) was notified.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Overshoot/missed approach, Bird strike, Aerodrome, runway or taxiway shutdown, Aerodrome - operations)

Follow-up information received from Maintenance & Manufacturing [2005/11/10]: A Civil Aviation Safety Inspector (CASI) contacted the operator's Quality Assurance staff for more information on this occurrence. The CASI was advised that this incident was captured in the aircraft's Journey Log (Part 2) as item No. 462748. Maintenance staff found evidence of a single bird strike (granging blow) on the right underside of the leading edge near the boot. The aircraft was inspected IAW 05-51-25. There was no damage to the engines, landing gear or leading edge of the wing. The aircraft was returned to service.
The (Air Canada) Jazz Air Limited Partnership Bombardier Regional Jet aircraft (C-FDJZ, operating as JZA8641) was on a scheduled IFR flight from Newark (Liberty) International Airport, NJ (U.S.A.) (KEWR) to Toronto (LBPIA) (CYYZ). The flight crew reported flying through bird activity while landing on runway 24L. The runway was closed for an inspection and the remains of a dead bird [species not reported] were recovered. Ops. impact -- three (3) following arriving aircraft (ACA879, ACA709 and WJA448) had to overshoot (due to the runway closure).

Initial information from T.S.B. Initial Notification (#A05F0140): The Air Canada Boeing 767-233 aircraft (C-GDSS, operating as flight ACA879) was preparing to depart on a scheduled IFR flight from Zurich-Kloten International Airport, Switzerland (LSZH) to Toronto (LBPIA) (CYYZ). There was a fire in the aft galley oven while the aircraft was taxiing out. The cabin crew extinguished the fire with a halon extinguisher. The aircraft returned to the gate where Maintenance staff found grease around the heat source. The oven was deactivated and the aircraft returned to service.

ACA879 A340 initiated a climb from FL350 to FL360 without clearance. UAL971 was same direction traffic at FL360 aprx 10 miles in trail. Controller observed ACA879 in the climb and asked pilot for confirmation. Pilot did not respond but aircraft was observed descending to FL350. Report filed. TSB case closed.

En-route from Zurich (Switzerland) to Toronto (LBPIA), the crew delared an emergency with suspected blown tires when the aircraft was 100 miles northeast of the airport. Closer in, the crew reported a no gear down indication on the left main landing gear. ERS were requested to stand by for the uneventful landing on runway 33 at 2049Z. Ops. impact -- a number of aircraft were held while ACA879 was given priority on approach and the runway configuration was changed. Damage -- left main tire blown; rest unknown.
