Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Aerodrome, runway or taxiway shutdown)

An Air Canada Embraer ERJ 190-100 IGW (C-FHJU / ACA692) from Toronto, ON (CYYZ) to St. John's, NL (CYYT) reported a possible bird strike on departure from Runway 24R. Arrivals were moved to Runway 24L and no departures were impacted. Nothing was found upon inspection.

Incident at ST. JOHN'S INTL NL (CYYT) (FOD (foreign object debris), Aerodrome, runway or taxiway shutdown)

On a field inspection, STAFF 23 (vehicle) found a flock of dead birds (approximately 10) on Runway 29 abeam the tactical air navigation aid (TACAN) site. Two aircraft, an Air Canada (ACA692) and a Provincial Airlines (PVL908), had recently landed Runway29 but neither aircraft had reported a bird strike. Both aircraft were advised. Runway 29 was closed until 2030Z to remove the foreign object debris (FOD).

Incident at ST. JOHN'S INTL NL (CYYT)

An Air Canada Airbus A320-211 (C-FPWD/ ACA692) from Toronto, ON (CYYZ) to St. John's, NL (CYYT) overshot Runway 34 due to having trouble tracking the beacon for the non-directional beacon (NDB) approach. The aircraft was too high on the approach and elected to overshoot. ACA692 Landed safely at 1931Z. The pilot believed it was due to an equipment on board the aircraft. No impact on operations.

A field foreman in St. John's, NL (CYYT) advised that a sweeper was disabled on the threshold of Runway 11 at 2009Z. Runway was closed and reopened at 2025Z. The arrival for an Air Canada Embraer ERJ 190-100 IGW (C-FHOY / ACA692) from Toronto, ON (CYYZ) to St. John's, NL (CYYT) was delayed approximately 20 minutes.

Air Canada (ACA692), when approximately 15 west of Trenton, ON (CYTR) at FL290 reported hearing an apparent distress call on guard from 'helicopter SWL', a weak signal and then out of range. No other reports or any emergency locator transmitter (ELT) received. Joint rescue co-ordination centre (JRCC) advised.

An Airbus A319 operated by Air Canada as flight number ACA692 was on an IFR flight from Toronto (CYYZ) to St-John (CYYT). At 1640Z, the crew reported technical problems requiring the aircraft to stop at Montréal/Trudeau (CYUL). No emergency was declared. ACA692 landed on Runway 24R without incident. No impact on ATC operations.

The Air Canada Embraer ERJ 170 200 SU aircraft (operating as ACA692) was departing on a scheduled IFR flight from Toronto (CYYZ) to St. John's (CYYT). After departing, the flight crew advised ATC that they suspected the aircraft had struck a bird. Airport staff inspected the runway and found no remains.

Incident at ST. JOHN'S INTL NL (CYYT) (Conflict - potential, Alleged Canadian Aviation Regulations (CARs) infraction)

ACA692 landed on Runway 11 in St. John?s (CYYT) at 20:49Z and was instructed to exit onto Runway 20 via the second right and to hold short of Runway 34 for traffic awaiting departure off Runway 34 (SPR1915). Weather at the time was 1/4SM in light rain and fog with a vertical visibility of 100 feet. ACA692 was observed on the Airport Surface Detection Equipment (ASDE) vacating Runway 16 instead of Runway 20, opposite to SPR1915 awaiting departure on Runway 34. ACA692 was instructed to exit Runway 16 and the SPR1915 departure was delayed by three minutes.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Wake turbulence/vortices)

The Air Canada Boeing 777-333ER aircraft (operating as flight ACA001) was departing on a scheduled IFR flight from Toronto (LBPIA) (CYYZ) to Narita International Airport, Chiba (Japan) (RJAA). The Air Canada Airbus A-319 aircraft (operating as flight ACA692) was departing on a scheduled IFR flight from Toronto (LBPIA) (CYYZ) to St. John's International Airport (CYYT). NAV CANADA staff at Toronto Tower advised that ACA001 departed off of runway 33R due to length requirements. ACA692 commenced their departure off of runway 06L after ACA001 had passed the taxiway TANGO intersection. The ACA692 flight crew subsequently complained of jet blast as they passed abeam runway 33R. Both aircraft continued without incident. Ops. impact -- unknown.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Conflict - potential, Communication navigation surveillance/air traffic system)

The Air Canada Airbus A-320 aircraft (operating as flight ACA692) was ready to depart on a scheduled IFR flight from Toronto (LBPIA) (CYYZ) to St. John's International Airport (CYYT). An unknown aircraft [no information supplied by NAV CANADA] was approaching to land on the same runway that ACA692 was about to depart off of. NAV CANADA staff at Toronto Tower advised that at 1600Z, the ACA692 flight crew was instructed to line their aircraft up and wait on runway 24R. The flight crew complied. The flight crew were then given their take-off clearance. The pilot responded that "he needed a minute." With the landing traffic now short final, the ACA692 flight crew was instructed to taxi their aircraft and exit the runway at taxiway DELTA with no delay. The arriving traffic on short final landed without incident. Ops. impact -- unknown.

The Air Canada Embraer ERJ-190-100-IGW aircraft (operating as flight ACA692) was departing on a scheduled IFR flight from Toronto (LBPIA) (CYYZ) to St. John's International Airport (CYYT). NAV CANADA staff at Toronto Tower reported that the flight crew advised of a possible birdstrike while departing off of runway 33L at 1600Z. The runway was closed for an inspection and the intact remains of one (1) swallow were recovered. Ops. impact -- unknown.

The Westjet Airlines Limited Boeing 737-700 series aircraft (operating as flight WJA2669) was concluding a scheduled IFR flight from Providenciales International Airport, Turks and Caicos (MBPV) to Toronto (LBPIA) (CYYZ). The NetJets Aviation, Inc. Cessna 650 Citation III aircraft (operating as flight EJA642) had just landed after an IFR flight from Charlotte/Douglas International Airport, NC (U.S.A.) (KCLT) to Toronto (LBPIA) (CYYZ). The Air Canada Airbus A-320 aircraft (operating as flight ACA592) had just landed after an IFR flight from Las Vegas (Mc Carran) International Airport, NV (U.S.A.) (KLAS). WJA2669 was overshot on the approach to runway 33L at 0046Z. EJA642 was two (2) aircraft ahead and was slow to exit onto taxiway FOXTROT2 and stopped on the exit. ACA692 (following EJA642) consequently had to exit at taxiway FOXTROT4 resulting in a longer runway occupancy. WJA2669 was instructed to overshoot.

At approximately 03:56Z, the aircraft was observed crossing the hold line and still moving. The aircraft was instructed to stop and complied. The aircraft stopped south of B/02 intersection. Traffic was ACA692, five nautical miles final for Runway 29. No action was necessary. Nil TSB.

Incident at Approximately 90NM west of LEXAK

ACA692, Embraer ERJ 190, enroute from Toronto (CYYZ) to St. John?s (CYYT), reported the failure of the primary VHF NAV/COMM equipment and the ATC lost transponder interrogation. The aircraft regained communication and radar identification with their secondary equipment. There was no traffic and no assistance was required. The aircraft landed at 19:45Z without further incident. Nil TSB.
